Aldolase is a blood test that measures the activity of the aldolase enzyme in serum. Aldolase helps break down glucose to produce energy and is present in many body tissues, particularly skeletal muscles, the liver, and the brain.

When muscle cells are inflamed, injured, or damaged, aldolase may enter the bloodstream and produce an increased result. The test is primarily used as a supportive investigation for muscle disorders, especially inflammatory myopathies. It is usually interpreted with creatine kinase, clinical symptoms, physical examination, and other laboratory investigations.

Detailed Information

Aldolase is an enzyme involved in glycolysis, the metabolic pathway through which glucose is broken down to generate energy. There are several forms of aldolase in the body. Aldolase A is found mainly in skeletal muscle and red blood cells, while other forms are more closely associated with the liver and other tissues.

Skeletal muscles contain relatively high concentrations of aldolase. Muscle-cell inflammation or injury can release the enzyme into the bloodstream. For this reason, elevated serum aldolase may be observed in conditions that damage muscle fibres.

Inflammatory myopathies are a group of disorders in which immune-system activity causes muscle inflammation and weakness. These include polymyositis, dermatomyositis, immune-mediated necrotising myopathy, and overlap myositis associated with connective-tissue diseases.

In some patients with inflammatory muscle disease, aldolase may be elevated even when creatine kinase is within the laboratory reference range. Therefore, the test can provide complementary information, but it should not replace creatine kinase or a complete clinical assessment.

An elevated aldolase result is not specific to one condition. It may occur with muscle trauma, strenuous exercise, muscular dystrophy, certain infections, medicine-related muscle injury, myocardial injury, liver disease, or disorders that cause breakdown of muscle tissue.

Low aldolase levels are usually not considered clinically significant. Interpretation primarily focuses on whether the enzyme is elevated and whether the result matches the patient’s symptoms and other findings.

Doctors may compare aldolase with creatine kinase, AST, ALT, lactate dehydrogenase, and myoglobin. Because some of these markers are also present in the liver and other tissues, interpreting their pattern can help identify the likely source of elevation.

Additional evaluation may include inflammatory markers, antinuclear antibodies, myositis-specific antibodies, thyroid testing, electrolyte assessment, electromyography, MRI of affected muscles, genetic tests, or muscle biopsy.

An elevated aldolase result cannot independently diagnose myositis, muscular dystrophy, or another muscle disease. A normal result also does not exclude all muscle conditions, especially disorders that do not cause active muscle-cell breakdown.

Exercise, intramuscular injections, trauma, seizures, and recent muscle procedures may temporarily affect muscle-enzyme concentrations. These factors should be communicated to the treating doctor.

Serial aldolase measurements may sometimes help assess changes during treatment, but clinical strength, symptoms, creatine kinase, imaging, and other investigations remain important. Small changes should be interpreted carefully because biological and analytical variation may occur.

Reference intervals vary according to the analytical method, age, and laboratory population. The result should always be interpreted using the reference range printed on the individual report.
