The thyroid gland produces hormones that regulate metabolism, energy use, heart rate, body temperature, digestion, growth, and several other body functions. Thyroid peroxidase is an enzyme required for the production of thyroxine and triiodothyronine.

In autoimmune thyroid disease, the immune system mistakenly recognizes thyroid components as harmful. It produces anti-TPO antibodies that target thyroid peroxidase and may contribute to inflammation and progressive damage to thyroid cells.

Anti-TPO antibodies are most commonly associated with Hashimoto's thyroiditis, a major cause of hypothyroidism. Hashimoto's thyroiditis may initially produce few symptoms, but progressive thyroid damage can cause fatigue, weight gain, cold intolerance, dry skin, constipation, hair thinning, menstrual changes, slow heart rate, and thyroid enlargement.

Anti-TPO antibodies may also be detected in Graves' disease, an autoimmune condition commonly associated with excessive thyroid-hormone production. Graves' disease may cause weight loss, heat intolerance, excessive sweating, tremors, nervousness, palpitations, frequent bowel movements, and eye-related symptoms.

A positive or elevated Anti-TPO result supports the presence of thyroid autoimmunity but does not independently determine current thyroid function, the severity of symptoms, or the amount of thyroid damage. Some people with detectable antibodies may initially have normal thyroid-hormone levels.

Low levels of Anti-TPO antibodies may occasionally be detected in people without clinically apparent thyroid disease. Therefore, the result should not be used alone to diagnose hypothyroidism, hyperthyroidism, Hashimoto's thyroiditis, or Graves' disease.

A negative result does not completely exclude autoimmune thyroid disease. Depending on the clinical findings, doctors may recommend TSH, free T4, free T3, anti-thyroglobulin antibodies, TSH-receptor antibodies, thyroid ultrasound, or other investigations.

Results should always be interpreted using the laboratory's reference range and in combination with symptoms, physical examination, thyroid-function tests, pregnancy status, medication history, and physician evaluation. Repeated Anti-TPO measurements are not always required because antibody levels may not directly reflect disease severity or treatment response.
