However, no single symptom or antibody result is sufficient to establish the diagnosis.

The standard laboratory evaluation generally focuses on lupus anticoagulant testing, anticardiolipin IgG and IgM antibodies, and anti-beta-2 glycoprotein I IgG and IgM antibodies. These established investigations are interpreted with compatible clinical findings and, where applicable, repeated testing to demonstrate antibody persistence.

A Supplementary, Non-Criteria Antibody

Phosphatidylcholine IgG is generally regarded as a supplementary or non-criteria antiphospholipid antibody. Evidence regarding its independent clinical significance remains limited and may vary according to the population, assay and associated antibody profile. The test should not replace standard APS investigations.

A positive phosphatidylcholine IgG result may add information to a broader clinical assessment, particularly when ordered by a rheumatologist, haematologist, neurologist, immunologist or high-risk pregnancy specialist. Nevertheless, it does not prove that a patient currently has a blood clot, will develop a clot or meets classification requirements for APS.

Sample Required

The investigation requires a serum sample. A healthcare professional collects venous blood, usually from a vein in the arm, into an appropriate collection tube. The blood is allowed to clot and is processed to separate the serum. The serum is then analysed using the method selected by the performing laboratory.

The collection procedure is brief and generally carries only minor risks. Some individuals may experience temporary discomfort, bruising, light-headedness or slight bleeding at the puncture site. Serious complications from routine venous blood collection are uncommon.

Preparation for the Test

Fasting is generally not required for an isolated phosphatidylcholine IgG antibody test. Patients may usually eat, drink water and take their regular medicines unless their doctor provides different instructions. Because this test may be ordered with other blood investigations that require fasting, patients should confirm the complete preparation requirements with the laboratory.

Inform the clinician about prescription medicines, anticoagulants, immune-suppressing treatment, recent infections, vaccinations, pregnancy and previously diagnosed autoimmune conditions. Do not stop anticoagulants, aspirin, steroids or any other prescribed medicine solely for this blood test unless the treating doctor specifically instructs you to do so.

How Is the Test Performed?

The serum is tested for binding between the patient's IgG antibodies and phosphatidylcholine antigen. An immunoassay technique may be used, although the exact analytical platform can vary. The measured signal is compared with calibrators and laboratory controls to produce a quantitative or qualitative result.

Assay standardisation for specialised non-criteria antiphospholipid antibodies may be less uniform than for established APS tests. This is one reason results should be interpreted using the laboratory-specific reference interval and in consultation with a clinician familiar with the patient's complete antibody profile.

Understanding the Results

Negative Result

A negative result means that phosphatidylcholine-reactive IgG antibodies were not detected above the laboratory's specified threshold. It does not exclude antiphospholipid syndrome, systemic lupus erythematosus, another autoimmune disease or a thrombotic disorder.

If APS is clinically suspected, the clinician may still request lupus anticoagulant, anticardiolipin IgG and IgM, and anti-beta-2 glycoprotein I IgG and IgM testing. The diagnostic assessment may also include imaging, coagulation studies, blood counts and investigations for alternative causes of thrombosis or pregnancy morbidity.

Borderline Result

A borderline value lies close to the laboratory cut-off and may have uncertain significance. Minor analytical variation, transient immune activity or an evolving antibody response may contribute to such a result. The clinician may recommend repeat testing after an appropriate interval or correlation with other antiphospholipid antibodies.

Positive Result

A positive result indicates that IgG antibodies reacting with phosphatidylcholine were detected above the laboratory threshold. It does not show where a blood clot is located, establish that a clot exists or confirm the cause of a previous clinical event.

Clinical significance is greater when an antibody finding is persistent and accompanies a compatible medical history or other relevant antiphospholipid antibodies. An isolated positive result in an individual without thrombosis, defined pregnancy morbidity or another supporting clinical feature may have uncertain importance.

Will Repeat Testing Be Required?

Repeat testing may be considered when the clinician needs to determine whether an antibody finding is persistent or transient. Standard APS laboratory assessment often requires qualifying antibodies to remain positive on separate occasions at least 12 weeks apart. However, phosphatidylcholine IgG is not itself a standard classification antibody, so the need and timing for repetition must be determined individually.

A doctor may avoid interpreting results obtained during an acute infection or temporary inflammatory illness as evidence of a persistent autoimmune antibody profile. Repeating the same assay through the same laboratory can make comparison easier, although the clinician may choose a different or more established investigation depending on the clinical question.

Important Limitations

This test should not be used alone to diagnose APS, systemic lupus erythematosus, multiple sclerosis, cardiovascular disease or another autoimmune condition. Research has explored possible associations between phosphatidylcholine-reactive antibodies and different diseases, but an association does not establish that the antibody caused a patient's symptoms or complications.

A positive result does not automatically justify aspirin, anticoagulant treatment or immune-suppressing medication. These treatments can carry significant risks and must be based on a confirmed clinical indication. Similarly, a negative result should not be used to discontinue prescribed treatment or end an investigation when clinical suspicion remains high.

When to Seek Urgent Medical Care

This antibody test is not designed to diagnose an acute blood clot. Seek immediate medical attention for sudden chest pain, difficulty breathing, coughing blood, swelling or pain in one leg, sudden weakness on one side, facial drooping, speech difficulty, severe unexplained headache, loss of vision or another possible symptom of thrombosis or stroke. Do not wait for an antibody report when emergency symptoms are present.

Clinical Interpretation Note

Phosphatidylcholine IgG antibody results must be interpreted in the context of the patient's clinical history, physical examination, standard APS investigations and other relevant tests. The antibody is best regarded as a supplementary laboratory marker.
