Prealbumin Blood Test
The Prealbumin blood test measures the concentration of prealbumin in a blood sample. Prealbumin is a protein produced mainly by the liver. It is also known as transthyretin because it transports thyroid hormones, particularly thyroxine, and helps carry vitamin A through the bloodstream by binding to retinol-binding protein.
Prealbumin has historically been used to support nutritional assessment, especially in hospitalised or seriously ill patients. Because it has a relatively short half-life, its concentration can change more quickly than some other blood proteins. However, prealbumin is also strongly affected by inflammation, infection, trauma, liver function, kidney function and other medical conditions.
A low prealbumin result does not independently diagnose malnutrition. Similarly, a result within the laboratory reference interval does not prove that nutritional status is adequate. The test should be interpreted alongside clinical history, dietary intake, weight changes, physical examination, inflammatory markers and other laboratory investigations.
What Is Prealbumin?
Prealbumin is the older name for transthyretin. The name was based on its position before albumin during certain laboratory protein-separation procedures. Despite the similar names, prealbumin and albumin are different proteins with different structures, functions and biological half-lives.
The liver produces most circulating transthyretin. Its primary transport functions involve thyroid hormones and retinol-binding protein, which carries vitamin A. Prealbumin has a shorter circulating half-life than albumin, so its concentration may change relatively quickly in response to acute illness, inflammation or altered protein synthesis.
Prealbumin is described as a negative acute-phase reactant. This means that its blood concentration commonly decreases during inflammation, infection, trauma or tissue injury, even when inadequate nutritional intake is not the principal cause. This characteristic is one reason the test must not be interpreted as a stand-alone marker of nutrition.
Why Is the Prealbumin Test Performed?
A healthcare professional may request the test as one component of a broader nutritional and medical assessment. It may be considered for patients who are seriously ill, recovering from major surgery, receiving nutritional support or at increased risk of protein-energy malnutrition.
The test may also be used in selected patients receiving enteral nutrition through a feeding tube or parenteral nutrition through a vein. Serial results can sometimes contribute to monitoring, but changes must be assessed alongside inflammation, fluid status, clinical progress and actual nutritional intake.
Additional reasons for testing may include unexplained weight loss, muscle wasting, weakness, poor wound healing, chronic digestive disease, suspected protein loss or liver dysfunction. A clinician may also request prealbumin before surgery as part of an overall risk or nutritional assessment.
Who May Need This Test?
People who have unintentionally lost weight, have a reduced appetite, struggle to eat adequately or have symptoms of malabsorption may require nutritional assessment. Prealbumin may be included when the clinician believes it will add useful information.
Hospitalised patients with severe infection, major burns, trauma, cancer, chronic organ disease or prolonged recovery may be at increased risk of nutritional depletion. However, these same conditions can cause inflammation-related reductions in prealbumin, making careful interpretation essential.
Patients receiving dialysis or living with chronic kidney disease may undergo specialised nutritional evaluation. Kidney dysfunction can alter prealbumin levels and may make results difficult to interpret without kidney-function testing. A dietitian or kidney specialist may use several clinical and laboratory indicators rather than relying on one protein measurement.
Children with concerns about growth or inadequate nutrient intake may also require nutritional assessment. Paediatric results should be interpreted using age-appropriate reference intervals and in combination with growth charts, dietary history and clinical examination.
Symptoms That May Suggest Nutritional Risk
Symptoms of inadequate nutrition can include unintentional weight loss, reduced muscle strength, fatigue, weakness, dizziness, feeling cold, dry skin, brittle hair, hair loss, poor wound healing or reduced ability to recover from illness. Children may show poor growth or delayed development.
These symptoms are not specific to malnutrition. They may also occur with anaemia, thyroid disease, infection, cancer, digestive disorders, kidney disease, liver disease or other medical conditions. Laboratory testing and professional evaluation are needed to determine the cause.
Sample Required and Collection Procedure
The Prealbumin test generally requires a serum sample collected from a vein. A healthcare professional cleans the collection site, usually inside the elbow, inserts a sterile needle and collects blood into an appropriate tube. The procedure normally takes only a few minutes.
Patients may feel a brief sting when the needle enters or leaves the vein. Minor bruising, slight bleeding or temporary light-headedness can occur. Serious complications from routine venous blood collection are uncommon.
The laboratory separates serum from the blood cells and measures prealbumin using a validated analytical method, commonly an immunochemical assay. The required tube, sample volume, processing conditions and storage requirements depend on the performing laboratory.
Is Fasting Required for the Prealbumin Test?
Special preparation is not usually required solely for a Prealbumin test. Fasting may be necessary if other blood investigations are ordered at the same time. Patients should follow the instructions provided by the doctor or diagnostic centre.
All prescription medicines, non-prescription medicines, vitamins and supplements should be reported. Certain medicines and treatments can affect prealbumin results. Patients must not stop or change any treatment unless instructed by their healthcare professional.
Understanding the Prealbumin Result
Prealbumin is commonly reported in milligrams per decilitre or another laboratory-specific unit. Reference intervals vary according to method, age and laboratory. The range printed on the individual report should be used for initial interpretation.
A result below the reference interval may suggest decreased protein synthesis, increased protein use or loss, inflammation, serious illness or inadequate nutritional intake. It does not reveal which of these factors is responsible. Clinical correlation and additional testing are required.
A result above the reference interval is less common and is not usually interpreted as evidence of excessive nutrition. Higher values may occur in some patients with kidney dysfunction, during high-dose corticosteroid therapy or in association with other medical conditions.
A single prealbumin measurement provides information from one point in time. When repeat testing is clinically appropriate, the trend may be more useful than one isolated value. Even then, changes in inflammation, hydration, organ function and treatment must be considered.
What Can Cause Low Prealbumin?
Inadequate protein or energy intake can contribute to a low result, particularly when supported by weight loss, loss of muscle mass, reduced dietary intake and other clinical findings. Malabsorption or digestive disease may also limit the body's ability to absorb nutrients.
Inflammation is a major cause of reduced prealbumin. During infection, trauma, burns or other acute illness, the liver changes its pattern of protein production. Prealbumin may fall as part of this acute-phase response even if nutritional intake has not changed significantly.
Liver disease can reduce prealbumin because the liver produces the protein. The degree of reduction does not, by itself, determine the type or severity of liver disease. Liver enzymes, bilirubin, albumin, clotting tests and clinical findings may be needed.
Protein-losing kidney or intestinal disorders may also contribute to low blood protein concentrations. Hyperthyroidism, malignancy and serious chronic illness can affect the result through changes in metabolism, inflammation or protein balance.
Possible Causes of a Low Result
- Inadequate protein or calorie intake.
- Inflammation or an acute-phase response.
- Serious infection or sepsis.
- Major trauma, surgery or burn injury.
- Chronic or severe liver disease.
- Protein-losing kidney disease.
- Protein-losing digestive or intestinal disorders.
- Malabsorption and chronic digestive disease.
- Hyperthyroidism or increased metabolic demand.
- Cancer or another serious chronic illness.
The presence of one of these conditions does not guarantee that prealbumin will be low. The result must be evaluated in the context of the patient's complete medical condition.
What Can Cause High Prealbumin?
A high prealbumin result is generally less common than a low result. It may occur in some patients with reduced kidney function because clearance and protein metabolism can be altered. Kidney-function tests such as serum creatinine and estimated glomerular filtration rate can help with interpretation.
High-dose corticosteroid treatment may increase prealbumin in some circumstances. Increased results have also been reported in association with certain haematological conditions, including Hodgkin lymphoma, but the test cannot diagnose these disorders.
Dehydration may concentrate blood components and should be considered when interpreting various protein measurements. A high result should be reviewed with the patient's fluid status, medicines, kidney function and other laboratory findings.
Prealbumin and Nutritional Assessment
Prealbumin was previously considered a sensitive laboratory marker of recent nutritional change because of its short half-life. Modern interpretation recognises that inflammation and disease severity can influence the concentration as much as, or more than, nutrient intake.
Some healthcare professionals continue to use prealbumin as one part of nutritional assessment in selected hospitalised or seriously ill patients. Others do not use it routinely to diagnose malnutrition because a low result may primarily reflect inflammation, infection or injury.
A proper nutritional assessment may include recent weight changes, body mass index, muscle mass, loss of body fat, appetite, dietary intake, swallowing ability, digestive symptoms, functional status and the findings of a physical examination. A dietitian may also evaluate calorie, protein, vitamin and mineral intake.
Laboratory investigations may include C-reactive protein or another inflammatory marker, albumin, total protein, complete blood count, electrolytes, kidney function, liver function, glucose, iron studies and selected vitamin or mineral tests. Not every patient requires every test.
Why Inflammation Matters
During inflammation, the liver increases the production of certain acute-phase proteins and reduces the production of negative acute-phase proteins such as prealbumin. As a result, prealbumin may decrease rapidly during acute illness regardless of the amount of protein consumed.
If inflammation is resolving, prealbumin may rise as the acute-phase response improves. This increase may occur alongside nutritional treatment but does not necessarily prove that nutrition alone caused the change. Reviewing prealbumin with C-reactive protein and clinical progress may provide better context.
Healthcare professionals should avoid increasing nutritional support solely to correct a low prealbumin number without evaluating the patient's needs and medical condition. Excessive feeding can also cause complications in vulnerable patients.
Prealbumin Versus Albumin
Prealbumin and albumin are separate proteins made by the liver. Albumin helps maintain fluid distribution and transports many substances in the blood. Prealbumin primarily transports thyroid hormone and retinol-binding protein.
Prealbumin has a shorter half-life and may change more rapidly. Albumin has a longer half-life and is affected by liver disease, kidney disease, inflammation, fluid balance and protein loss. Neither test should be used alone to diagnose malnutrition.
Despite its name, prealbumin is not an immature form or direct precursor of albumin. Testing one cannot replace testing the other when a clinician requires information about both proteins.

Additional investigations may include C-reactive protein, albumin, total protein, liver enzymes, bilirubin, kidney-function tests, urine protein, thyroid tests, complete blood count, iron studies or selected vitamin and mineral tests. Testing should be guided by symptoms and clinical findings.
If prealbumin is being monitored during hospital treatment or nutritional support, repeat testing may be considered. The interval should be determined by the healthcare team. Small short-term changes should not be interpreted without considering inflammation, hydration and overall clinical progress.
Benefits and Limitations
The test is a simple blood investigation and can provide useful information about a liver-produced transport protein. Its relatively rapid response to changes in illness may help clinicians observe trends in selected patients.
Its major limitation is lack of specificity. A low result can arise from inadequate nutrition, inflammation, infection, trauma, liver disease or protein loss. The test cannot determine dietary protein intake, diagnose a specific vitamin deficiency or identify the precise cause of weight loss.
Prealbumin should not be used as the only basis for diagnosing malnutrition, prescribing nutritional support or judging recovery. Clinical assessment remains essential.
When Should Medical Advice Be Sought?
Consult a healthcare professional for unexplained weight loss, persistent loss of appetite, progressive weakness, muscle wasting, swallowing difficulty, prolonged diarrhoea, repeated vomiting or poor wound healing. Early assessment can help identify nutritional and medical causes.
Urgent medical care may be necessary for severe dehydration, confusion, fainting, inability to eat or drink, breathing difficulty or rapid deterioration during a serious illness. A Prealbumin test should not delay emergency treatment.
