The Apolipoprotein Evaluation Panel is a blood test panel that measures Apolipoprotein A1, Apolipoprotein B and Lipoprotein(a). Apolipoprotein A1, also called Apo A1, is the primary protein in high-density lipoprotein particles. It supports reverse cholesterol transport, a process through which excess cholesterol is carried away from tissues for processing and removal. Apolipoprotein B, or Apo B, is present on LDL and other potentially artery-damaging particles. Because each atherogenic particle generally contains one Apo B molecule, its level provides an estimate of particle number. Lipoprotein(a), abbreviated Lp(a), is a largely inherited lipoprotein associated with cardiovascular and aortic-valve risk when elevated.

Preparation Before Test
A routine blood sample is required. Fasting is not always necessary for Apo A1, Apo B or Lipoprotein(a), but your doctor may request an eight-to-twelve-hour fast when the panel is performed with a complete fasting lipid profile. A higher Apo B level generally indicates more atherogenic particles circulating in the blood. A lower Apo A1 value may reflect reduced protective HDL-related capacity, although HDL function cannot be assessed fully using Apo A1 alone. Lipoprotein(a) contains an Apo B-containing particle attached to apolipoprotein(a). Its concentration is strongly influenced by inherited genetic variation. Lifestyle changes may have only a modest effect on Lp(a), but controlling LDL cholesterol, blood pressure, diabetes, smoking and other modifiable risks remains important. Treatment decisions depend on the patient's total cardiovascular-risk profile and current medical guidance.
Results should be interpreted using the units, analytical methods and reference intervals provided by the performing laboratory. Apo B, Apo A1 and Lipoprotein(a) methods or units may differ between laboratories. An elevated result does not prove that an artery is currently blocked, while values within the stated range do not remove all cardiovascular risk. The treating doctor may correlate the panel with a standard lipid profile, blood pressure, glucose control, kidney function, family history and cardiovascular imaging where appropriate.
