PAPP-A (Pregnancy-Associated Plasma Protein A) Test
The PAPP-A (Pregnancy-Associated Plasma Protein A) Test measures pregnancy-associated plasma protein A in maternal blood. PAPP-A is produced mainly by the placenta during pregnancy and rises as pregnancy progresses.
PAPP-A is commonly measured during the first trimester as one component of combined prenatal screening. A complete first-trimester combined screen usually considers maternal age, gestational age, serum PAPP-A, free beta-human chorionic gonadotropin and fetal nuchal translucency measured by ultrasound.
The test helps calculate the probability that a pregnancy may be affected by selected chromosomal conditions, particularly trisomy 21, trisomy 18 or trisomy 13. PAPP-A by itself cannot diagnose or exclude any chromosomal condition.
A low PAPP-A value may also be associated with altered placental function and a higher probability of complications such as fetal growth restriction, pre-eclampsia or preterm birth. However, most pregnancies with an isolated low PAPP-A result do not develop a serious complication.
The result must be converted into a multiple of the median, or MoM, and adjusted for gestational age and relevant maternal or pregnancy characteristics. A raw concentration should not be interpreted without the laboratory's validated screening calculation.

Understanding PAPP-A
PAPP-A is a large protein produced by placental tissue. It influences the availability of insulin-like growth factors, which contribute to placental development and fetal growth. Maternal blood concentrations normally increase substantially with gestational age.
Because PAPP-A changes rapidly during early pregnancy, an isolated numerical concentration is difficult to interpret. Screening programmes compare the result with the expected median for the exact gestational age and express it as MoM.
A value of 1.0 MoM is approximately equal to the median expected for that gestational age after appropriate adjustment. A result below 1.0 MoM is below the median, but this does not automatically mean that the pregnancy is abnormal.
Maternal weight, ethnicity, smoking, diabetes, assisted conception, number of fetuses and other variables may affect marker distributions. The risk-calculation software should account for the factors required by the validated screening programme.
What Does the PAPP-A Test Measure?
The test measures PAPP-A in maternal serum or plasma using a validated immunoassay. The laboratory may report the raw concentration, the calculated MoM or both. For prenatal risk assessment, the corrected MoM is more clinically useful than the raw concentration.
PAPP-A is often included in the dual-marker test together with free beta-hCG. In a complete combined first-trimester screen, these biochemical markers are assessed with maternal age and the fetal nuchal-translucency measurement.
Pregnancies affected by trisomy 21 often show a pattern that includes reduced PAPP-A and increased free beta-hCG. Trisomy 18 and trisomy 13 may be associated with reduced values of both markers. These patterns overlap substantially with unaffected pregnancies and cannot provide a diagnosis.
A low PAPP-A result may be defined differently between programmes. Values around or below the fifth percentile, commonly approximately 0.4 MoM, are often described as low. The performing laboratory and treating obstetric team should determine the applicable threshold.
Clinical Indications and Applications
First-Trimester Combined Screening: The principal application is risk assessment for selected fetal chromosomal conditions. PAPP-A should be combined with the other required screening variables rather than interpreted independently.
Dual-Marker Testing: When PAPP-A and free beta-hCG are measured together, the biochemical result can contribute to a first-trimester screening calculation. An ultrasound assessment remains important for accurate gestational dating and nuchal-translucency measurement.
Assessment After a High-Chance Screen: A higher-chance screening result may lead to discussion of cell-free DNA screening or diagnostic procedures such as chorionic villus sampling or amniocentesis. The appropriate option depends on gestational age, local protocol and patient preference.
Low PAPP-A and Placental Surveillance: An isolated low result may prompt closer monitoring of maternal blood pressure and fetal growth. The exact follow-up plan varies according to obstetric history, other risk factors and local maternity guidance.
Multiple Pregnancy: PAPP-A interpretation differs in twin and higher-order pregnancies. Chorionicity and the validated multiple-pregnancy screening algorithm must be considered.
Assisted Conception: In-vitro fertilisation and other assisted-reproduction factors may influence marker interpretation. Accurate details should be supplied to the screening laboratory.
Why Doctors Recommend This Test
First-trimester screening provides information early enough for patients to consider further evaluation and make informed decisions. PAPP-A contributes biochemical information that complements ultrasound findings and maternal characteristics.
The test is a screening tool. A lower PAPP-A level changes a probability estimate but does not mean that the fetus definitely has a chromosomal condition. Similarly, an in-range result cannot guarantee that the fetus is unaffected.
Low PAPP-A can also serve as a placental risk marker. Some patients with very low values have a higher probability of a small-for-gestational-age baby, pre-eclampsia or early delivery. Nevertheless, PAPP-A has limited predictive accuracy when used alone, and many affected pregnancies have normal values.
The obstetrician interprets the result alongside maternal history, blood pressure, ultrasound findings and other markers. Additional monitoring should be individualised rather than based only on one laboratory value.
Preparation Before the Test
Fasting is generally not required. The patient can usually eat, drink and take routine medicines unless another investigation scheduled at the same time requires special preparation.
Accurate pregnancy dating is essential. Provide the first day of the last menstrual period, estimated date of delivery and ultrasound crown-rump length when available. The screening laboratory also needs the date and time of specimen collection.
Inform the centre about maternal date of birth, weight, ethnicity, smoking status, diabetes, assisted conception, number of fetuses and relevant previous pregnancy history. These details may influence the risk calculation.
Provide information about vanishing twin pregnancy, recent blood transfusion, organ transplantation or other factors that could affect screening interpretation. The obstetrician or fetal-medicine specialist will determine whether the standard algorithm is appropriate.
Do not take or stop any medicine specifically to alter PAPP-A. If the test is being used in a defined screening programme, follow its gestational-age collection window carefully.
Blood Sample Collection Procedure
A trained phlebotomist collects a small blood sample from a vein, usually in the arm. The skin is cleaned, a tourniquet is applied and a sterile needle is inserted into the selected vein.
Blood is collected into the laboratory-specified tube. The procedure usually takes a few minutes and does not directly contact the fetus. A brief sting, minor bruising or temporary light-headedness may occur.
The sample is labelled with patient and pregnancy details and transported under appropriate conditions. An insufficient, haemolysed, incorrectly labelled or delayed specimen may need to be recollected.
Laboratory Analysis and Risk Calculation
The laboratory measures PAPP-A using an automated immunoassay. The numerical concentration is converted into a gestational-age-specific MoM and adjusted using the screening programme's validated variables.
When performed as combined screening, the system integrates PAPP-A, free beta-hCG, nuchal-translucency thickness, maternal age and other information to calculate a pregnancy-specific probability for the screened chromosomal conditions.
The report may describe the result as lower chance or higher chance according to the programme's threshold. These terms represent screening probabilities and are not diagnostic conclusions.
A higher-chance result should be discussed with an obstetrician, fetal-medicine specialist or genetic counsellor. Available next steps may include detailed ultrasound assessment, cell-free DNA screening or diagnostic testing.
Cell-free DNA is also a screening test and does not provide a definitive diagnosis. Chorionic villus sampling and amniocentesis analyse fetal or placental genetic material and are considered diagnostic, but they are invasive and require informed counselling.
Normal Reporting Time
The report for the PAPP-A Blood Test is generally available within 1 to 2 working days after the laboratory receives an acceptable sample and complete pregnancy information.
A final combined-screening risk calculation may take longer when the laboratory is waiting for the nuchal-translucency measurement or other required details. Reporting time can also vary according to laboratory workflow.

Understanding the Test Results
PAPP-A Near the Expected Median: A result near 1.0 MoM is close to the gestational-age-specific median. It must still be combined with the other screening variables to calculate risk.
Low PAPP-A: A reduced value can increase the calculated probability of selected chromosomal conditions and may be associated with impaired placental function. It does not independently diagnose a fetal or placental disorder.
Very Low PAPP-A: A markedly reduced value may prompt fetal-medicine review and closer pregnancy surveillance. The management plan depends on the combined screening result and other maternal or fetal risk factors.
Higher-Chance Combined Result: This means that the calculated probability is above the programme threshold. Further screening or diagnostic testing should be discussed, but the result does not confirm that the fetus is affected.
Clinical Limitations
PAPP-A cannot be interpreted reliably without gestational age and relevant maternal information. A raw concentration should not be labelled normal or abnormal using a general non-pregnancy reference range.
The test does not diagnose Down syndrome, Edwards syndrome, Patau syndrome, pre-eclampsia or fetal growth restriction. False-positive and false-negative screening results are possible.
Incorrect gestational dating, inaccurate maternal details, multiple pregnancy, vanishing twin, assisted conception and specimen-related factors may affect the calculated risk.
A normal PAPP-A result cannot exclude all chromosomal, genetic or structural conditions. Prenatal ultrasound and routine antenatal care remain necessary regardless of the screening result.
Important Safety Information
A higher-chance result should be reviewed with an obstetrician or genetic counsellor before decisions are made. Do not assume that a screening result is a diagnosis, and do not make pregnancy-management decisions based on PAPP-A alone.
Patients with low PAPP-A should follow the monitoring plan recommended by their maternity team. Seek urgent obstetric assessment for vaginal bleeding, severe abdominal pain, severe headache, visual disturbance, sudden swelling, reduced fetal movement or other concerning pregnancy symptoms.
