Free PSA (FPSA) Test
The Free PSA (FPSA) test is a blood test that measures the amount of prostate-specific antigen circulating freely, or unbound, in the bloodstream. Prostate-specific antigen is a protein produced mainly by cells in the prostate gland. PSA enters the blood in two principal forms: PSA attached to certain blood proteins and PSA that remains unattached. The unattached form is known as free PSA.
A free PSA result is usually interpreted together with a total PSA result. Total PSA includes both bound and free forms of the protein. Comparing free PSA with total PSA allows the laboratory or healthcare professional to calculate the percentage of free PSA. This percentage may provide additional information when a total PSA result is borderline or moderately increased.
The Free PSA test does not independently diagnose prostate cancer. PSA levels can be affected by several cancerous and noncancerous conditions, including benign prostate enlargement, prostatitis, urinary infection and recent prostate-related procedures. Results must therefore be interpreted by a qualified healthcare professional alongside total PSA, age, symptoms, risk factors, physical examination findings and previous laboratory results.
What Is Prostate-Specific Antigen?
Prostate-specific antigen is an enzyme produced predominantly by prostate gland cells. Its normal biological function is associated with semen, but a small amount enters the bloodstream. Blood PSA can be measured using sensitive laboratory immunoassays.
PSA is prostate-specific but not cancer-specific. This means that an increased PSA concentration indicates activity or disturbance involving prostate tissue but does not reveal the exact cause. Prostate cancer can increase PSA, but so can benign prostatic hyperplasia, prostate inflammation, infection, urinary retention and recent manipulation of the prostate.
In blood, PSA exists in protein-bound and free forms. Total PSA measures both forms together, while the FPSA test measures only the unbound portion. The relationship between these values may help distinguish between patterns more commonly associated with benign prostate disease and patterns that may require further investigation.
Why Is the Free PSA Test Performed?
A doctor may request free PSA when a total PSA result is within an intermediate or diagnostic grey zone. In this situation, total PSA alone may not provide enough information to estimate whether the increase is more likely to be related to a benign prostate condition or prostate cancer. Free PSA can contribute additional information to this risk assessment.
Benign prostate conditions are generally associated with a greater proportion of PSA circulating in the free form. Prostate cancer may be associated with a smaller free proportion and a greater proportion attached to blood proteins. Therefore, a lower percent free PSA may correspond to a higher relative likelihood of prostate cancer, while a higher percentage may suggest a lower relative likelihood.
This relationship is not absolute. Some patients without cancer may have a low free PSA percentage, while some patients with cancer may have a higher percentage. The test must not be used as a stand-alone reason to confirm or exclude cancer, recommend surgery or make treatment decisions.
Who May Need a Free PSA Test?
The test may be considered for people with a borderline or moderately increased total PSA result. It may also be requested when a doctor is evaluating urinary symptoms, an enlarged prostate, an abnormal digital rectal examination, a family history of prostate cancer or another recognised risk factor.
Possible urinary symptoms include difficulty beginning urination, a weak or interrupted urine stream, frequent urination, urgency, waking repeatedly at night to urinate or feeling that the bladder has not emptied completely. These symptoms are commonly caused by benign prostate enlargement, but they still require appropriate medical assessment.
Early prostate cancer frequently produces no noticeable symptoms. PSA testing may therefore be discussed as part of an informed screening decision. Whether screening is appropriate depends on factors such as age, family history, overall health, individual risk and personal preferences. Screening can have both benefits and possible harms, including false-positive results, unnecessary investigations and detection of slow-growing cancers that might never cause illness.
How Is Percent Free PSA Calculated?
Percent free PSA is generally calculated by dividing the free PSA concentration by the total PSA concentration obtained from the same blood sample and multiplying the result by 100. For example, if free PSA is 1 ng/mL and total PSA is 5 ng/mL, the calculated percent free PSA is 20%.
The calculation is meaningful only when compatible free and total PSA measurements are available. Ideally, both tests should be performed using the same sample and compatible analytical methods. A free PSA result without a corresponding total PSA value may have limited clinical usefulness.
Laboratories may calculate the percentage automatically or report free and total PSA separately. Some laboratories provide percent free PSA only when total PSA falls within a particular range. The interpretation policy depends on the analytical platform, clinical validation and laboratory protocol.
Sample Required and Collection Procedure
The test requires a venous blood sample. A trained phlebotomist cleans the collection site, usually on the inside of the elbow, inserts a sterile needle into a vein and collects blood into an appropriate tube. The procedure normally takes only a few minutes.
Some people may experience brief discomfort, minor bruising, light-headedness or slight bleeding at the puncture site. Serious complications from routine blood collection are uncommon. Inform the collection team if you have a history of fainting during blood tests, take blood-thinning medicines or have a bleeding disorder.
The laboratory separates serum from the collected blood and measures free PSA using an immunoassay. The result is generally reported in nanograms per millilitre. Reporting units, measuring ranges and methods may vary between laboratories.
Is Fasting Required for the Free PSA Test?
Fasting is generally not required specifically for the Free PSA test. Patients may normally eat and drink unless the doctor has ordered other investigations requiring fasting. Always follow the preparation instructions provided by the laboratory or healthcare professional.
Patients should report all prescription medicines, over-the-counter medicines, vitamins and supplements. Medicines should not be stopped or changed without instructions from the treating doctor. Accurate information about medicines and recent medical procedures helps the clinician interpret the result correctly.
Preparation Before Sample Collection
Certain conditions and activities may temporarily affect PSA measurements. Patients should inform their doctor about a recent urinary infection, prostatitis, fever, urinary retention, catheterisation, cystoscopy, prostate biopsy, prostate surgery or another urinary-tract procedure. Testing may need to be postponed until the temporary influence has resolved.
Ejaculation and vigorous activities that place pressure on the prostate, such as prolonged cycling, may temporarily influence PSA in some individuals. The healthcare professional may advise avoiding these activities for a specific period before testing. Patients should follow the instructions given for their clinical situation.
Blood collection may be scheduled before a digital rectal examination or significant prostate manipulation when both are planned. A routine digital rectal examination usually has a limited effect on PSA, but consistent sample timing can help reduce avoidable variation.
Medicines such as finasteride and dutasteride can lower total PSA measurements. These medicines may be prescribed for benign prostate enlargement or hair loss. Testosterone therapy and other treatments may also influence prostate assessment. The doctor must know about these medicines when reviewing the report.
Understanding the Free PSA Result
A free PSA value should not normally be interpreted independently. Its clinical value comes mainly from comparison with the corresponding total PSA concentration and calculation of percent free PSA. The laboratory report may include both the measured free PSA and the calculated percentage.
In general, a lower percent free PSA is associated with a higher relative probability of prostate cancer, while a higher percentage is more often associated with benign prostate conditions. However, no single percentage can definitively confirm or exclude cancer. Cutoffs and estimated probabilities vary according to age, total PSA concentration, laboratory method and the population used to establish the guidance.
The doctor should use the interpretation ranges supplied by the performing laboratory. Patients should not compare results with unrelated online ranges because laboratories may use different analysers, antibodies, calibration systems and reporting policies.
A result within a favourable range cannot guarantee that cancer is absent. Similarly, a result in a higher-risk range does not mean that cancer is definitely present. Further evaluation is based on the complete clinical picture rather than the free PSA result alone.
What Can Cause an Abnormal PSA Pattern?
Prostate cancer is one possible cause of an abnormal total and free PSA pattern, but many noncancerous factors can influence the measurements. Benign prostatic hyperplasia is a common age-related enlargement of the prostate and can increase PSA. Prostatitis and urinary tract infections may also cause significant temporary elevations.
Other influences include urinary retention, catheterisation, cystoscopy, prostate biopsy, prostate surgery, recent ejaculation, vigorous cycling and natural biological variation. The time between these events and sample collection can affect the result.
Improper sample handling, unsuitable storage and differences between testing platforms can also affect comparability. When PSA is monitored over time, testing at the same laboratory using the same method may provide more consistent results.
Factors That Can Affect Test Interpretation
- A missing or incompatible total PSA result.
- Benign enlargement of the prostate gland.
- Prostatitis or a urinary tract infection.
- Recent urinary retention or catheterisation.
- Recent prostate biopsy, surgery or manipulation.
- Ejaculation or prolonged cycling before testing.
- Finasteride, dutasteride and other relevant medicines.
- Differences between laboratory methods and calibration systems.
- Natural day-to-day biological variation in PSA levels.
- Incomplete information about symptoms, age or medical history.
What Happens After an Abnormal Result?
An abnormal free PSA or percent free PSA result does not automatically mean that a biopsy is required. The healthcare professional may review temporary causes, treat an infection if present or repeat total and free PSA after an appropriate interval. Repeating the test under standardised conditions may help determine whether the result persists.
Further assessment may include a digital rectal examination, urine tests, urology consultation, prostate volume assessment, multiparametric MRI, another validated biomarker test or an individualised prostate cancer risk calculator. The appropriate next step depends on age, symptoms, total PSA, percent free PSA, previous results, family history and examination findings.
A prostate biopsy may be recommended when the overall assessment suggests a clinically important risk. Biopsy is used to confirm or exclude cancer by examining prostate tissue. It carries potential risks, including bleeding, infection, blood in the urine or semen and temporary difficulty urinating. The benefits and risks should be discussed with a urologist.
Benefits and Limitations of Free PSA Testing
The main benefit of the FPSA test is that it may provide additional risk information when total PSA alone is inconclusive. In selected patients, it may help doctors and patients make more informed decisions about repeat testing, imaging, specialist referral or biopsy.
The principal limitation is that free PSA remains a risk-assessment marker rather than a definitive diagnostic test. It cannot identify the location or size of a prostate lesion, determine tumour aggressiveness or establish whether cancer has spread. It also cannot replace imaging, clinical examination or biopsy when those investigations are medically indicated.
False-positive and false-negative interpretations are possible. Some benign conditions may create a concerning pattern, while some cancers may not produce an obviously abnormal result. The test should always be interpreted by a qualified healthcare professional.
When Should a Patient Seek Medical Advice?
Consult a healthcare professional for persistent difficulty urinating, a weak urine stream, blood in urine or semen, unexplained pelvic discomfort, recurrent urinary infection, unexplained weight loss or persistent back or bone pain. These symptoms can have many causes and do not necessarily mean cancer, but they require proper assessment.
Urgent medical attention may be necessary if a person cannot pass urine, develops severe pain, has fever with urinary symptoms or becomes acutely unwell. A Free PSA blood test is not an emergency investigation and should not delay urgent treatment.
