Melatonin Test
The Melatonin test is a specialized laboratory investigation used to measure the concentration of melatonin in a blood sample. Melatonin is a hormone produced mainly by the pineal gland in the brain. It plays an important role in regulating the body's internal 24-hour clock, known as the circadian rhythm, and helps coordinate the normal sleep-wake cycle.
Melatonin production is closely controlled by light and darkness. Levels are normally low during daylight hours, begin to rise in the evening under dim-light conditions, remain elevated during the night, and decline toward morning. Bright light during the evening or night can suppress or delay melatonin production. Because melatonin changes considerably according to the time of day, the exact collection time and lighting conditions are essential for interpreting the result.
The test may be requested when a doctor or sleep specialist is investigating a possible circadian rhythm sleep-wake disorder, evaluating unusual sleep timing, studying melatonin secretion, or monitoring melatonin exposure. A single blood sample provides the concentration at one specific time. A series of samples collected at planned intervals may be required when the purpose is to assess the timing of melatonin onset or the phase of the circadian clock.
The Melatonin test is not a routine screening test for every person with insomnia or poor sleep. Sleep problems can result from medical conditions, psychological factors, medicines, lifestyle, breathing disorders, restless legs syndrome, shift work, or environmental factors. The result must therefore be interpreted with the patient's sleep history, collection time, light exposure, medication use, work schedule, and other clinical findings.

Clinical Indications and Applications
Circadian Rhythm Sleep-Wake Disorders: Testing may be considered when a patient's sleep schedule appears to be misaligned with the desired or socially required time. Examples include delayed sleep-wake phase disorder, advanced sleep-wake phase disorder, irregular sleep-wake rhythm, non-24-hour sleep-wake rhythm disorder, and some problems associated with shift work.
Delayed Sleep-Wake Phase: People with delayed sleep timing may have difficulty falling asleep until very late and difficulty waking at the required time. Appropriately timed melatonin measurements may help determine whether the biological night begins later than expected.
Advanced Sleep-Wake Phase: Some individuals become sleepy unusually early in the evening and wake very early in the morning. Melatonin assessment may contribute to evaluating whether their circadian phase is advanced.
Dim-Light Melatonin Onset Assessment: Dim-light melatonin onset, commonly called DLMO, refers to the time at which melatonin begins to rise in the evening under controlled dim-light conditions. It is an established marker of circadian phase. DLMO assessment generally requires multiple carefully timed samples rather than one random sample.
Shift-Work Evaluation: Night shifts and rotating work schedules can disrupt the normal relationship between light exposure, melatonin secretion, sleep, meals, and activity. A specialist may use timed melatonin measurements in selected cases to understand circadian misalignment.
Jet Lag and Schedule Disruption: Rapid travel across time zones can temporarily misalign the internal biological clock with local time. Routine testing is not generally required for jet lag, but melatonin measurement may be used in specialized clinical or research assessments.

Why Doctors Recommend This Test
Doctors may recommend the Melatonin test when symptoms and sleep patterns suggest that the patient's internal biological clock is not aligned with the required sleep schedule. The test can provide objective information about melatonin secretion, particularly when samples are collected at specific times and under controlled lighting.
Melatonin levels vary substantially throughout the day. A result cannot be interpreted appropriately without knowing the collection time. A low daytime level may be normal, while a similar concentration during the expected biological night may require different interpretation. Age, sleep schedule, light exposure, medicines, supplements, recent travel, and shift work can also influence the result.
A single measurement may be suitable when the objective is to determine the melatonin concentration at a particular time or evaluate possible exposure to a melatonin product. When the objective is to determine circadian phase, multiple samples collected every 30 to 60 minutes over a planned evening period may be required. The sleep specialist should specify the collection schedule.
The test does not independently diagnose insomnia, sleep apnoea, narcolepsy, restless legs syndrome, depression, anxiety, or another sleep-related condition. A complete evaluation may include a detailed sleep history, sleep diary, actigraphy, overnight polysomnography, respiratory monitoring, neurological assessment, or other laboratory tests.
Preparation Before the Test
Follow the exact instructions provided by the referring doctor and laboratory. The required preparation depends on whether the test involves one sample or a series of timed samples. Do not select the collection time without medical guidance because timing is central to the interpretation.
Record the exact date and time of collection. Inform the laboratory about your normal bedtime, wake-up time, recent sleep schedule, night-shift work, and travel across time zones. A sleep diary may be requested for several days before testing.
Inform the doctor about all prescription medicines, non-prescription medicines, sleep aids, nutritional supplements, herbal products, and melatonin-containing products. Exogenous melatonin can substantially increase the measured concentration. Do not stop melatonin or any prescribed medicine unless instructed by the treating doctor.
The doctor may advise avoiding melatonin supplements for a specific period before testing when the purpose is to assess natural hormone production. The appropriate interval depends on the product, dose, formulation, timing, clinical purpose, and medical history.
Light exposure must be controlled when the test is intended to assess circadian phase or DLMO. Bright room lighting, mobile phones, computer screens, televisions, and other light-emitting devices may suppress melatonin production. Samples may need to be collected under dim-light conditions according to a specialist protocol.
Caffeine, nicotine, alcohol, strenuous exercise, irregular meals, and sleep deprivation may influence sleep physiology or melatonin patterns. Follow the preparation instructions given by the sleep specialist. Fasting is not always required, but the laboratory may provide dietary restrictions for timed evening sampling.

What Happens During the Procedure?
For a single blood measurement, a trained phlebotomist cleans the skin over a suitable vein, usually in the arm, and collects a small blood sample using a sterile needle. The procedure generally takes only a few minutes.
For a circadian phase assessment, several samples may be collected at specified intervals before and after the usual bedtime. The patient may need to remain in a dimly lit environment throughout the collection period. The exact time of every specimen and relevant lighting conditions should be documented.
The laboratory may require plasma or another validated specimen type depending on the analytical method. The sample is processed and analysed using a sensitive method capable of measuring low melatonin concentrations. Appropriate specimen handling is important because melatonin levels can be very low and the sample may require separation, refrigeration, freezing, or protection according to the performing laboratory's protocol.
The risks associated with blood collection are minimal. Brief discomfort, minor bruising, tenderness, or mild bleeding may occur at the needle site. Fainting or infection is uncommon. Inform the phlebotomist if you feel dizzy or unwell.

The report for the Melatonin test is generally available within 7 to 10 working days. This is a specialized investigation that may require batch processing or testing at a referral laboratory. Understanding the Test Results
The Melatonin test provides a numerical concentration with a specified unit and laboratory reporting information. Interpretation depends heavily on the exact collection time, the patient's age, normal sleep schedule, light exposure, medication use, supplement intake, and the analytical method.
Expected Daily Pattern: Natural melatonin concentrations are generally low during daylight hours, rise during the evening under dim-light conditions, remain higher during the biological night, and fall toward morning. A single concentration should be compared with the expected pattern for the documented collection time rather than interpreted as a general normal or abnormal value.
Low or Undetectable Concentration: A low daytime level may be physiologically expected. A low evening or nighttime result may occur because the sample was collected before melatonin onset, the patient was exposed to bright light, the biological clock was delayed, or melatonin production was reduced. A single low value does not diagnose a sleep disorder.
Elevated Concentration: A higher value may be expected during the biological night. Elevated results may also occur after taking melatonin supplements or medicines containing melatonin. The dose, formulation, and time since administration can considerably influence the concentration.
Shifted Melatonin Timing: When several samples are collected, an unusually late rise may support delayed circadian timing, while an unusually early rise may support advanced timing. The specialist interprets the pattern with the patient's sleep schedule and symptoms.
There is no single universal melatonin reference interval suitable for every collection time and clinical purpose. The laboratory's method-specific reporting information and the specialist's collection protocol should be used. Treatment decisions should not be based on one isolated result.
Clinical Limitations
Melatonin secretion changes rapidly with time and environmental light. A sample collected at the wrong time or under inappropriate lighting may not answer the clinical question. Bright light during evening collection can suppress melatonin and alter DLMO assessment.
A single random measurement cannot reliably establish the complete circadian pattern. Multiple timed samples may be necessary. Differences in laboratory methods, specimen type, assay sensitivity, age, medications, supplements, sleep deprivation, and individual physiology can affect results.
Taking melatonin before the test may produce concentrations that reflect the supplement rather than natural pineal secretion. Patients must disclose the product name, dose, formulation, and time of the last dose.
The test does not determine sleep quality and cannot independently diagnose common causes of insomnia. Normal melatonin secretion does not exclude sleep apnoea, restless legs syndrome, psychiatric conditions, medication effects, poor sleep habits, or other sleep disorders.
Important Safety Information
Do not start, stop, or change the dose of melatonin supplements or prescription medicines based only on a laboratory result. Although melatonin products are widely available, their dose, purity, release pattern, interactions, and suitability can vary. Use them under appropriate medical guidance, particularly during pregnancy, breastfeeding, childhood, older age, anticoagulant treatment, or management of chronic illness.
Persistent insomnia, severe daytime sleepiness, breathing pauses during sleep, sudden sleep attacks, confusion, significant mood changes, or impaired driving requires prompt medical evaluation. A laboratory test should not delay appropriate clinical care.
