Macroprolactin Test

The Macroprolactin test is a specialized blood investigation used to determine whether an elevated total prolactin result is caused mainly by macroprolactin or by biologically active monomeric prolactin. It is particularly useful when a patient has a high prolactin measurement but has few or none of the symptoms normally associated with clinically significant hyperprolactinemia.

Prolactin is a hormone produced by the anterior pituitary gland, a small endocrine gland located at the base of the brain. Its best-established physiological role is stimulating breast-milk production after childbirth. Prolactin also influences reproductive hormones and can affect ovulation, menstrual cycles, fertility, testosterone levels, sexual function, and bone health when biologically active prolactin remains significantly elevated.

Prolactin circulates in different molecular forms. Monomeric prolactin is the smaller and more biologically active form. Macroprolactin is a larger molecular complex, usually formed when prolactin binds to an immunoglobulin, commonly immunoglobulin G. Because of its size and reduced access to prolactin receptors, macroprolactin generally has lower biological activity and is cleared from the circulation more slowly.

Many routine prolactin immunoassays detect both monomeric prolactin and macroprolactin. As a result, a person may have an elevated total prolactin result even though the biologically active monomeric fraction is within its expected range. This condition is called macroprolactinemia. Identifying it can help prevent unnecessary repeat testing, medication, pituitary imaging, anxiety, or other interventions.

The Macroprolactin test does not simply provide one total hormone value. Depending on the laboratory method, the report may include total prolactin, prolactin remaining after polyethylene glycol precipitation, monomeric or unprecipitated prolactin, percentage recovery, or the percentage of precipitated prolactin. These results must be interpreted using the laboratory's method-specific reference intervals.

Clinical Indications and Applications

Elevated Prolactin Without Symptoms: Macroprolactin testing is commonly considered when total prolactin is elevated but the patient has regular menstrual cycles, no inappropriate milk production, no infertility, no sexual dysfunction, and no other typical features of active hyperprolactinemia.

Discordance Between Results and Clinical Findings: A doctor may recommend this test when the measured prolactin concentration appears inconsistent with the patient's symptoms, physical examination, reproductive hormone results, or pituitary imaging.

Persistent Mild or Moderate Prolactin Elevation: Repeatedly elevated prolactin can arise from stress, pregnancy, breastfeeding, medicines, hypothyroidism, kidney disease, pituitary disorders, or macroprolactin. Screening for macroprolactin may help clarify the pattern.

Female Reproductive Symptoms: Biologically active hyperprolactinemia may cause irregular or absent menstrual periods, infertility, galactorrhoea, reduced libido, vaginal dryness, and low estrogen levels. Macroprolactin testing may be used when the total result and symptoms do not correspond.

Male Reproductive Symptoms: Elevated active prolactin can contribute to reduced libido, erectile dysfunction, infertility, low testosterone, decreased muscle mass, and, occasionally, breast enlargement or galactorrhoea. Macroprolactin testing can help determine whether the total prolactin elevation represents active hormone excess.

Before Extensive Pituitary Evaluation: In selected patients with elevated prolactin but no clear clinical evidence of active hyperprolactinemia, macroprolactin screening may be performed before proceeding to extensive investigation. Imaging decisions remain the responsibility of the treating doctor.

Monitoring an Established Abnormality: Repeat testing may occasionally be used when previous results showed macroprolactinemia or when the patient's symptoms, medicines, or prolactin pattern changes.

Why Doctors Recommend This Test

Doctors recommend the Macroprolactin test because routine total prolactin measurements do not always distinguish between different molecular forms. An elevated total result may be caused mainly by a large, less biologically active macroprolactin complex rather than an increase in active monomeric prolactin.

Without macroprolactin assessment, some patients may be incorrectly diagnosed with clinically significant hyperprolactinemia. This may lead to unnecessary pituitary MRI, endocrine treatment, repeated blood tests, or concern about a pituitary tumour.

Conversely, detecting macroprolactin does not automatically mean that no further investigation is necessary. A patient may have macroprolactin together with elevated monomeric prolactin. Pituitary adenomas and other causes of hyperprolactinemia can coexist with macroprolactinemia. The post-precipitation or monomeric prolactin result is therefore clinically important.

The doctor also considers pregnancy, breastfeeding, stress, sleep, exercise, nipple stimulation, medicines, hypothyroidism, kidney failure, liver disease, pituitary stalk disorders, and prolactin-secreting pituitary adenomas. Macroprolactin is one part of a complete hyperprolactinemia evaluation.

How Is Macroprolactin Measured?

A commonly used laboratory screening technique is polyethylene glycol, or PEG, precipitation. The laboratory first measures total prolactin in the untreated serum sample. A portion of the sample is then mixed with a PEG solution.

PEG causes large proteins and protein complexes, including much of the macroprolactin, to precipitate. The sample is centrifuged, and the prolactin remaining in the clear supernatant is measured. This remaining fraction primarily represents monomeric prolactin, although the method is not perfectly selective.

The laboratory may calculate the percentage of prolactin recovered after PEG treatment or the percentage precipitated from the total. Laboratories may use different calculation formats, cut-offs, assay platforms, and reference intervals. A percentage indicating macroprolactin in one method cannot automatically be applied to another method.

PEG precipitation is a screening method rather than an absolute molecular identification technique. Gel-filtration chromatography can separate prolactin according to molecular size and is considered a more definitive method, but it is technically demanding, expensive, and not routinely available.

Preparation Before the Test

Follow the instructions provided by the referring doctor and Focus Diagnostics. Fasting may not always be required, but some laboratories or clinicians prefer a morning fasting specimen to reduce physiological variation.

Prolactin can rise temporarily during sleep, emotional stress, pain, strenuous exercise, sexual activity, nipple stimulation, and breast examination. The doctor may advise morning collection after the patient has been awake for a suitable period and has rested quietly before blood collection.

Avoid strenuous exercise and significant physical stress before testing when possible. Arrive early enough to sit calmly for approximately 20 to 30 minutes if directed by the laboratory.

Inform the doctor about pregnancy, breastfeeding, menstrual history, fertility treatment, pituitary disease, thyroid disease, kidney disease, liver disease, recent chest injury, and previous prolactin results.

Provide a complete list of prescription medicines, non-prescription medicines, hormonal treatments, psychiatric medicines, anti-nausea medicines, blood-pressure medicines, pain medicines, supplements, and herbal products. Antipsychotics, antidepressants, metoclopramide, domperidone, estrogens, verapamil, opioids, and several other medicines can increase prolactin.

Do not stop any medicine without advice from the prescribing doctor. Sudden discontinuation of psychiatric, hormonal, or other essential treatment may be unsafe.

High-dose biotin can interfere with some immunoassays. Inform the laboratory if you take biotin, vitamin B7, multivitamins, or hair, skin, and nail supplements. Follow the laboratory's instructions about withholding biotin before collection.

What Happens During the Test?

A trained phlebotomist cleans the skin over a vein, usually in the arm, and collects a small blood sample using a sterile needle. The procedure generally takes only a few minutes.

After collection, the needle is removed and pressure is applied to the site. Mild pain, bruising, tenderness, or minor bleeding can occur and usually resolves quickly. Inform the phlebotomist if you have a bleeding disorder, take blood-thinning medicines, or have previously fainted during blood collection.

The sample is processed to separate serum. The laboratory measures total prolactin and performs PEG precipitation to estimate the monomeric or unprecipitated fraction. The report may include several values and an interpretive comment regarding the presence or absence of macroprolactin.

Normal Reporting Time

The report for the Macroprolactin test is generally available within 5 to 7 working days. It may require specialized batch processing, PEG precipitation, repeat analysis, or testing at a referral laboratory.

Understanding the Test Results

The report may include total prolactin, post-PEG or unprecipitated prolactin, monomeric prolactin, percentage recovery, percentage precipitation, and an interpretation. Results must be reviewed using the method-specific reference information on the report.

Macroprolactin Detected: This means a substantial proportion of the total measured prolactin consists of the larger macroprolactin form. If the remaining monomeric prolactin is within its reference interval and the patient has no symptoms, clinically significant prolactin excess may be less likely.

Macroprolactin Not Detected: If total prolactin remains elevated and macroprolactin is not the explanation, the doctor may investigate physiological, medication-related, endocrine, kidney, liver, hypothalamic, or pituitary causes.

Elevated Monomeric Prolactin: If the active monomeric fraction remains above its reference interval after macroprolactin removal, true hyperprolactinemia may be present. Additional clinical investigation may be required even when macroprolactin is also detected.

Indeterminate Result: Some laboratories report an intermediate recovery range in which the presence of macroprolactin cannot be classified confidently. The doctor may repeat the test, review the absolute monomeric concentration, or request another method.

Management should be based on the monomeric prolactin concentration, symptoms, reproductive hormone status, medicines, thyroid function, pregnancy status, kidney function, and pituitary assessment. Patients should not interpret the percentage alone.

Clinical Limitations

PEG precipitation is a screening method and does not exclusively remove macroprolactin. Some monomeric prolactin may also precipitate, and recovery varies between assay systems. Laboratories must use method-specific calculations and reference intervals.

Different immunoassays have different sensitivity to macroprolactin. Results obtained using different platforms may not be directly comparable.

Macroprolactin detection does not exclude a pituitary adenoma, medication effect, hypothyroidism, pregnancy, kidney disease, or another cause of elevated active prolactin. Clinical correlation remains necessary.

Stress, sleep, exercise, nipple stimulation, medicines, biotin interference, and sample timing may influence total prolactin. Repeat testing under controlled conditions may be required.
