Lithium Test
The Lithium test is a therapeutic drug-monitoring blood test that measures the concentration of lithium in serum. It is primarily used for patients taking prescribed lithium to treat or prevent episodes of bipolar disorder and selected other psychiatric conditions. The result helps the treating doctor determine whether the concentration is within the individual's prescribed target, too low to provide the intended benefit, or high enough to increase the risk of toxicity.
Lithium can be highly effective, but the difference between an effective concentration and a potentially harmful concentration can be relatively narrow. Its level is affected by dose, formulation, timing of the last dose, kidney function, age, hydration, salt intake, illness, pregnancy, and interactions with medicines. Regular blood testing is therefore an important part of safe lithium treatment.
A routine monitoring sample should generally be collected as a trough specimen, commonly about 12 hours after the last dose and immediately before the next scheduled dose. Testing at a consistent interval after dosing allows serial results to be compared. A sample collected too soon after a dose may appear higher than a standard trough level and may lead to incorrect interpretation.
The target concentration is individualised. It depends on whether lithium is being used for an acute episode or maintenance, clinical response, age, previous relapse, adverse effects, kidney function, pregnancy status, and the prescribing clinician's treatment plan. Patients should use the target written by their treating psychiatrist or physician rather than adjusting treatment according to a general internet range.
When lithium toxicity is suspected, blood should be collected urgently according to emergency medical guidance rather than waiting for the routine 12-hour trough time. Clinical symptoms may require urgent treatment even before the laboratory result is available.

Clinical Indications and Applications
After Starting Lithium: The doctor checks the lithium concentration after treatment begins to confirm that the prescribed dose produces an appropriate trough level. Testing is usually performed after enough time has passed for the concentration to reach a steady state.
After a Dose Change: A repeat level is generally required after increasing or decreasing the dose. The result helps the clinician determine whether further adjustment is necessary.
Routine Long-Term Monitoring: Once treatment and blood levels are stable, lithium is measured periodically. The monitoring interval depends on clinical guidance, age, kidney function, interacting medicines, previous results, treatment stability, and other risk factors.
Suspected Lithium Toxicity: Urgent testing is required when a patient taking lithium develops symptoms such as worsening tremor, vomiting, diarrhoea, muscle weakness, lack of coordination, unsteady walking, slurred speech, confusion, unusual sleepiness, blurred vision, seizures, or reduced consciousness.
Acute Illness or Dehydration: Fever, vomiting, diarrhoea, reduced fluid intake, excessive sweating, and dehydration can increase lithium concentrations by changing kidney handling and body sodium-water balance. The treating doctor may recommend urgent or additional monitoring.
Kidney Function Changes: Lithium is eliminated primarily through the kidneys. Reduced kidney function can slow lithium clearance and increase the concentration. Kidney function should be monitored throughout treatment.
Medication Interactions: Several medicines may increase lithium concentrations or the risk of toxicity. Important examples include certain non-steroidal anti-inflammatory medicines, ACE inhibitors, angiotensin receptor blockers, and thiazide diuretics. Other medicines can also interact, so patients must inform every prescriber and pharmacist that they take lithium.
Pregnancy and Major Physiological Changes: Pregnancy, delivery, major changes in body fluid, significant weight changes, and ageing may alter lithium handling. Monitoring plans may need to be individualised and more frequent.
Change in Clinical Response: Testing may be requested when mood symptoms return, adverse effects develop, adherence is uncertain, the lithium brand or formulation changes, or the dosing schedule is modified.
Why Doctors Recommend This Test
Doctors recommend lithium monitoring because the medicine has a narrow therapeutic index. A dose that is appropriate for one person may be too low or too high for another. Even in the same patient, the concentration can change because of illness, kidney function, fluid balance, salt intake, or drug interactions.
A commonly cited trough therapeutic interval is approximately 0.5 to 1.2 mmol/L, but the exact target must be determined by the treating clinician. Some maintenance plans use a narrower target, while selected patients may require a different goal. Laboratory reference intervals and treatment guidelines can also differ.
A result within a general therapeutic interval does not guarantee that the patient is free from adverse effects. Some people experience side effects at concentrations tolerated by others. Toxicity can occasionally occur at an apparently therapeutic measurement, especially when symptoms, timing, chronic exposure, or tissue distribution are considered.
Monitoring is not limited to serum lithium. Long-term therapy generally requires kidney function, thyroid function, calcium, electrolytes, weight, and other clinical assessments. An electrocardiogram or additional monitoring may be recommended for selected patients.
Preparation Before the Test
For routine therapeutic monitoring, arrange the blood collection approximately 12 hours after the last lithium dose or according to the exact instructions of the prescribing doctor. Record the time of the last dose, dose amount, formulation, dosing schedule, and blood collection time.
If lithium is taken once daily at night, the blood sample is commonly collected the following morning before the next dose. If it is taken more than once daily, ask the prescriber which dose should be delayed until after collection. Do not independently skip or change a dose.
Fasting is generally not required for lithium measurement unless other blood tests require fasting. Maintain normal fluid and salt intake unless your doctor has given different instructions. Do not deliberately drink excessive water or restrict salt before testing.
Inform the healthcare provider about all prescription medicines, non-prescription medicines, pain relievers, diuretics, blood-pressure medicines, supplements, herbal products, and changes in treatment. Do not start or stop an interacting medicine without professional advice.
Tell the doctor about vomiting, diarrhoea, fever, excessive sweating, dehydration, reduced urine output, kidney disease, thyroid disease, pregnancy, or major dietary changes.
A serum tube should be used according to laboratory protocol. Tubes containing lithium heparin must not be used for lithium measurement because they can contaminate the specimen and produce an inaccurate result.
What Happens During the Test?
A trained phlebotomist confirms the timing of the last dose and collects a small blood sample from a vein in the arm. The procedure generally takes only a few minutes.
The needle is removed and pressure is applied to the site. Mild discomfort, bruising, tenderness, or minor bleeding may occur and normally resolves quickly. Inform the phlebotomist if you take anticoagulants, have a bleeding disorder, or have previously fainted during blood collection.
The sample is processed to separate serum and analysed using a validated biochemical method. The report provides the measured lithium concentration, unit, and laboratory reference information.
If toxicity is suspected, the result is communicated urgently according to laboratory critical-result procedures. Repeat lithium measurements, kidney function, electrolytes, electrocardiography, and other emergency investigations may be required.
Normal Reporting Time
The report for the Lithium test is generally available within 2 to 4 hours after the sample reaches the laboratory. Routine monitoring results are usually issued on the same day.
Suspected toxicity should be treated as an urgent medical situation. Turnaround may vary according to sample transport, collection-centre location, repeat analysis, quality control, and emergency communication procedures.
Who Should Consider the Lithium Test?
- Patients before or soon after starting prescribed lithium therapy.
- Individuals whose lithium dose has recently changed.
- Patients undergoing routine long-term lithium monitoring.
- Individuals with symptoms that may indicate lithium toxicity.
- Patients with vomiting, diarrhoea, fever, dehydration, or major fluid loss.
- People with changing kidney function or reduced urine output.
- Patients starting or stopping a medicine that can interact with lithium.
- Individuals with recurring mood symptoms or significant adverse effects.
- Pregnant patients or older adults requiring closer monitoring.
- Patients specifically advised to undergo testing by their psychiatrist or physician.
Understanding the Test Results
The result is generally reported in mmol/L. It should be interpreted using the prescribed target, exact time since the last dose, treatment phase, kidney function, symptoms, and laboratory reference information.
Within the Prescribed Target: A result within the individual's target suggests that the measured trough concentration is consistent with the treatment plan. The doctor also considers clinical response, adverse effects, adherence, thyroid and kidney function, and other monitoring results.
Low Lithium Concentration: A low trough value may occur because the dose is insufficient, doses were missed, the sample was collected too late, absorption changed, fluid or sodium intake changed, or another medicine reduced the concentration. It does not automatically mean the dose should be increased.
High Lithium Concentration: An elevated result may occur because of excessive dosing, reduced kidney clearance, dehydration, salt depletion, acute illness, medication interaction, or incorrect sample timing. The prescriber determines whether the dose should be withheld, reduced, or otherwise managed.
Potentially Toxic Result: Higher concentrations increase the likelihood of toxicity, but the patient's symptoms and clinical condition are essential. Chronic toxicity may be clinically important even when the numerical value is not extremely high.
A non-trough sample should be interpreted cautiously. The doctor may request a repeat properly timed level if the patient is clinically stable. When toxicity is suspected, treatment must not be delayed solely to obtain a standard trough sample.
Clinical Limitations
The serum lithium concentration changes according to the time since the last dose. Results collected at different post-dose intervals may not be comparable. Dose and collection times must be documented accurately.
A single result cannot assess adherence over a long period or determine the ideal dose without clinical evaluation. Formulation, dosing schedule, kidney function, age, pregnancy, hydration, sodium intake, and interacting medicines affect interpretation.
Symptoms can occur within a laboratory therapeutic interval, and some high measurements may be caused by collection in an unsuitable tube or at the wrong time. The result must be interpreted clinically.
The test does not evaluate kidney, thyroid, parathyroid, or cardiac complications of lithium. Separate monitoring is required.
Important Safety Information
Possible lithium toxicity requires urgent medical care. Warning symptoms include worsening coarse tremor, vomiting, diarrhoea, marked weakness, unsteady walking, poor coordination, slurred speech, confusion, unusual drowsiness, blurred vision, muscle twitching, seizures, or reduced consciousness.
Do not take an extra dose to replace a missed dose unless instructed by the prescriber. Do not increase, reduce, or stop lithium according to one laboratory value without medical guidance.
During vomiting, diarrhoea, fever, severe sweating, or reduced fluid intake, contact the treating healthcare team promptly for advice. Dehydration can increase lithium levels.
Avoid starting over-the-counter anti-inflammatory pain medicines or making major changes to dietary salt without discussing them with a doctor or pharmacist familiar with lithium treatment.
