Nickel, 24-Hour Urine Test

The Nickel, 24-Hour Urine test measures the total amount of nickel excreted in urine over a complete 24-hour period. Nickel is a naturally occurring metallic element used in stainless steel, alloys, batteries, electroplating, welding materials, coins, jewellery, pigments and several industrial processes. People may be exposed through food, water, air, tobacco smoke, consumer products or the workplace.

Small amounts of nickel may enter the body through normal environmental and dietary exposure. The absorbed nickel is transported in the bloodstream and eliminated mainly through urine. Measuring urine collected over 24 hours can help assess recent systemic exposure, particularly to soluble nickel compounds that are absorbed and excreted more readily.

The test may be requested when occupational or environmental exposure is suspected, when workplace biological monitoring is required or when a clinician is evaluating an unexpectedly elevated nickel result. It does not independently diagnose nickel poisoning or predict whether exposure has caused organ damage.

Urinary nickel measurement requires strict collection and handling because nickel contamination from containers, metal objects, dust or laboratory equipment can produce a falsely elevated result. A special metal-free container and careful adherence to the collection instructions are essential.

What Is Nickel?

Nickel is a hard, silver-white metal that occurs naturally in soil, rocks, water and dust. It is resistant to corrosion and is therefore widely used to manufacture stainless steel and other alloys. Nickel compounds are also used in electroplating, rechargeable batteries, catalysts, ceramics and pigments.

Nickel has not been established as an essential nutrient for humans. Most people encounter small amounts through food and water without developing toxicity. The clinical significance of exposure depends on the chemical form, dose, route, frequency and duration.

Skin contact with nickel is a common cause of allergic contact dermatitis in sensitised individuals. However, the 24-hour urine nickel test is not the appropriate test for diagnosing nickel skin allergy. Patch testing or another allergy evaluation is used for suspected contact hypersensitivity.

How Can Nickel Exposure Occur?

Exposure can occur through inhalation, ingestion, skin contact or certain medical devices. Occupational inhalation is particularly relevant in industries where nickel-containing dust, fumes or soluble compounds are generated.

Occupational Exposure

Workers who may encounter nickel include people involved in nickel refining, mining, smelting, electroplating, stainless-steel production, welding, battery manufacturing, metal grinding, alloy production and catalyst manufacturing. The amount absorbed depends on ventilation, protective equipment, work practices and the solubility and particle size of the nickel compound.

Urinary nickel testing may be included in an occupational-health programme to assess absorbed exposure. Results must be interpreted with workplace air monitoring, timing of collection and the type of nickel compound involved.

Environmental and Consumer Exposure

Nickel is naturally present in certain foods and drinking water. Environmental exposure may also arise from contaminated air, industrial emissions, tobacco smoke or nearby metal-processing activities. Jewellery, watches, belt buckles and other metal objects commonly cause local skin allergy but may not produce substantially elevated urinary nickel.

Medical Devices and Implants

Some metallic implants and devices contain nickel. Wear, corrosion or degradation may release metal ions in selected circumstances. Interpretation requires knowledge of the implant material, symptoms, time since implantation and other measured metals. A urine result alone does not establish implant failure or justify removal.

Why Is the 24-Hour Urine Nickel Test Performed?

A physician, occupational-health specialist, toxicologist or industrial-health professional may request the test to assess recent nickel exposure. It may be useful when the exposure varies throughout the day because a 24-hour collection captures total urinary excretion over a complete cycle.

Common reasons include:

  • Suspected occupational exposure to nickel dust, fumes or soluble compounds.
  • Environmental exposure investigation.
  • Biological monitoring under an occupational-health programme.
  • Follow-up of an elevated random urine or serum nickel result.
  • Assessment after a recognised exposure incident.
  • Evaluation of selected patients with a nickel-containing implant when clinically indicated.

The test is not a routine screening test for people without a credible exposure history. It is also not recommended as a general wellness or nutritional test.

Possible Health Effects of Excessive Nickel Exposure

The effects depend on the exposure route and nickel compound. Inhaled nickel dust or fumes may irritate the nose, throat and lungs. Repeated occupational exposure to some nickel compounds has been associated with respiratory disease and an increased risk of certain cancers.

High-level exposure may affect the respiratory system, gastrointestinal tract or kidneys. Nickel carbonyl is an especially hazardous volatile compound that can cause severe delayed lung injury following inhalation. Suspected acute exposure requires urgent medical evaluation and should not wait for routine laboratory testing.

Symptoms that may occur after significant exposure include:

  • Cough, chest tightness or breathing difficulty.
  • Throat, nose or eye irritation.
  • Nausea, vomiting or abdominal discomfort.
  • Headache, dizziness or unusual weakness.
  • Work-related respiratory symptoms.
  • Kidney-function abnormalities in selected cases.

These symptoms are non-specific and may have many other causes. The urine test cannot establish whether nickel caused a particular symptom.

Sample Requirements

All urine passed during a complete 24-hour period must be collected in the special container supplied or approved by the laboratory. The container should be certified or suitable for trace-element analysis. Ordinary household containers, metal vessels and unapproved bottles must not be used.

The total collection volume and exact start and finish times are required. Missing even one urine specimen can produce an inaccurate calculation and may require the collection to be repeated.

How to Collect a 24-Hour Urine Sample

  1. Obtain the correct trace-element collection container from Focus Diagnostics.
  2. Select a day when every urine sample can be collected.
  3. On the first morning, empty the bladder into the toilet and note the exact time. Do not place this first urine in the container.
  4. Collect every urine sample passed after the starting time throughout the day and night.
  5. At exactly the same time the following morning, empty the bladder and add this final specimen to the container.
  6. Keep the container stored according to laboratory instructions, commonly refrigerated or in a cool location.
  7. Return the complete collection promptly with the start time, finish time and any requested exposure information.

If any urine is missed, spilled or discarded, contact the laboratory. The collection may need to be restarted with a new container.

Avoiding Specimen Contamination

Contamination is a major concern in trace-metal testing. Do not place metal utensils, foil, jewellery or other objects near or inside the collection container. Avoid transferring urine through an unapproved vessel. Do not rinse the container or add any substance unless specifically instructed.

Workplace dust on hands or clothing can contaminate the outside or inside of the container. Wash hands thoroughly and collect the sample away from the exposure area whenever possible. Follow any occupational-health collection protocol provided.

Preparation for the Test

Fasting is generally not required. Patients may usually follow their normal diet and prescribed treatment unless their doctor gives different instructions. A typical diet should not be changed abruptly solely to influence the result.

Inform the healthcare provider about occupational exposure, hobbies involving metalwork, smoking, supplements, medicines, kidney disease, dialysis, metallic implants and recent imaging procedures using contrast media.

Contrast Media and Trace-Metal Testing

Gadolinium- or iodine-containing contrast material can interfere with some inductively coupled plasma mass spectrometry trace-element assays. If contrast has been administered recently, the laboratory may advise delaying collection, sometimes for at least 96 hours. Confirm the exact interval with Focus Diagnostics before beginning the 24-hour collection.

How Is Urinary Nickel Measured?

The laboratory records the total urine volume and analyses a representative portion using a validated trace-element method, commonly inductively coupled plasma mass spectrometry. This technique can measure very low concentrations of metals.

The final result may be reported as micrograms of nickel excreted per 24 hours. Laboratories may also report the concentration per litre. Reference intervals and units vary, so the range printed on the patient's report must be used.

Understanding the Test Result

The result should be interpreted according to the laboratory's method-specific reference interval, collection completeness and exposure history. One elevated result does not automatically establish toxic exposure.

Elevated 24-Hour Urine Nickel

An elevated result may indicate recent occupational or environmental exposure, especially to soluble nickel compounds. Possible sources should be reviewed, including welding, electroplating, stainless-steel production, battery work, metal grinding, tobacco exposure, industrial dust and nickel-containing devices.

Unexpected elevation may also result from sample contamination. Before concluding that significant exposure occurred, the clinician may review collection methods, repeat the test using strict metal-free procedures or request a serum nickel measurement.

Normal Urinary Nickel

A result within the reference interval suggests that excessive absorbed and excreted nickel was not detected during the collection period. However, it does not completely exclude exposure. Some poorly soluble inhaled nickel compounds may be absorbed slowly or may not produce a proportionate increase in urine.

A normal value does not eliminate the need for workplace air monitoring, respiratory assessment or preventive measures when a credible hazard exists.

Reference Values

Reference intervals differ between laboratories. As one example, some laboratories use an adult reference value below 6 micrograms per 24 hours, while paediatric reference intervals may not be established. This example must not replace the range printed on the Focus Diagnostics report.

Can the Result Measure Nickel Toxicity?

Urinary nickel primarily indicates absorbed exposure and excretion. It does not directly measure the amount retained in a particular organ and does not accurately predict health effects. Toxicity assessment requires symptoms, physical examination, exposure intensity, kidney function and other investigations.

The result also does not distinguish every chemical form of nickel. Soluble and insoluble compounds differ in absorption, distribution and elimination.

  • Serum or plasma nickel measurement.
  • Random urine nickel-to-creatinine ratio.
  • Kidney-function tests, including serum creatinine and urinalysis.
  • Complete blood count and liver-function tests.
  • Pulmonary-function testing for respiratory exposure.
  • Chest imaging when medically indicated.
  • Workplace air and surface monitoring.
  • Other trace-metal tests when mixed exposure is suspected.
  • Patch testing for suspected nickel contact allergy.

Nickel Allergy Versus Nickel Exposure

Nickel allergy is an immune reaction that commonly produces an itchy, red or blistering rash where nickel contacts the skin. A person can have severe nickel contact allergy while having a normal urinary nickel result.

Conversely, a person may have increased urinary nickel from occupational exposure without developing contact dermatitis. The 24-hour urine test should therefore not be promoted as a diagnostic test for nickel allergy.

Benefits of the 24-Hour Urine Test

A complete 24-hour collection accounts for changes in urine concentration and excretion throughout the day. It may provide a more representative estimate of total nickel excretion than a single random specimen.

The test is non-invasive and can support occupational or environmental exposure assessment when collected correctly and interpreted with other evidence.

Limitations of the Test

Incomplete urine collection and external contamination can produce misleading results. Special metal-free materials and ultraclean handling are required. Recent contrast administration may interfere with some analytical methods.

Urine measurements are more useful for recent exposure than remote historical exposure. Poorly soluble nickel compounds may be underrepresented. The test cannot independently diagnose organ toxicity, nickel allergy or occupational disease.
