Progesterone Test

The Progesterone test measures the concentration of progesterone in a blood sample. Progesterone is an important steroid hormone involved in ovulation, the menstrual cycle, fertility and pregnancy. In women who menstruate, progesterone is mainly produced by the corpus luteum in the ovary after ovulation. During pregnancy, the placenta becomes an important source of progesterone. Smaller amounts are also produced by the adrenal glands and testes.

Progesterone levels naturally change throughout the menstrual cycle. They are generally low during the follicular phase, increase after ovulation and remain elevated during the luteal phase. If pregnancy does not occur, the progesterone level falls and menstruation begins. If pregnancy occurs, progesterone helps maintain the uterine lining and supports the developing pregnancy.

A doctor may order this test to determine whether ovulation has occurred, evaluate menstrual irregularities, investigate possible causes of infertility, assess corpus luteum function or monitor progesterone levels during early pregnancy and fertility treatment. Why Is the Progesterone Test Performed?

The Progesterone test is commonly performed as part of a fertility or reproductive-hormone evaluation. The test does not predict the exact day of ovulation with complete certainty, and one result may not fully describe progesterone production throughout the luteal phase.

Role of Progesterone in the Menstrual Cycle

The menstrual cycle is controlled by coordinated changes in several hormones. During the first part of the cycle, follicles develop within the ovary. A surge in luteinising hormone helps trigger ovulation, during which a mature egg is released. The remaining follicle becomes the corpus luteum and begins producing progesterone.

Progesterone prepares the endometrium, or uterine lining, for possible implantation of a fertilised egg. If implantation does not occur, the corpus luteum breaks down, progesterone falls and the uterine lining is shed during menstruation. If pregnancy occurs, hormonal signals maintain progesterone production until the placenta can produce sufficient amounts.

Who May Need a Progesterone Test?

The test may be considered for women undergoing an infertility evaluation, those with irregular or absent menstrual periods, patients receiving ovulation-induction treatment and individuals whose doctors need to assess luteal-phase hormone production. It may also be ordered during pregnancy when clinically appropriate.

Progesterone measurement can occasionally be used in men or children when a specialist is investigating an adrenal, gonadal or steroid-production disorder. In these circumstances, it is usually interpreted with several other hormone measurements rather than used alone.

What Sample Is Required?

A venous blood sample is required. Serum is commonly preferred, although certain laboratories may accept an appropriate plasma sample. The exact collection tube and specimen-processing requirements depend on the testing platform used by the laboratory.

The blood sample should be collected on the date recommended by the doctor. Timing is particularly important when the purpose is to assess ovulation. The collection centre should be informed about the patient's menstrual-cycle dates, pregnancy status and hormone medicines when relevant.

When Should the Test Be Taken?

How Is the Blood Sample Collected?

A trained phlebotomist collects blood from a vein, usually in the arm. The skin is cleaned, a sterile needle is inserted and blood is collected into the appropriate tube. The needle is removed after collection and pressure is applied to the puncture site.

The procedure usually takes only a few minutes. Temporary discomfort, mild bruising, light-headedness or minor bleeding may occur. Serious complications are uncommon. Patients taking blood-thinning medicines or those with a bleeding disorder should inform the collection staff.

How Is Progesterone Measured?

What Can a High Progesterone Level Mean?

A higher progesterone level in a woman who is not pregnant commonly indicates that ovulation has recently occurred and that the corpus luteum is functioning. Progesterone naturally rises during pregnancy and changes as pregnancy progresses. Prescribed progesterone or fertility treatment can also increase the measured concentration.

Less commonly, an elevated result may be associated with certain ovarian cysts, adrenal conditions or steroid-enzyme abnormalities. A high value cannot identify the cause by itself. Interpretation requires the clinical context and, when appropriate, other hormone tests or imaging.

What Can a Low Progesterone Level Mean?

A low result may be normal if the sample was collected before ovulation, during the early follicular phase, after menopause or at an inappropriate point in the cycle. If the test was correctly timed during the expected luteal phase, a low concentration may suggest that ovulation did not occur or that progesterone production was lower than expected.

One low value does not independently diagnose infertility, luteal-phase deficiency, miscarriage or an abnormal pregnancy. Progesterone can fluctuate, and an incorrectly timed sample can produce a misleading result. The doctor may recommend repeat testing, ultrasound or additional reproductive-hormone investigations.

Factors That Can Affect the Result

  • Irregular menstrual cycles or uncertain ovulation timing.

Progesterone Testing and Fertility

Progesterone testing can provide supporting evidence that ovulation occurred, but it does not measure ovarian reserve and cannot identify every cause of infertility. Fertility can be affected by age, egg quality, fallopian-tube factors, uterine conditions, semen parameters, thyroid disorders, prolactin abnormalities and several other factors.
