The Angiotensin Converting Enzyme (ACE) – CSF test measures the concentration or activity of angiotensin-converting enzyme in cerebrospinal fluid. Cerebrospinal fluid, commonly called CSF, is the clear fluid surrounding the brain and spinal cord. ACE is an enzyme involved in regulating blood pressure and fluid balance, but it may also be produced in increased amounts by activated immune cells called macrophages within granulomas.
Doctors may use CSF ACE testing as one component of an evaluation for inflammatory or granulomatous conditions affecting the central nervous system. It is most commonly considered when neurosarcoidosis is suspected. Neurosarcoidosis occurs when sarcoid inflammation affects the brain, spinal cord, cranial nerves or surrounding tissues. However, the ACE CSF test has limited sensitivity and specificity and cannot independently confirm or exclude this condition.

The test may also be considered when magnetic resonance imaging or routine CSF findings show inflammation that requires further investigation. Doctors generally interpret CSF ACE alongside medical history, neurological examination, brain or spinal imaging, serum ACE, chest imaging and other laboratory findings. In some situations, examination or biopsy of a more safely accessible affected tissue may be necessary.
An increased CSF ACE result is not unique to neurosarcoidosis. Other inflammatory, infectious or malignant conditions may sometimes affect the result. Similarly, a result within the laboratory reference interval does not rule out neurosarcoidosis. The test is therefore supportive rather than definitive.
Preparation Before Test
This test requires cerebrospinal fluid, which is usually collected through a lumbar puncture by a qualified medical professional in an appropriate clinical setting. The laboratory itself does not replace the medical assessment required before this procedure. Your healthcare team will explain the procedure, its benefits and its possible risks.
Inform the doctor about blood-thinning medicines, bleeding disorders, low platelet counts, allergies, pregnancy, recent infections and all current medications. Do not discontinue anticoagulants or any prescribed medicine without medical instructions. Imaging or blood tests may be needed before lumbar puncture in selected patients.
You may be asked to remain still in a curled side-lying or seated position during collection. After the procedure, follow the clinical team's instructions about observation, activity and hydration. Seek medical advice if you develop a severe or persistent headache, fever, increasing back pain, weakness, fluid leakage or other concerning symptoms.

ACE testing in CSF is a specialized investigation and is generally reported within a few working days. Turnaround time may vary because the specimen may require scheduled processing or referral to a specialized laboratory. ACE contributes to the renin-angiotensin system by converting angiotensin I into angiotensin II. In sarcoidosis, immune cells accumulate and form granulomas. These activated cells can produce ACE, which is why ACE may be measured in blood or cerebrospinal fluid when sarcoidosis is being investigated. Serum and CSF ACE measurements are not interchangeable, and neither test is sufficiently accurate to diagnose sarcoidosis alone.
CSF ACE results are interpreted using the reference interval and analytical method of the performing laboratory. The doctor may also review CSF cell count, protein, glucose, oligoclonal bands, immunoglobulins, cultures, molecular tests or cytology, depending on the clinical concern. MRI findings and evidence of sarcoidosis in the lungs, lymph nodes, skin, eyes or other organs can provide important context.
A raised result may support the clinical suspicion of neurosarcoidosis, but alternative causes must be assessed. A normal result cannot exclude the condition because some affected patients do not have increased CSF ACE. Diagnosis usually depends on the combined evidence from symptoms, imaging, laboratory studies and, whenever appropriate and feasible, tissue biopsy.
