Aspartate Aminotransferase (AST/SGOT) is a blood test used to measure the level of aspartate aminotransferase, an enzyme found in the liver, skeletal muscles, heart, kidneys, brain, and red blood cells. AST was previously called serum glutamic-oxaloacetic transaminase or SGOT.
When cells containing AST become damaged, the enzyme may be released into the bloodstream. Elevated AST may therefore indicate liver-cell injury, muscle damage, or injury involving another AST-containing tissue. The result is usually interpreted with alanine aminotransferase, alkaline phosphatase, bilirubin, creatine kinase, and other investigations.

Aspartate aminotransferase is an enzyme that helps transfer amino groups during amino acid metabolism. It is present inside cells in several organs and tissues, with substantial concentrations found in the liver, heart, and skeletal muscles.
When liver cells are inflamed or damaged, AST can leak into the bloodstream. Increased levels may occur with viral hepatitis, fatty liver disease, alcohol-associated liver injury, medication-related liver damage, reduced blood flow to the liver, autoimmune hepatitis, or other liver conditions.
AST is not specific to the liver. Muscle injury, strenuous exercise, muscle disorders, heart injury, haemolysis, and certain other conditions may also increase the result. An elevated AST value cannot independently identify the affected organ or the cause of the increase.
AST is commonly interpreted with alanine aminotransferase. ALT is found more predominantly in the liver and is generally more liver-specific. The relative pattern and degree of elevation of AST and ALT may provide useful diagnostic information.
The AST-to-ALT ratio may be considered in selected clinical situations, but it should never be interpreted alone. Age, medical history, symptoms, alcohol exposure, medications, other laboratory results, and the degree of enzyme elevation must also be considered.
Mild increases may be temporary and can occur after exercise, muscle injury, alcohol consumption, illness, or medicine use. Markedly elevated values may indicate significant tissue injury and require prompt medical evaluation.
A normal AST result does not completely exclude liver disease because certain chronic conditions may be present without a substantial enzyme elevation. Additional investigations may include ALT, ALP, GGT, bilirubin, albumin, total protein, prothrombin time, viral markers, creatine kinase, ultrasound, elastography, or other imaging.
Reference intervals vary according to laboratory methodology, age, sex, and clinical circumstances. Results should always be interpreted using the range stated in the laboratory report and together with physician evaluation.
