The Organic Acid Screen – Quantitative – Urine is a specialised biochemical investigation that measures organic acids and related metabolites excreted in urine. Organic acids are produced naturally when the body processes carbohydrates, fats, amino acids and other nutrients. Most of these substances are normally present in small and predictable quantities. When a metabolic pathway is disrupted, certain organic acids may accumulate and produce a recognisable biochemical pattern.

Gas chromatography–mass spectrometry, commonly called GC-MS, is frequently used to separate, identify and measure these compounds. Many results are reported relative to urine creatinine because the concentration of urine changes according to hydration and kidney function. A quantitative report provides measured concentrations for reportable metabolites, but the overall biochemical pattern is usually more important than one isolated value.

Some conditions produce intermittent or subtle abnormalities, while others are more effectively detected using plasma amino acids, an acylcarnitine profile, enzyme analysis or genetic testing. The metabolites included in the test can also vary according to the laboratory method.
Why Is This Test Recommended?
A paediatrician, neurologist, endocrinologist, geneticist, metabolic specialist or other healthcare professional may recommend the quantitative urine organic acid screen when symptoms suggest an abnormal metabolic pathway. It is commonly considered for newborns, children or adults experiencing unexplained neurological or metabolic symptoms.
Possible clinical indications include poor feeding, persistent or recurrent vomiting, lethargy, developmental delay, developmental regression, failure to thrive, seizures, abnormal muscle tone, unusual urine or body odour, metabolic acidosis, unexplained ketosis, low blood glucose, elevated lactate, increased ammonia or severe deterioration during infections and fasting.
The test may also be requested after an abnormal newborn screening result, when another biochemical investigation is abnormal, or when a family member has a confirmed inherited metabolic disorder. Importance of Collecting the Sample During Symptoms
Some metabolic disorders produce their strongest biochemical abnormalities during an acute episode triggered by fever, infection, prolonged fasting, vomiting or another physiological stress. The concentration of abnormal metabolites may decrease after treatment or when the patient becomes clinically stable. Therefore, a urine sample collected during symptoms may be more informative, provided collection does not interfere with urgent medical treatment.

Inform the doctor and laboratory about prescribed medicines, antibiotics, nutritional supplements, specialised formulas, recent dietary changes, prolonged fasting, intravenous fluids or parenteral nutrition. How Is the Test Performed?
The collected urine is prepared and analysed using a specialised laboratory technique, commonly gas chromatography–mass spectrometry.
