Nickel, Serum Test

The Nickel, Serum test measures the concentration of nickel in the liquid portion of blood. Nickel is a naturally occurring metallic element used extensively in stainless steel, metal alloys, batteries, electroplating, welding materials, jewellery, coins, electronic products and industrial manufacturing. Small amounts may also be present in food, water and the environment.

Nickel has no clearly established essential biological role in humans. Most people are exposed to very low quantities without developing systemic toxicity. Higher exposure may occur in selected workplaces, through contaminated environments, during certain industrial incidents or, in specific clinical situations, from metal-containing medical devices.

Serum nickel testing may be used as a supportive investigation when excessive nickel exposure is suspected or to verify an elevated urinary nickel result. Urine is generally the preferred specimen for routine assessment of nickel exposure because most absorbed nickel is eliminated through the kidneys. A serum result should therefore be interpreted with exposure history, urine nickel measurement, symptoms and other relevant investigations.

What Is Nickel?

Nickel is a silvery-white metal that resists corrosion and tolerates high temperatures. These characteristics make it useful in stainless steel, nickel alloys, rechargeable batteries, catalysts, electronics, metal plating, welding and manufacturing. Nickel compounds may exist in soluble and insoluble forms, and their absorption and toxicity can differ.

Exposure may occur through inhalation, ingestion or skin contact. In occupational settings, inhalation of nickel-containing dust, fumes or aerosols may be important. Skin contact is a common cause of allergic contact dermatitis but does not necessarily produce an elevated serum nickel concentration.

Absorbed nickel circulates in blood and is largely removed from the body through urine. The biological concentration can vary according to the timing, route and chemical form of exposure, as well as kidney function and specimen-collection quality.

Why Is the Serum Nickel Test Performed?

A physician, occupational-health specialist, toxicologist or surgeon may request the test when there is a credible possibility of systemic nickel exposure. It may also be ordered to confirm an unexpectedly elevated urine nickel concentration or as part of a broader metal-exposure assessment.

Occupational Exposure

People may encounter nickel at work in industries involving:

  • Stainless-steel or alloy production.
  • Welding, cutting, grinding or metal finishing.
  • Electroplating and metal coating.
  • Nickel refining, mining or smelting.
  • Battery production, recycling or disposal.
  • Electronic-waste handling.
  • Catalyst production and chemical manufacturing.
  • Jewellery and metal-component manufacturing.

Biological testing does not replace workplace air monitoring, industrial-hygiene assessment or evaluation of respiratory protection and ventilation. The occupational physician considers all these sources of information together.

Environmental or Accidental Exposure

Testing may be considered after a documented spill, fire, industrial incident or environmental contamination involving nickel. The decision depends on the route, amount and timing of exposure and the presence of symptoms.

A serum nickel value cannot identify the exact nickel compound or prove where exposure occurred. Environmental sampling and occupational records may be required to determine the source.

Metal-Containing Medical Devices

Nickel is present in some medical and dental alloys. In selected patients with metal-containing implants or devices, a specialist may consider metal-ion testing when device wear, corrosion or systemic exposure is suspected. The clinical value depends strongly on the device type and manufacturer-specific guidance.

Serum nickel testing does not determine whether an implant is functioning normally and cannot diagnose a mechanical problem. Imaging, physical examination, device history and other metal measurements may be required. Results should be interpreted by an orthopaedic surgeon, cardiologist or other relevant specialist.

Nickel Toxicity and Possible Symptoms

The effects of excessive nickel exposure depend on the compound, route, concentration and duration. Inhalation of nickel-containing dust or fumes may irritate the nose, throat and lungs. High or repeated occupational exposure can contribute to respiratory disease. Certain occupational nickel compounds are associated with increased cancer risk after prolonged inhalation exposure.

Possible symptoms after significant exposure may include:

  • Cough, throat irritation or breathing difficulty.
  • Headache, dizziness or weakness.
  • Nausea, vomiting or abdominal discomfort.
  • Skin irritation after direct contact.
  • Reduced urine output or other organ-related symptoms in severe exposure.

These symptoms are non-specific and may result from many other conditions. An elevated nickel result alone does not establish that nickel caused the symptoms.

Serum Nickel Testing Is Not an Allergy Test

Nickel allergy is a delayed hypersensitivity reaction that commonly causes itchy, red, blistered or scaly skin after contact with jewellery, watch straps, belt buckles, clothing fasteners, mobile devices or other nickel-releasing objects. This condition is known as nickel allergic contact dermatitis.

The amount of nickel circulating in serum does not diagnose contact allergy. A person can have severe nickel contact dermatitis while having no evidence of excessive systemic nickel exposure. Conversely, an increased serum concentration does not prove that a person has nickel allergy.

Patch testing performed and interpreted by a dermatologist is commonly used to evaluate allergic contact dermatitis. In selected specialised situations, cellular immune testing may be considered. Patients seeking evaluation for a rash should not rely on serum nickel measurement as an allergy test.

Why Is Urine Often Preferred?

Urinary nickel measurement is generally preferred for biomonitoring exposure and potential toxicity. A 24-hour urine collection can reduce the effect of variation across the day, while a random urine nickel-to-creatinine ratio may be used in selected settings.

Serum nickel can be useful to verify an elevated urine result or provide additional information in a carefully assessed exposure. The two specimens reflect nickel kinetics differently and should not be considered interchangeable. The doctor selects the appropriate specimen according to the exposure scenario.

Sample Requirements

The test requires serum collected from venous blood. Trace-metal testing needs a metal-free collection system because ordinary needles, tubes, stoppers, containers or laboratory equipment may introduce nickel contamination. The sample must be collected and processed according to a strict trace-element protocol.

A metal-free serum tube and transport vial may be required. The laboratory should confirm the collection kit, minimum volume, centrifugation instructions, storage temperature and transportation procedure before the patient arrives.

Importance of Preventing Contamination

Nickel is widely present in the environment and in metal equipment. Even small external contamination can cause a falsely elevated result because the concentrations being measured are very low. Rigorous collection and handling procedures are therefore essential.

Unexpected elevation without a credible exposure history may reflect contamination. The doctor or laboratory may recommend recollection using confirmed metal-free materials and may compare the result with urinary nickel.

Is Fasting Required?

Fasting is generally not required unless the doctor or laboratory provides different instructions. Patients should follow any preparation requirements for other tests collected during the same visit.

Tell the healthcare provider about occupational exposure, supplements, medical implants, recent procedures, kidney disease and any previous metal-test results. The timing of the sample in relation to the suspected exposure should also be recorded.

Contrast Media Precaution

Gadolinium-containing MRI contrast and iodine-containing radiographic contrast may interfere with many metal tests performed using inductively coupled plasma mass spectrometry. When clinically appropriate, collection should generally be delayed for at least 96 hours after administration of such contrast media.

Patients with significantly reduced kidney function may clear contrast more slowly. The doctor and laboratory may recommend a longer delay. Necessary imaging or emergency treatment should never be postponed merely for this test.

How Is Serum Nickel Measured?

The laboratory commonly measures nickel using inductively coupled plasma mass spectrometry, abbreviated as ICP-MS. The specimen is introduced into a high-temperature plasma that converts elements into charged ions. The mass spectrometer then separates and quantifies nickel ions according to their mass-to-charge characteristics.

ICP-MS is highly sensitive, which makes strict contamination control essential. The report provides the serum nickel concentration with the laboratory's reference interval and measurement units. Reference values from different laboratories should not be compared without considering the method and specimen type.

Understanding the Results

Elevated Serum Nickel

An increased concentration may suggest recent or ongoing systemic exposure, particularly when supported by an occupational or environmental history and an elevated urine nickel result. Possible sources include nickel-containing dust, fumes, solutions, alloys or, in selected cases, a medical device.

An elevated result does not reveal the exposure route, chemical form, exact source or duration. It also does not independently predict toxicity. Kidney function, timing and specimen contamination must be considered.

If the result is unexpected, the doctor may request repeat serum testing using a verified trace-metal collection procedure, a 24-hour urine nickel measurement, a random urine nickel-to-creatinine ratio or workplace exposure assessment.

Normal Serum Nickel

A value within the laboratory reference interval means that an increased serum concentration was not detected at the time of sampling. It does not exclude every previous or intermittent exposure. Nickel may move between blood, tissues and urine, and the measured concentration depends on collection timing.

A normal result also does not exclude nickel contact allergy because allergy is an immune response rather than a measurement of systemic metal concentration.

Unexpected or Borderline Result

A borderline result may be difficult to interpret without a clear exposure history. Minor elevation can occur because of contamination, recent exposure, analytical variation or individual biological differences. Recollection and urinary testing may be more informative than drawing a conclusion from one borderline value.

Additional Investigations

Depending on the exposure and symptoms, additional investigations may include:

  • Twenty-four-hour urinary nickel.
  • Random urine nickel-to-creatinine ratio.
  • Kidney and liver function tests.
  • Complete blood count and routine urinalysis.
  • Chest imaging or lung-function assessment after inhalational exposure.
  • Workplace air sampling and industrial-hygiene review.
  • Testing for other metals involved in the same exposure.
  • Device imaging or specialist assessment when an implant is involved.
  • Dermatological patch testing for suspected nickel contact allergy.

The doctor selects investigations based on the exposure route and clinical presentation. Not every patient requires every test.

Benefits and Limitations

Serum nickel testing provides a sensitive quantitative measurement and may help verify excessive exposure identified by urinary testing. It can contribute to occupational, environmental or device-related evaluations when ordered for an appropriate reason.

Important limitations include vulnerability to contamination, dependence on collection timing and an inability to identify the exact source or chemical form of nickel. Serum is not the preferred specimen for routine exposure biomonitoring, and the test cannot diagnose nickel allergy.

No treatment decision should be based solely on one serum value. Clinical findings, urine nickel, kidney function and exposure assessment must be considered together.
