It is commonly associated with preliminary screening for cystinuria and selected amino-acid metabolism or transport disorders.
Cystinuria is an inherited condition in which the kidneys do not properly reabsorb cystine and certain other amino acids from filtered urine. Excess cystine can accumulate in urine and form crystals or stones because cystine has limited solubility, especially in acidic urine. Identifying increased urinary cystine can therefore contribute to the investigation of recurrent kidney stones, particularly in children or young adults.
The nitroprusside test is qualitative. It generally reports whether the relevant chemical reaction is positive or negative rather than providing the exact concentration of cystine. A positive result is a screening finding and requires confirmation using quantitative urinary cystine measurement, urine amino-acid analysis, stone analysis or genetic testing when clinically indicated.
What Is the Nitroprusside Reaction?
The classical cyanide–nitroprusside reaction is based on the detection of sulfhydryl groups. Cystine contains a disulfide bond and is first reduced to cysteine using a reducing reagent. The released sulfhydryl groups then react with sodium nitroprusside in an alkaline environment, producing a red, purple or magenta colour when sufficient reactive compounds are present.
The intensity and timing of the colour reaction are evaluated according to the laboratory's procedure. Because the test detects a chemical property rather than a uniquely disease-specific molecule, certain other sulfur-containing compounds and medicines can potentially produce a positive reaction.
Nitroprusside chemistry is also used in some urine ketone methods. However, the specialised qualitative spot-urine nitroprusside test described here is used as a screening reaction for cystine and related sulfur-containing amino acids. What Is Cystinuria?
Cystinuria is a hereditary disorder affecting the transport of cystine and the dibasic amino acids ornithine, lysine and arginine in the kidney tubules and intestinal tract. The principal clinical problem is excessive cystine in urine. Cystine is less soluble than the other affected amino acids and can precipitate to form crystals and urinary stones.
Cystine stones may occur in the kidneys, ureters or bladder. They can develop repeatedly and may begin during childhood, adolescence or early adulthood. Stone recurrence can lead to pain, urinary obstruction, infection or progressive kidney damage if not appropriately managed.
The condition is usually inherited in an autosomal recessive pattern, although the genetic and biochemical classifications are complex. Variants in genes involved in renal amino-acid transport, particularly SLC3A1 or SLC7A9, may be responsible. Genetic testing is not required in every patient but may be useful in selected cases.
Symptoms That May Suggest Cystine Stones
A doctor may consider cystinuria testing when a patient experiences:
- Recurrent kidney or urinary-tract stones.
- Stone formation during childhood, adolescence or early adulthood.
- Severe pain in the side, back, lower abdomen or groin.
- Blood in urine.
- Painful urination or frequent urinary urgency.
- Nausea or vomiting accompanying renal colic.

These symptoms are not specific to cystinuria. Calcium oxalate, uric acid, infection-related and other stone types are much more common. Laboratory and imaging investigations are needed to determine the cause.
Why Is This Test Performed?
The qualitative urine nitroprusside test may be ordered as an initial, rapid and relatively inexpensive screen when excessive cystine excretion is suspected. It may help identify patients who require more specific quantitative testing.
It can be particularly useful when evaluating recurrent stone formation, a stone at an unusually young age, a family history of cystinuria or characteristic hexagonal crystals in urine. It may also form part of a broader metabolic urine screen for sulfur-containing amino acids.
The test is not normally used to determine the effectiveness of treatment in a patient with confirmed cystinuria because qualitative results do not provide an exact urinary cystine concentration. Quantitative testing and assessment of urine volume, pH and cystine supersaturation are more suitable for ongoing monitoring.
Spot Urine Sample Requirements
The test requires a spot urine specimen collected in a clean, laboratory-approved container. A random specimen may be accepted, although a first-morning urine sample may sometimes be preferred because it is generally more concentrated. How to Collect the Sample
- Wash your hands before starting the collection.
- Use the clean or sterile container supplied or recommended by the laboratory.
- When instructed, collect a clean-catch midstream urine specimen.
- Avoid touching the inside of the container or lid.
- Prevent contamination with stool, tissue paper, water, disinfectant or menstrual blood.
- Close the container firmly and label it with the required patient and collection details.
- Submit the specimen promptly or store it according to laboratory instructions.
A freshly collected sample is preferable because prolonged storage, bacterial growth, precipitation and changes in urine chemistry may affect the specimen. The laboratory should be consulted regarding refrigeration and the maximum permitted transport time.
Is Fasting Required?
Fasting is generally not required for this qualitative urine screening test. Patients may usually eat and drink normally unless their doctor has ordered additional tests requiring special preparation.
Normal hydration should generally be maintained. Excessive water intake immediately before collection may produce very dilute urine and potentially reduce the concentration of cystine below the detection threshold. Patients should not intentionally dehydrate themselves because dehydration increases the risk of stone formation.
Medicines and Possible Interference
Patients should inform the doctor and laboratory about all medicines and supplements. Sulfur-containing medicines or compounds, including N-acetylcysteine, some penicillin-related medicines and other substances with reactive sulfhydryl groups, may interfere with the chemical reaction and potentially produce a false-positive result.
Do not stop any prescribed medicine without medical advice. The doctor can interpret the result in relation to treatment or arrange a more specific confirmatory method if interference is suspected.
How Is the Test Performed?
The laboratory prepares the urine according to its validated protocol and adds reagents that allow cystine or related compounds to react with nitroprusside. Development of a characteristic red-purple or magenta colour within the specified time is interpreted as a positive reaction. Absence of the expected colour is generally reported as negative.
Exact reagents and procedural details can differ between laboratories. Because some traditional reagents are hazardous and unstable, the procedure must be performed by trained laboratory personnel under controlled conditions. It is not intended as a home test.
Understanding the Test Result
Positive Nitroprusside Test
A positive result indicates that the urine contains a detectable amount of cystine or another compound capable of producing the nitroprusside reaction. In a patient with recurrent stones or characteristic urinary crystals, the finding may support suspected cystinuria.
A positive result is not a definitive diagnosis. The reaction can also detect homocystine or other sulfhydryl-containing compounds, and certain medicines may cause interference. Confirmation is required using a quantitative and more specific analytical method.
Negative Nitroprusside Test
A negative result means that the relevant substances were not detected above the method's threshold in that specimen. It does not completely exclude cystinuria. A dilute sample, low cystine concentration, incomplete chemical reaction or analytical limitations may produce a negative screening result.
If clinical suspicion remains high because of recurrent stones, early age at onset, family history or hexagonal crystals, the doctor may request quantitative urinary cystine analysis despite a negative screen.
Confirmatory and Additional Investigations
Follow-up testing may include:
- Quantitative cystine measurement in a 24-hour urine collection.
- Spot urine cystine-to-creatinine ratio when appropriate.
- Quantitative urine amino-acid analysis.
- Microscopic examination for hexagonal cystine crystals.
- Stone composition analysis using infrared spectroscopy or X-ray diffraction.
- Urine pH, volume and cystine supersaturation assessment.
- Kidney function tests and routine urinalysis.
- Ultrasound or non-contrast CT for urinary stones.
- Genetic testing for selected patients or families.
A 24-hour quantitative cystine measurement is generally more useful for confirming excessive excretion and monitoring established cystinuria. Stone analysis is particularly valuable because identifying a removed stone as cystine strongly supports the diagnosis.
Difference Between This Test and a Urine Ketone Test
Sodium nitroprusside is also used in routine urine strips and tablets that detect acetoacetate, one of the ketone bodies. However, a urine ketone test is ordered and reported specifically as ketones or acetone bodies and is commonly used in diabetes and ketoacidosis assessment.

Management After Confirmed Cystinuria
If cystinuria is confirmed, treatment aims to reduce cystine concentration and improve its solubility in urine. The doctor may recommend high fluid intake distributed throughout the day, reduced sodium consumption and urine alkalinisation. Some patients require medicines that bind cystine and form more soluble compounds.
Management must be individualised and supervised by a nephrologist or urologist. Regular urine studies, imaging and kidney-function assessment may be required because cystine stones commonly recur. When Is Urgent Medical Attention Required?
