Adenosine Deaminase (ADA) – Synovial Fluid is a specialized biochemistry test that measures adenosine deaminase enzyme activity in fluid collected from a joint. This joint fluid is known as synovial fluid.
ADA is an enzyme involved in purine metabolism and is associated with lymphocyte activity during cellular immune responses. Increased ADA activity in synovial fluid may support the evaluation of tuberculous arthritis, a form of tuberculosis affecting a joint.

Synovial fluid is a lubricating fluid found inside movable joints such as the knee, hip, shoulder, ankle, elbow, and wrist. It reduces friction between joint surfaces and supports smooth movement.
Joint-fluid accumulation can occur due to bacterial infection, tuberculosis, inflammatory arthritis, crystal-related arthritis, injury, bleeding, or degenerative joint disease. Laboratory analysis helps doctors investigate the cause.
Tuberculous arthritis develops when Mycobacterium tuberculosis affects a joint. It commonly produces slowly progressive pain, swelling, stiffness, and reduced movement. Fever and other general tuberculosis symptoms may be mild or absent.
Large weight-bearing joints such as the hip and knee are frequently affected, although tuberculosis can involve other joints. Delayed diagnosis may lead to cartilage damage, bone destruction, deformity, or loss of joint function.
ADA converts adenosine into inosine as part of purine metabolism. Its activity is associated with lymphocytes, particularly T lymphocytes involved in immune responses against tuberculosis. Increased cellular immune activity within an infected joint may raise synovial-fluid ADA.
An elevated synovial-fluid ADA result may increase clinical suspicion of tuberculous arthritis when symptoms, imaging, exposure history, and other fluid findings support the diagnosis. However, diagnostic cut-off values may vary according to the laboratory method and patient population.
An elevated ADA result does not independently confirm joint tuberculosis. Increased values may also occur in bacterial septic arthritis, rheumatoid arthritis, crystal-related arthritis, and other inflammatory joint disorders.
A normal or low ADA result does not completely exclude tuberculous arthritis. Disease stage, previous treatment, immune status, specimen condition, and analytical sensitivity may affect the measured activity.
Additional synovial-fluid investigations may include appearance, viscosity, cell count and differential count, crystals, glucose, protein, Gram stain, bacterial culture, acid-fast bacillus smear, mycobacterial culture, and TB molecular testing.
Mycobacterial culture, molecular detection, or synovial biopsy with histopathological examination may provide more direct evidence of tuberculosis. X-ray, ultrasound, CT, or MRI may also be recommended depending on the affected joint.
Synovial fluid is collected through arthrocentesis. During this procedure, a trained healthcare professional cleans the skin and introduces a sterile needle into the joint to withdraw fluid. Ultrasound guidance may be used for deeper or difficult-to-access joints.
Blood contamination, insufficient volume, incorrect container use, delayed processing, or improper storage may affect specimen suitability and result reliability. The laboratory should receive accurate information about the joint and specimen source.
A hot, severely painful, rapidly swollen joint accompanied by fever or illness may indicate septic arthritis. This requires urgent medical evaluation and should not wait for routine test booking or an ADA result.
Results should always be interpreted with symptoms, examination findings, imaging, routine synovial-fluid analysis, microbiology, and histopathology when performed. Treatment should not be initiated or excluded solely on the basis of ADA.
