About this test
The Beta 2 Microglobulin – Urine Spot test measures beta-2 microglobulin, commonly abbreviated as B2M, in a single urine sample. B2M is a small protein found on the surface of most nucleated cells and is released into the bloodstream during normal cell turnover. It is filtered from the blood by the kidney's glomeruli and is then normally reabsorbed and broken down by the proximal renal tubules.
Only a small amount of B2M is usually present in urine when the filtering and reabsorbing functions of the kidneys are intact. An increased urinary concentration can occur when the proximal tubules cannot reabsorb the filtered protein effectively. The test can therefore provide information about renal tubular function and possible tubular injury.

The result does not identify the cause of injury by itself.
The test may be considered for people exposed occupationally or environmentally to substances that can damage the renal tubules, such as cadmium or mercury. It may also be used in selected patients receiving medicines with potential kidney toxicity. Monitoring requirements depend on the specific exposure, treatment and patient's existing kidney function.
Urine B2M may also contribute to the investigation of Fanconi syndrome or another proximal tubular disorder, particularly when there are additional findings such as glucose, phosphate, bicarbonate, uric acid or amino-acid loss in urine. A complete diagnosis requires other blood and urine measurements.
Preparation Before Test
A freshly collected spot urine specimen is required. The laboratory may request a clean-catch, midstream sample in a sterile or approved container. Wash your hands, clean the genital area according to the collection instructions, begin urinating into the toilet and then collect the middle portion without touching the inside of the container.
Beta-2 microglobulin is unstable in acidic urine and can break down when urine pH is low. Therefore, collection, preservation and transport instructions must be followed carefully. Focus Diagnostics may use a specially prepared container, check the specimen pH or apply a laboratory-approved preservation procedure. Patients should not add substances to the urine or attempt to change its pH themselves.
Submit the sample promptly and store it only as instructed. Inform your doctor about current medicines, recent infections, kidney disease, heavy-metal exposure and occupational history. Do not stop prescribed medicines without medical advice. Strenuous exercise and acute illness can influence several urinary proteins, so provide relevant information to the healthcare team.

The precise turnaround time may vary according to sample stability, testing schedule and whether referral processing is required. A specimen that does not meet pH, storage or transport requirements may need recollection.
Who Should Take This Test?
This test may be appropriate for patients with suspected renal tubular dysfunction, unexplained kidney abnormalities or possible exposure to a tubular toxin. It may also be recommended for selected individuals receiving treatment that requires monitoring for kidney tubular injury.
The test is not a routine standalone screen for all kidney disease. Serum creatinine and estimated glomerular filtration rate mainly assess filtration, while urine B2M provides different information related to tubular reabsorption. The doctor selects the appropriate combination of tests based on the clinical concern.
Detailed Information
An increased urine B2M result may indicate reduced reabsorption by the proximal renal tubules. This pattern can occur in tubulointerstitial disease, heavy-metal toxicity, Fanconi syndrome and other causes of tubular injury. However, the result is not specific, and temporary changes may occur during illness or other physiological stress.
A low or undetectable result is usually expected when tubular handling is normal, but an artificially low value can occur if B2M degrades in an acidic or improperly stored sample. Specimen quality and urine pH are therefore important parts of interpretation. Reference intervals and reporting units vary between laboratories.
Some reports may express B2M as a concentration, while others may use a ratio to urine creatinine to account for urine dilution. The doctor may correlate the result with serum B2M, serum creatinine, eGFR, urine albumin, total protein, electrolytes, glucose, phosphate and other tubular markers.
Blood B2M and urine B2M are not interchangeable. Blood concentrations can rise with reduced kidney filtration, increased immune-cell activity and certain haematological conditions. Urine B2M is used mainly to assess tubular handling. Neither test independently confirms a particular kidney disease, cancer or toxic exposure.
