Albumin – Serum is a routine blood test that measures the concentration of albumin in serum. Albumin is the most abundant protein in blood plasma and is produced mainly by the liver.

Albumin is produced by the liver and released into the bloodstream. It contributes significantly to oncotic pressure, which helps keep fluid inside blood vessels and prevents excessive leakage into surrounding tissues.
Albumin also acts as a carrier protein. It binds and transports hormones, bilirubin, fatty acids, calcium, vitamins, and several medicines. Changes in albumin concentration may therefore influence the measured or biologically active levels of some substances.
A low serum albumin level is known as hypoalbuminemia. It may occur when the liver produces less albumin, the kidneys lose albumin into urine, the digestive system does not absorb enough nutrients, or the body experiences significant inflammation.
Possible causes of reduced albumin include chronic liver disease, cirrhosis, nephrotic syndrome, chronic kidney disease with protein loss, malnutrition, malabsorption, inflammatory conditions, severe infection, burns, and certain gastrointestinal disorders.
Low albumin may contribute to swelling of the feet and legs, facial puffiness, abdominal fluid accumulation, or fluid around the lungs. However, these symptoms can have several causes and require complete medical evaluation.
A high serum albumin result is less common and is usually associated with dehydration or reduced plasma water. Certain medicines and prolonged application of a tourniquet during blood collection may also influence the measured concentration.
Serum albumin alone cannot diagnose a specific liver, kidney, or nutritional disorder. Results are commonly interpreted with total protein, globulin, albumin-to-globulin ratio, bilirubin, liver enzymes, serum creatinine, eGFR, urine albumin, urine protein, and clinical findings.
Albumin has a relatively long half-life, so its concentration may not change immediately during an acute illness. Inflammation can reduce albumin independently of nutritional intake. Therefore, it should not be used as the sole marker of malnutrition.
Pregnancy and fluid administration can lower measured albumin through dilution. Dehydration may increase the concentration. The patient’s hydration and clinical condition should therefore be considered during interpretation.
