Amylase - Body Fluid is a specialized biochemical test used to measure amylase enzyme activity in a body-fluid sample, such as pleural fluid, peritoneal or ascitic fluid, or fluid collected from a surgical drain. Amylase is a digestive enzyme produced mainly by the pancreas and salivary glands that helps break down carbohydrates.

Amylase may enter an abnormal body-fluid collection when pancreatic inflammation, pancreatic duct disruption, a pancreatic fistula, or another condition causes enzyme-rich pancreatic fluid to leak. Increased amylase in pleural fluid may also be associated with esophageal rupture, certain malignancies, pneumonia, or other disorders. Results are usually compared with a simultaneously measured serum amylase level and interpreted with symptoms, imaging, fluid appearance, and other laboratory findings.

Amylase is an enzyme that breaks complex carbohydrates into smaller sugars. Most circulating amylase originates from the pancreas and salivary glands. When pancreatic tissue becomes inflamed or a pancreatic duct is damaged, amylase-rich pancreatic secretions may leak into surrounding tissues and body cavities.

Body-fluid amylase testing may be performed on pleural fluid collected from around the lungs, peritoneal or ascitic fluid collected from the abdomen, and fluid obtained from a surgical or interventional drain. The specimen source must be clearly identified because interpretation differs according to the type of fluid.

In peritoneal or ascitic fluid, markedly increased amylase compared with serum may indicate that pancreatic fluid is present in the abdominal cavity. This may occur with acute or chronic pancreatitis, pancreatic duct disruption, rupture of a pancreatic pseudocyst, trauma, or postoperative leakage.

Pancreatic ascites occurs when enzyme-rich pancreatic secretions leak into the peritoneal cavity. Patients may develop progressive abdominal swelling, discomfort, weight changes, breathing difficulty, or persistent fluid accumulation. Body-fluid amylase is interpreted with fluid protein, albumin, lipase, cell counts, cultures, and imaging findings.

Pleural fluid amylase may become elevated when pancreatic fluid reaches the chest through a fistula or through openings in the diaphragm. Pancreatic pleural effusions may be large or recurrent and can cause chest discomfort, coughing, or shortness of breath.

Elevated pleural fluid amylase is not specific for pancreatic disease. It may also occur with esophageal rupture, certain amylase-producing malignancies, pneumonia, or other conditions. When available, amylase isoenzyme testing may help determine whether the enzyme is predominantly pancreatic or salivary in origin.

Amylase measurement in surgical drain fluid may help identify a postoperative pancreatic fistula. A persistent or substantially elevated drain-fluid amylase level compared with serum may indicate leakage of pancreatic secretions following pancreatic or upper abdominal surgery.

There is no single universal reference interval for every type of body fluid. Doctors generally compare the fluid amylase level with the serum amylase level and evaluate the fluid-to-serum relationship. Interpretation also depends on the collection site, clinical condition, timing, and laboratory method.

A high body-fluid amylase result does not independently identify the exact cause. Further evaluation may include serum amylase and lipase, fluid lipase, protein, albumin, cell count, microbiology, cytology, CT, MRI, MRCP, endoscopy, or other procedures.

This general body-fluid test may not be appropriate for pancreatic cyst fluid. Pancreatic cyst aspirates often require a dedicated cyst-fluid amylase test together with carcinoembryonic antigen, glucose, cytology, molecular analysis, and imaging findings.

The fluid-collection procedure depends on the location. Pleural fluid may be collected through thoracentesis, ascitic fluid through paracentesis, and drain fluid directly from a properly placed drainage system. These procedures must be performed or supervised by qualified healthcare professionals.
