About this test

The AMYLASE - ASCITIC FLUID test measures the activity of amylase in fluid collected from the abdominal cavity. Ascitic fluid accumulates between the layers of tissue lining the abdomen and its organs. Ascites can develop for many reasons, including chronic liver disease, heart failure, kidney disease, infection, malignancy and pancreatic disorders. Measuring amylase helps doctors determine whether pancreatic enzyme leakage or another amylase-related process may be contributing to the fluid accumulation.

Amylase is an enzyme that helps digest carbohydrates. It is produced mainly by the pancreas and salivary glands. When the pancreatic duct is disrupted, a pancreatic pseudocyst leaks or severe pancreatic inflammation is present, enzyme-rich pancreatic fluid may enter the abdominal cavity. This can produce a substantially elevated ascitic-fluid amylase result. The test is interpreted with serum amylase, fluid protein, albumin, cell count, culture and imaging findings.

Possible symptoms include increasing abdominal swelling, pain, nausea, reduced appetite, weight loss, breathlessness or recurrent fluid accumulation.

High amylase activity can support pancreatic ascites, especially when other fluid characteristics and clinical findings are compatible. Pancreatic ascites may result from disruption of the pancreatic duct or leakage from a pseudocyst. Imaging such as CT, MRI, MRCP or endoscopic retrograde cholangiopancreatography may be needed to identify the location and cause of a leak.

Ascitic-fluid amylase can also rise in some cases of intestinal perforation, bowel ischaemia, abdominal malignancy or after abdominal procedures. Therefore, an elevated value is not specific to pancreatic disease. The result must be reviewed with symptoms, examination, imaging and a complete ascitic-fluid analysis.

Preparation Before Test

Ascitic fluid is collected through paracentesis, a procedure in which a trained medical professional inserts a needle or catheter into the abdominal cavity. The procedure is performed in a hospital, clinic or other suitable medical setting. Ultrasound guidance may be used to choose a safe collection site.

Patients should inform the medical team about anticoagulants, antiplatelet medicines, bleeding disorders, pregnancy, allergies and previous abdominal procedures. The doctor may request blood clotting tests or other investigations before paracentesis. Fasting is not always necessary, but patients should follow the procedure-specific instructions provided by the treating team.

The specimen must be collected in the correct laboratory container, labelled as ascitic fluid and transported promptly. A paired serum amylase sample may be requested to compare blood and fluid concentrations. The fluid is often divided into separate containers when cell count, culture, cytology, albumin, protein and other tests are also required.

Ascitic-fluid amylase is generally processed as a routine biochemistry assay, and the result is commonly available on the same day or within 24 hours after the specimen reaches the laboratory. Turnaround time may vary according to collection time, transport, sample quality and laboratory workflow. Other ascitic-fluid investigations, particularly culture or cytology, may require additional time.

The laboratory reports amylase activity using its validated method and units. There is no single universal cutoff suitable for every situation. Doctors may compare the fluid result with the serum amylase concentration, the laboratory's reference information and the overall clinical pattern. A markedly elevated fluid value relative to serum can strengthen suspicion of a pancreatic source.

Amylase is only one component of ascitic-fluid assessment. Albumin in serum and ascitic fluid may be used to calculate the serum-ascites albumin gradient, which helps evaluate whether portal hypertension is present. Total protein, cell count with differential, Gram stain, bacterial culture, glucose, lactate dehydrogenase, triglycerides, bilirubin or cytology may be ordered according to the suspected cause.

A low or non-elevated fluid amylase does not identify the cause of ascites and does not exclude all pancreatic conditions. Similarly, a high result does not independently establish pancreatic duct leakage. Imaging and specialist assessment remain essential.

If pancreatic ascites is confirmed, management depends on the underlying problem and may include nutritional support, drainage, medicines that reduce pancreatic secretion, endoscopic pancreatic-duct stenting or surgery. Suspected bowel perforation or infected ascites requires urgent medical evaluation. Treatment decisions should be made by the responsible gastroenterology, surgical or hospital team.
