Phosphatidyl Ethanolamine IgM Antibody – Serum Test
The Phosphatidyl Ethanolamine IgM Antibody – Serum test detects and measures IgM-class antibodies directed against phosphatidylethanolamine in a blood serum sample. Phosphatidylethanolamine is an important phospholipid found in biological cell membranes, particularly on the inner surface of the membrane lipid bilayer.
Antibodies against phosphatidylethanolamine belong to the broad group of antiphospholipid antibodies. The test may be requested as a complementary investigation in selected patients undergoing evaluation for antiphospholipid syndrome, unexplained arterial or venous thrombosis, recurrent pregnancy complications or another autoimmune condition.
Anti-phosphatidylethanolamine IgM is generally regarded as a non-criteria or supplementary antiphospholipid antibody. It is not one of the principal laboratory markers used independently to classify antiphospholipid syndrome. A positive result does not establish the diagnosis, while a negative result does not exclude it.
Interpretation requires the patient's clinical history and established antiphospholipid tests, particularly lupus anticoagulant, anticardiolipin IgG and IgM, and anti-beta-2 glycoprotein I IgG and IgM. Persistent antibody positivity and a compatible clinical event are important when antiphospholipid syndrome is being considered.
What Is Phosphatidylethanolamine?
Phosphatidylethanolamine is a naturally occurring phospholipid and a major structural component of cell membranes. It is found predominantly on the inner, cytoplasmic surface of the membrane lipid bilayer and contributes to membrane shape, flexibility, fusion and cellular signalling.
Unlike cardiolipin and some other negatively charged phospholipids, phosphatidylethanolamine is considered electrically neutral under physiological conditions. The immune system may nevertheless produce antibodies that react with phosphatidylethanolamine or with complexes formed between this phospholipid and associated proteins.
The clinical role of these antibodies is less clearly established and less standardised than the role of conventional antiphospholipid antibodies. This is why the test is used as additional evidence rather than as a stand-alone diagnostic investigation.
What Is an IgM Antibody?
Immunoglobulin M, abbreviated IgM, is one of the principal antibody classes produced by the immune system. IgM antibodies often appear early during an immune response, although they may also persist or occur naturally without producing disease.
Detection of phosphatidylethanolamine-specific IgM indicates that the serum contains IgM antibodies that bind the test antigen. The result does not explain why the antibodies developed or prove that they are responsible for a blood clot, pregnancy complication or another symptom.
Some IgM antiphospholipid antibodies may appear temporarily during infection, inflammation or immune stimulation. A single positive result may therefore be less clinically meaningful than persistent positivity demonstrated on repeat testing when the specialist considers it appropriate.
What Are Antiphospholipid Antibodies?
Antiphospholipid antibodies are a heterogeneous group of autoantibodies directed against phospholipids, phospholipid-binding proteins or complexes formed between them. They include established laboratory markers as well as several supplementary antibodies that remain under clinical investigation.
The principal laboratory tests used in an antiphospholipid syndrome evaluation include:
- Lupus anticoagulant testing.
- Anticardiolipin IgG and IgM antibodies.
- Anti-beta-2 glycoprotein I IgG and IgM antibodies.
Additional tests may include antibodies against phosphatidylethanolamine, phosphatidylserine-prothrombin complexes, phosphatidylserine, phosphatidylcholine or other targets. These non-criteria antibodies may sometimes be considered when the clinical suspicion is strong but conventional tests are negative or inconclusive.
What Is Antiphospholipid Syndrome?
Antiphospholipid syndrome is an autoimmune disorder associated with arterial, venous or small-vessel thrombosis and defined pregnancy complications. Diagnosis requires a compatible clinical history together with persistent, clinically significant antiphospholipid antibody findings.
The syndrome may occur independently, known as primary antiphospholipid syndrome, or in association with another autoimmune condition, especially systemic lupus erythematosus. Not every person with an antiphospholipid antibody develops a clot or pregnancy complication.
Conventional antiphospholipid antibodies are generally required to be present on at least two occasions separated by an appropriate interval, commonly at least 12 weeks, when persistent positivity is being established. Anti-phosphatidylethanolamine IgM does not independently fulfil the standard laboratory criteria.
Why Is the Phosphatidylethanolamine IgM Test Performed?
A rheumatologist, haematologist, obstetrician, neurologist or another specialist may request the test as part of an extended antiphospholipid antibody evaluation. It is usually ordered for a specific clinical indication and not as routine screening in people without symptoms or risk factors.
Unexplained Thrombosis
The test may be considered when a patient experiences an unexplained deep-vein thrombosis, pulmonary embolism, stroke, myocardial infarction or another arterial or venous thrombotic event, particularly at an unusually young age or without conventional risk factors.
A positive result may provide supplementary evidence of antiphospholipid autoimmunity, but it cannot establish that the antibody caused the clot. Imaging is used to confirm thrombosis, and conventional APS tests remain central to laboratory assessment.
Pregnancy Complications
Antiphospholipid syndrome can be associated with defined pregnancy complications, including recurrent early pregnancy loss, fetal death, severe placental insufficiency, pre-eclampsia and premature delivery in selected clinical circumstances.
Anti-phosphatidylethanolamine antibodies have been studied in recurrent pregnancy loss and implantation failure. However, their clinical value remains less standardised than conventional APS markers. A positive IgM result does not independently explain miscarriage or confirm obstetric antiphospholipid syndrome.
Pregnancy loss has many possible causes, including chromosomal, anatomical, endocrine, genetic, infectious and placental factors. A complete evaluation should be directed by an obstetrician or fertility specialist.
Systemic Autoimmune Disease
People with systemic lupus erythematosus, Behçet disease or another autoimmune disorder may undergo antiphospholipid testing when there is thrombosis, pregnancy morbidity, thrombocytopenia or another relevant clinical feature.
This test may add information to an extended antibody profile, but it does not diagnose systemic lupus erythematosus or another autoimmune disease by itself.
Who May Need This Test?
The test may be considered for patients with unexplained or recurrent thrombosis, selected pregnancy complications, suspected antiphospholipid syndrome, systemic autoimmune disease or a strong clinical suspicion despite negative conventional antiphospholipid antibody results.
It may also be requested when a specialist wants to assess a broader spectrum of phospholipid-reactive antibodies. Testing without a relevant clinical indication can identify incidental antibodies that may not have pathological significance.
Clinical Events That May Lead to Testing
An extended antiphospholipid antibody evaluation may be considered in patients with:
- Unexplained deep-vein thrombosis.
- Pulmonary embolism.
- Stroke or transient ischaemic attacks at an unusually young age.
- Recurrent arterial or venous thrombosis.
- Defined recurrent pregnancy loss.
- Unexplained fetal death.
- Severe pre-eclampsia or placental insufficiency.
- Unexplained thrombocytopenia.
- Livedo reticularis or selected vascular skin changes.
- Systemic lupus erythematosus with possible thrombotic risk.
Sudden chest pain, severe breathlessness, one-sided weakness, difficulty speaking or painful swelling of one leg may indicate an emergency. Immediate medical assessment is required rather than waiting for antibody results.
Sample Requirements
The test requires a venous blood sample. Blood is generally collected into a plain or serum-separator tube. After clotting, the laboratory separates serum from the blood cells and uses it for antibody measurement.
The specimen should be collected, processed and stored according to the laboratory protocol. Gross haemolysis, significant lipaemia, contamination, heat treatment or insufficient volume may make the sample unsuitable, depending on the assay.
Is Fasting Required?
Fasting is generally not required for the Phosphatidyl Ethanolamine IgM Antibody test. Patients may usually follow their regular diet unless another investigation ordered at the same time requires fasting.
Patients should follow the exact preparation instructions provided by Focus Diagnostics or the requesting doctor. Drinking a normal amount of water may make blood collection easier.
Medicines and Clinical Information
Inform the healthcare provider about anticoagulants, aspirin, other antiplatelet medicines, immunosuppressive therapy, recent infections, pregnancy and diagnosed autoimmune diseases. Previous antiphospholipid antibody reports should be provided whenever available.
Do not stop anticoagulants or any other prescribed medicine independently. Although anticoagulants may affect some lupus anticoagulant assays, the effect on solid-phase antibody assays can differ. The clinician should know the full medicine list when interpreting a combined APS evaluation.
How Is the Test Performed?
The laboratory commonly measures phosphatidylethanolamine IgM antibodies using an enzyme-linked immunosorbent assay or another validated immunoassay. The patient's serum is exposed to phosphatidylethanolamine antigen, and bound IgM is detected using a labelled reagent.
The result may be reported as a numerical value with negative, borderline or positive interpretive categories. Units, calibration and decision limits vary between laboratories and assay manufacturers. Results produced using different methods may not be directly interchangeable.
Understanding the Test Result
The result should be evaluated using the method-specific reference interval printed on the laboratory report. Interpretation depends on the strength and persistence of positivity, clinical history and results from conventional antiphospholipid tests.
Positive Phosphatidylethanolamine IgM Result
A positive result means that IgM antibodies reacting with phosphatidylethanolamine were detected above the assay's cutoff. It may provide supplementary evidence of an antiphospholipid immune response in an appropriate clinical setting.
A positive result does not independently diagnose antiphospholipid syndrome, prove that an active clot is present or establish the cause of a pregnancy complication. Some healthy individuals and people with infection or inflammation may have transient or clinically non-specific positivity.
Borderline Result
A borderline result lies close to the assay decision limit. Minor analytical or biological variation may move a result across the cutoff. The doctor may consider repeat testing and review the result with other APS markers.
Treatment should not be started solely because of an isolated borderline anti-phosphatidylethanolamine IgM result.
Negative Result
A negative result means that phosphatidylethanolamine IgM was not detected above the assay's decision limit. It does not exclude antiphospholipid syndrome because the condition is primarily evaluated using lupus anticoagulant, anticardiolipin and anti-beta-2 glycoprotein I antibodies.
A patient may also have another antibody isotype or a non-autoimmune cause of thrombosis or pregnancy morbidity.
Why May Repeat Testing Be Recommended?
Antiphospholipid antibodies can appear temporarily during infections, inflammatory episodes or medication exposure. Repeat testing may help determine whether an initially positive result persists.
When conventional antiphospholipid antibodies are being assessed for APS, persistence is generally evaluated at least 12 weeks apart. The need and timing for repeating a non-criteria phosphatidylethanolamine antibody should be decided by the treating specialist.

Depending on the patient's history, the doctor may request:
- Lupus anticoagulant profile.
- Anticardiolipin IgG and IgM antibodies.
- Anti-beta-2 glycoprotein I IgG and IgM antibodies.
- Phosphatidylethanolamine IgG or IgA antibodies.
- Anti-phosphatidylserine-prothrombin antibodies.
- Complete blood count and platelet count.
- Prothrombin time and activated partial thromboplastin time.
- ANA and additional autoimmune antibody tests.
- Kidney-function tests and urinalysis.
- Doppler ultrasound, CT or other imaging when thrombosis is suspected.
Can the Result Predict a Future Blood Clot?
A single phosphatidylethanolamine IgM result cannot accurately predict whether a person will develop a future blood clot. Risk depends on previous thrombosis, the complete antibody profile, persistence, conventional cardiovascular factors, smoking, immobility, surgery, pregnancy and hormone exposure.
Patients should not start aspirin, heparin, warfarin or another anticoagulant solely because this antibody is positive. Such treatments can cause bleeding and must be prescribed for a clearly defined clinical indication.
Can This Test Diagnose the Cause of Recurrent Miscarriage?
No single antibody test can determine the cause of recurrent pregnancy loss. A positive anti-phosphatidylethanolamine IgM result may prompt the specialist to review the complete antiphospholipid profile, but it does not establish causation.
The evaluation may also include genetic, anatomical, hormonal and other appropriate investigations. Treatment should be individualised and guided by an obstetrician, fertility specialist, rheumatologist or haematologist.
Benefits of the Test
The test requires only a standard serum sample and may detect an additional phospholipid-reactive IgM antibody not included in routine APS panels. It may be useful in carefully selected patients when conventional tests do not fully explain the clinical picture.
Its value is complementary. The result is most useful when reviewed together with established APS markers and a well-documented clinical history.
Limitations of the Test
Anti-phosphatidylethanolamine IgM is a non-criteria antibody and cannot independently establish antiphospholipid syndrome. Clinical relevance is less standardised than for lupus anticoagulant, anticardiolipin and anti-beta-2 glycoprotein I antibodies.
Assay antigen preparation, calibration, units and cutoffs can differ. A positive result may be temporary or non-specific, while a negative result does not exclude APS. The test should not be used as general screening in low-risk individuals.
