Porphobilinogen Quantitative – Urine 24 Hours Test

The Porphobilinogen Quantitative – Urine 24 Hours test measures the total amount of porphobilinogen, commonly abbreviated as PBG, excreted in urine during a complete 24-hour period. Porphobilinogen is an intermediate substance produced during the body’s formation of heme. Heme is an essential component of haemoglobin and several enzymes that support oxygen transport, energy production and normal cellular functions.

Under normal circumstances, enzymes in the heme-production pathway convert porphobilinogen and other intermediate compounds through a carefully regulated sequence. In certain disorders called porphyrias, reduced activity of a pathway enzyme may cause porphobilinogen, aminolevulinic acid or porphyrins to accumulate. Measuring urinary PBG can therefore provide important evidence when an acute hepatic porphyria is suspected.

A 24-hour collection measures PBG excretion across an entire day and may reduce some of the variation associated with a single urine specimen. However, current diagnostic strategies may use a properly handled random urine PBG specimen during active symptoms because it can support faster clinical decisions. The collection type should always be selected by the requesting clinician and performing laboratory.

Why Is the Test Performed?

This test may be requested when a person has signs or symptoms that raise concern for an acute porphyria. Acute porphyrias are uncommon metabolic disorders that primarily affect the nervous system and may cause episodes of severe illness. The principal acute hepatic porphyrias associated with increased urinary PBG include acute intermittent porphyria, hereditary coproporphyria and variegate porphyria.

Symptoms of an acute porphyria may resemble many more common medical conditions. Laboratory testing is therefore important, but no symptom or isolated laboratory value should be used alone to establish the diagnosis.

Clinical Situations in Which Testing May Be Considered

  • Severe or recurrent unexplained abdominal pain.
  • Nausea, vomiting or constipation without a clear cause.
  • Muscle weakness, limb pain, tingling or other neurological symptoms.
  • Anxiety, agitation, confusion, hallucinations or seizures during an unexplained illness.
  • Dark, reddish or brown urine, particularly during symptomatic episodes.
  • Unexplained low blood sodium associated with compatible symptoms.
  • Breathing difficulty or progressive weakness during a suspected acute attack.
  • A personal or family history suggesting an inherited porphyria.
  • Follow-up of a previously abnormal PBG or porphyrin result.

Understanding Porphobilinogen and Porphyria

Porphyrias are a group of inherited or acquired disorders involving enzymes required for heme production. They are broadly described as acute porphyrias, which mainly cause neurological and abdominal symptoms, or cutaneous porphyrias, which mainly produce skin sensitivity to sunlight. Some types, including hereditary coproporphyria and variegate porphyria, may cause both acute neurological episodes and skin symptoms.

Urinary PBG is particularly important when an acute hepatic porphyria is being investigated. Concentrations may rise substantially during an acute attack. The value may decrease between attacks in some patients, so collection timing can affect diagnostic sensitivity. When possible, testing is commonly performed while compatible symptoms are present and before condition-specific treatment alters biochemical findings.

How Is the 24-Hour Urine Sample Collected?

Accurate collection is essential because missing even one urine specimen can make the calculated 24-hour excretion unreliable. The laboratory will provide an approved container and specific instructions. Porphobilinogen and related compounds can be affected by light, temperature, urine pH and delayed transport, so the container may need to be protected from light and kept refrigerated or stored as otherwise directed.

General 24-Hour Collection Procedure

  1. Select a start time, usually in the morning, according to the laboratory’s instructions.
  2. At the start time, empty the bladder into the toilet and do not add this first urine to the container.
  3. Record the exact start date and time.
  4. Collect every urine specimen passed during the following 24 hours in the supplied container.
  5. At the same time on the next day, empty the bladder and add this final urine to the container.
  6. Record the finishing time and confirm that no specimen was missed.
  7. Keep the collection protected from light and at the required temperature throughout the collection period.
  8. Return the container promptly with the requested clinical and medication information.

Do not collect urine directly into a large container if doing so creates a risk of contact with preservatives. A separate clean collection vessel may be provided. Follow the laboratory’s safety instructions carefully.

What If a Sample Is Missed?

If any urine is missed, spilled or discarded during the 24-hour period, contact the laboratory. In many situations, the collection must be restarted with a new container because the reported daily amount would otherwise be incomplete. Do not estimate or add water to replace a missed specimen.

Preparation Before the Test

Preparation requirements vary between laboratories. Some facilities instruct patients to avoid alcohol for at least 24 hours before and during collection. The clinician may also review prescription medicines, non-prescription medicines, hormonal treatments, supplements and recent exposures because certain substances can influence symptoms, interfere with testing or trigger attacks in susceptible individuals.

Do not stop any prescribed medicine without medical advice. If a medication is considered relevant, the clinician will determine whether it should be continued, adjusted or documented for interpretation. Maintain your usual fluid intake unless you receive different instructions. Excessive fluid consumption can dilute urine, while dehydration may produce a more concentrated specimen.

Tell the healthcare team if you are menstruating, have a urinary infection, cannot complete the collection, have recently received intravenous fluids or have a medical condition that affects urine output. These factors may influence sample quality or interpretation.

How Is the Result Interpreted?

The laboratory reports the quantitative PBG result using its validated units and reference interval. In a 24-hour test, the result may be expressed as the total amount excreted per day. Reference limits vary according to the analytical method and laboratory, so the value should be compared only with the range printed on the report.

Elevated Porphobilinogen

A clearly elevated urinary PBG result during compatible symptoms may strongly support an acute hepatic porphyria. The degree of elevation, symptom pattern and accompanying aminolevulinic acid or porphyrin results help guide the next stage of evaluation. Additional testing is usually required to confirm the finding and identify the specific type.

Follow-up may include repeat quantitative urine testing, urine porphyrin fractionation, aminolevulinic acid measurement, plasma fluorescence testing, faecal porphyrin analysis, enzyme studies or molecular genetic testing. The correct combination depends on the suspected porphyria and clinical presentation.

Normal or Low Porphobilinogen

A result within the laboratory’s reference interval makes an acute porphyria less likely when the specimen was collected correctly during active symptoms. However, interpretation requires attention to collection timing, specimen handling, urine dilution and the specific porphyria under consideration. A normal PBG value does not exclude every form of porphyria, particularly conditions that mainly affect the skin and are evaluated with other porphyrin measurements.

If clinical suspicion remains high, the physician may request repeat testing during symptoms or select other blood, urine, stool or genetic tests. Patients should not attempt to exclude or diagnose porphyria based solely on one result.

Factors That Can Affect Test Accuracy

  • Failure to collect every urine specimen during the 24-hour period.
  • Incorrect recording of the collection start or finish time.
  • Exposure of the sample to light.
  • Improper temperature during collection, storage or transport.
  • Delayed delivery to the laboratory.
  • Use of an incorrect container or preservative.
  • Very dilute urine caused by excessive fluid intake.
  • Collection outside an active symptomatic episode.
  • Certain medicines, supplements, alcohol or chemical exposures.
  • Contamination with stool, toilet water, menstrual blood or cleaning products.

Limitations of the Test

Urinary PBG testing is an important biochemical tool, but it does not independently identify every porphyria or determine the exact genetic cause. A 24-hour result may also delay assessment when a patient is experiencing a potentially serious acute attack. For this reason, clinicians may prefer a rapid random urine PBG measurement during active symptoms and use additional testing as appropriate.

The result should be correlated with the patient’s symptoms, physical examination, medical history, medication exposure, family history and other laboratory findings. Mild or borderline abnormalities may require confirmation, while a significant elevation requires prompt professional evaluation.

Safety of the Test

Urine collection is non-invasive and has no direct physical risk. Practical risks relate mainly to handling a container that may include a preservative. Do not touch or discard any preservative, and keep the container away from children. If the preservative contacts the skin or eyes, follow the laboratory’s safety instructions and seek appropriate assistance.

When Is Urgent Medical Care Needed?

Suspected acute porphyria attacks can become serious. Seek urgent medical assessment for severe or worsening abdominal pain accompanied by weakness, confusion, seizures, fainting, breathing difficulty, persistent vomiting or a rapid change in mental state. These symptoms have many possible causes and should not be assumed to represent porphyria without medical evaluation.

Important Information for Patients

A positive screening result is not the final diagnosis, and a negative result does not exclude all porphyria types. Specialist review may be recommended after an abnormal result. Genetic counselling may also be appropriate when an inherited porphyria is confirmed because certain biological relatives may benefit from targeted evaluation.
