The C-Reactive Protein (CRP) - Quantitative test measures the concentration of C-reactive protein in the blood. CRP is an acute-phase protein produced mainly by the liver in response to inflammation. Its concentration can increase when the body is responding to an infection, injury, surgery, tissue damage or an inflammatory medical condition.
A quantitative CRP test provides a numerical measurement rather than simply reporting the result as positive or negative. This allows doctors to assess the degree of inflammation and compare results over time. A rising value may indicate increasing inflammatory activity, while a falling value may suggest that inflammation is resolving or responding to treatment.
CRP is a nonspecific marker. It can show that inflammation is present, but it cannot independently identify its exact location or cause. The result must therefore be interpreted alongside symptoms, physical examination findings, medical history and other laboratory or imaging investigations.

A doctor may recommend a quantitative CRP test when symptoms suggest an acute infection or inflammatory condition. The selection of additional investigations depends on the patient's symptoms and clinical condition.
The test may also be used to monitor inflammatory disorders such as rheumatoid arthritis, inflammatory bowel disease, vasculitis and certain other autoimmune conditions. Serial results may help doctors evaluate whether disease activity is increasing or whether treatment is reducing inflammation.
CRP may be measured before or after surgery and during the evaluation of tissue injury. Persistently elevated or increasing levels may lead the doctor to investigate possible infection, ongoing inflammation or another complication. However, CRP alone cannot confirm any specific complication.
This standard quantitative CRP test should not be confused with high-sensitivity CRP, also called hs-CRP. Standard CRP is generally used to evaluate significant inflammation or infection. The hs-CRP test measures much lower concentrations and may be used in selected cardiovascular risk assessments.
Preparation Before Test
Fasting is generally not required for a CRP quantitative test unless other tests collected at the same time require fasting. A healthcare professional collects a blood sample from a vein, usually in the arm. The procedure generally takes only a few minutes.
Inform the doctor about all medicines, supplements and ongoing treatments. Certain medicines may affect inflammation or CRP concentrations. Do not stop antibiotics, anti-inflammatory medicines, steroids or any prescribed treatment without medical advice.
Tell the doctor about recent infections, injuries, surgery, vaccination, pregnancy, smoking and chronic inflammatory conditions because these factors may affect interpretation. Strenuous exercise can temporarily influence inflammatory markers in some people, so follow any preparation instructions provided by the doctor or laboratory.

The quantitative CRP result is commonly available on the same day or within 24 hours after the laboratory receives the blood sample. Reporting time may vary depending on specimen collection time, laboratory workflow, result verification and whether other investigations were ordered with the test.
Who Should Take This Test?
This test may be appropriate for people with symptoms suggesting infection or inflammation, including unexplained fever, chills, swelling, persistent pain, marked fatigue or symptoms of an autoimmune disorder. It may also be requested for patients being monitored during treatment for an infection or inflammatory disease.
Doctors may order the test for patients with rheumatoid arthritis, inflammatory bowel disease, vasculitis or other conditions where monitoring inflammatory activity may assist clinical management. It can also be performed following surgery or significant injury when medically indicated.
The test is not recommended as a stand-alone method for diagnosing a specific disease. A normal CRP result does not exclude every infection or inflammatory condition, and an elevated result does not automatically indicate a serious bacterial infection.

When inflammation occurs, immune signalling substances stimulate the liver to release acute-phase proteins, including CRP. The CRP concentration may rise relatively rapidly after an inflammatory stimulus and can change as the underlying process improves or worsens. This makes CRP useful for monitoring trends.
High CRP may be associated with bacterial or viral infections, autoimmune diseases, inflammatory bowel disease, tissue injury, burns, surgery and several other conditions. The extent of elevation can provide information about the inflammatory response, but values can overlap between different diseases.
A low CRP concentration generally indicates that there is no substantial systemic inflammatory response at the time of collection. However, some localised infections, early disease stages and certain inflammatory disorders may not cause a major CRP increase. Clinical assessment remains important even when the result is within the laboratory's reference interval.
Reference intervals and measurement units may differ between laboratories depending on the analytical method. Results are commonly expressed in milligrams per litre. The reference interval printed on the individual laboratory report should always be used rather than applying a range from another laboratory.
CRP is often interpreted with a complete blood count, erythrocyte sedimentation rate, procalcitonin, cultures, liver and kidney function tests or imaging studies. The appropriate combination depends on the suspected condition. ESR may respond more slowly than CRP and can remain elevated longer, so the two tests provide different but complementary information.
A single elevated result should not be used to identify the cause of inflammation. Doctors may recommend repeat testing to observe whether the value is increasing or decreasing. Interpretation should consider the magnitude of elevation, symptoms, duration of illness, medicine use and results of other investigations.
