About this test

The Anti Thyroglobulin Antibody (ATG) test is a blood test that detects antibodies directed against thyroglobulin, a protein produced and stored within the thyroid gland. Thyroglobulin plays an important role in the production of the thyroid hormones thyroxine (T4) and triiodothyronine (T3). Normally, the immune system does not significantly attack this protein. In autoimmune thyroid disorders, however, it may mistakenly produce anti-thyroglobulin antibodies.

Anti-thyroglobulin antibodies, also called TgAb, are commonly associated with Hashimoto thyroiditis and may also be present in Graves disease or other autoimmune thyroid conditions. The test is generally interpreted with thyroid-stimulating hormone, free T4, anti-thyroid peroxidase antibodies and the patient's symptoms. A positive result supports thyroid autoimmunity but does not independently establish a specific diagnosis.

When the thyroid is overactive, possible symptoms include unexplained weight loss, heat intolerance, sweating, tremor, anxiety, rapid heartbeat, frequent bowel movements and sleep disturbance. Anti-thyroglobulin antibodies may be found in some people with Graves disease, although other antibody tests are more directly associated with that condition.

The ATG test is also important when thyroglobulin is measured after treatment for differentiated thyroid cancer. Anti-thyroglobulin antibodies can interfere with some thyroglobulin laboratory methods, potentially causing a misleading result. For this reason, doctors may request both thyroglobulin and TgAb during follow-up. The antibody pattern over time may provide additional context, but interpretation must be based on the assay method and the patient's complete clinical history.

Preparation Before Test

A routine blood sample is required. Fasting is generally not necessary when the ATG test is ordered alone. If it is combined with other fasting investigations, follow the instructions provided by your doctor or Focus Diagnostics.

Inform your doctor about thyroid medicines, supplements, recent thyroid procedures, radioactive iodine treatment and all other medications. Biotin supplements can interfere with some laboratory tests, so disclose the product and dose before collection. Do not stop biotin, thyroid medicine or any prescribed treatment unless advised by a qualified healthcare professional.

If this test is being used for thyroid-cancer follow-up, try to use the same laboratory or a comparable testing method for serial measurements whenever your treating specialist recommends it. Differences between assays can affect comparisons over time.

A positive or elevated ATG result indicates that antibodies against thyroglobulin are present. This finding is compatible with thyroid autoimmunity, but it does not show how well the thyroid is functioning. A person may have antibodies while thyroid hormone levels remain within their reference intervals, and some people with autoimmune thyroid disease may not have detectable TgAb.

The measured antibody concentration does not necessarily reflect the severity of symptoms or the extent of thyroid damage.
