PSA Total (Prostatic Specific Antigen, Total) Test
The PSA Total test is a blood test that measures the total concentration of prostate-specific antigen in the bloodstream. PSA is a protein produced mainly by cells of the prostate gland. Small quantities normally enter the blood, but the level can increase when prostate tissue grows, becomes inflamed, is infected or develops cancerous changes.
Total PSA includes PSA circulating freely and PSA attached to certain blood proteins. The test is commonly used to support prostate cancer screening decisions, investigate symptoms that may be related to the prostate and monitor selected patients with known prostate disease. It may also be used after prostate cancer treatment to help assess treatment response or detect possible recurrence.
An increased PSA result does not automatically mean that prostate cancer is present. Benign prostatic hyperplasia, prostatitis, urinary infection, urinary retention and recent prostate procedures can also increase PSA. Similarly, a PSA result within the laboratory reference interval cannot completely exclude prostate cancer. The result must be interpreted alongside age, symptoms, medical history, examination findings and previous PSA measurements.
What Is Prostate-Specific Antigen?
Prostate-specific antigen is an enzyme produced primarily by prostate gland cells. Its main biological function is associated with semen, but a small amount passes into the bloodstream. The laboratory measures this circulating PSA using a sensitive immunoassay and usually reports the result in nanograms per millilitre.
The term prostate-specific means that PSA is produced predominantly by prostate tissue. It does not mean that PSA is specific to prostate cancer. Both normal and abnormal prostate cells can produce PSA, and multiple benign conditions can raise its concentration.
In blood, PSA exists in two principal forms. One portion is attached to proteins, while another portion circulates freely. A total PSA test measures both forms together. A separate free PSA test may be requested when additional risk information is needed, especially when total PSA is borderline or moderately increased.
Why Is the PSA Total Test Performed?
A healthcare professional may request total PSA as part of an informed prostate cancer screening assessment. Screening means looking for possible signs of disease before symptoms develop. PSA screening may help identify some prostate cancers at an earlier stage, but it also has limitations and possible harms that should be discussed before testing.
The test may also be requested when a patient has urinary symptoms, an enlarged prostate, an abnormal digital rectal examination, suspected prostatitis or a significant family history of prostate cancer. PSA is only one part of the assessment and does not replace a medical consultation or physical examination.
For patients already diagnosed with prostate cancer, serial total PSA measurements may be used to monitor disease activity, evaluate response to treatment or look for biochemical evidence of recurrence. The expected PSA level and significance of a change depend on the treatment received and the individual clinical situation.
Who May Need a PSA Total Test?
PSA testing may be discussed with adults who are considering prostate cancer screening. Whether screening is appropriate depends on age, overall health, family history, individual risk factors, previous results and personal preferences. There is no single screening plan suitable for everyone.
A doctor may also recommend testing for symptoms such as difficulty starting urination, a weak or interrupted urinary stream, frequent urination, urgency, waking repeatedly at night to urinate or a feeling that the bladder has not emptied completely. These symptoms are commonly caused by benign prostate enlargement, but appropriate evaluation is still important.
Other reasons for testing may include blood in urine or semen, persistent pelvic discomfort, suspected prostate inflammation, follow-up of a previous abnormal PSA result or monitoring after prostate cancer diagnosis and treatment. Early prostate cancer may cause no symptoms, while advanced symptoms can also occur with noncancerous conditions.
How Is the PSA Test Performed?
The PSA Total test requires a venous blood sample. A trained phlebotomist cleans the collection area, usually inside the elbow, inserts a sterile needle into a vein and collects blood into a suitable tube. The procedure generally takes only a few minutes.
Some patients may experience brief discomfort, minor bruising, slight bleeding or temporary light-headedness. Serious complications from routine blood collection are uncommon. Patients who have a bleeding disorder, take blood-thinning medicine or have previously fainted during blood collection should inform the phlebotomist.
The blood is processed to obtain serum, and the PSA concentration is measured using a laboratory immunoassay. Different laboratories may use different analysers, antibodies and calibration systems. When PSA is monitored over time, using the same laboratory and analytical method may improve comparability.
Is Fasting Required for the PSA Total Test?
Fasting is generally not required specifically for a PSA Total test. Patients may normally eat and drink unless another investigation ordered at the same time requires fasting. Follow the instructions provided by the doctor or diagnostic centre.
Patients should provide an accurate list of prescription medicines, over-the-counter products and supplements. Medicines should not be stopped or changed without medical advice. Certain medicines can significantly affect PSA interpretation.
How Should a Patient Prepare?
Several temporary factors can influence PSA results. Patients should inform their healthcare professional about prostatitis, urinary infection, fever, recent urinary retention, catheterisation, cystoscopy, prostate biopsy, prostate surgery or another procedure involving the prostate or urinary tract. The doctor may recommend waiting for an appropriate period before testing.
Ejaculation may temporarily increase PSA in some individuals. Vigorous cycling or another activity that places prolonged pressure on the prostate may also influence the result. The doctor or laboratory may advise avoiding these activities for a specified period before sample collection.
Blood collection may be performed before significant prostate manipulation when both are scheduled. A routine digital rectal examination generally has a limited effect on PSA, but collecting the blood first can help maintain consistent testing conditions.
Finasteride and dutasteride, which may be prescribed for benign prostate enlargement or hair loss, can lower measured PSA. Testosterone therapy and other medicines may influence prostate health or PSA interpretation. Patients must report these treatments but should not discontinue them unless instructed by the treating doctor.
Understanding the PSA Total Result
PSA is usually reported in nanograms per millilitre. There is no single PSA value that can definitively distinguish prostate cancer from benign prostate conditions. A result above the laboratory reference interval indicates that further clinical assessment may be appropriate, but it does not confirm cancer.
PSA levels tend to increase with age and prostate size. Some laboratories or clinicians use age-related reference guidance, while others consider a fixed threshold together with individual risk factors. The reference interval shown on the laboratory report should be used for initial assessment.
A clinician may consider the absolute PSA value, age, symptoms, family history, medicines, prostate examination, previous results and the speed of change over time. PSA velocity should not be used alone to diagnose cancer or decide whether a biopsy is required.
One mildly increased result may be repeated after a suitable interval, particularly when a temporary cause is possible. Repeat testing should ideally occur under similar preparation conditions and, where practical, using the same laboratory method.
What Can Cause an Increased PSA Result?
Prostate cancer is one possible cause of an increased PSA level, but benign prostatic hyperplasia is much more common and can also raise PSA. Benign prostatic hyperplasia is a noncancerous enlargement of the prostate that frequently develops with age.
Prostatitis, urinary tract infection and inflammation can cause temporary or sometimes marked PSA elevations. Urinary retention, catheterisation, cystoscopy, prostate biopsy, surgery and other procedures involving the prostate may also affect the measurement.
Ejaculation and vigorous cycling before sample collection may produce smaller temporary changes. Natural biological variation means that PSA can differ between samples even without an important change in health. Laboratory methods and sample handling can also contribute to variation.
Factors That Can Affect Test Accuracy or Interpretation
- Benign prostatic hyperplasia or age-related prostate enlargement.

Does a High PSA Mean Prostate Cancer?
No. A high PSA result cannot independently diagnose prostate cancer. PSA is produced by both benign and malignant prostate tissue. Many patients with increased PSA do not have cancer, and some patients with prostate cancer may have PSA levels that are not substantially increased.
If the result is abnormal, the doctor may recommend repeating PSA, treating an infection when present or performing a digital rectal examination. Additional assessment may include free PSA, percent free PSA, another validated biomarker, prostate volume measurement, a risk calculator or multiparametric MRI.
A prostate biopsy may be recommended when the overall assessment indicates a clinically important risk. Biopsy involves collecting small tissue samples for microscopic examination and is used to confirm or exclude cancer. The decision should consider potential benefits and risks, including bleeding, infection and temporary urinary difficulty.
Can a Normal PSA Exclude Prostate Cancer?
A PSA result within the stated reference interval cannot completely exclude prostate cancer. No PSA threshold provides perfect separation between cancer and noncancerous disease. A doctor may recommend further assessment despite a normal-looking result if symptoms, examination findings, family history or other risk factors are concerning.
Patients should not rely on a single PSA value as proof that the prostate is healthy. Results should be interpreted within the complete clinical context, and follow-up should be based on professional advice.
PSA Testing After Prostate Cancer Treatment
Serial PSA measurements are frequently used after prostate cancer treatment. Following complete surgical removal of the prostate, PSA is expected to fall to a very low or undetectable level. A persistent or rising result may require further evaluation, although interpretation depends on the assay and clinical circumstances.
After radiation therapy, hormone therapy or other treatment, PSA may decline differently and can fluctuate. The treating specialist considers the pattern over time rather than one isolated value. Patients should follow the monitoring schedule recommended by their oncology or urology team.
Benefits and Limitations of PSA Testing
The PSA Total test is a simple blood test that may help detect prostate abnormalities and support earlier identification of some cancers. It is also valuable for monitoring many patients after prostate cancer diagnosis or treatment.
However, PSA screening can produce false-positive results that lead to anxiety, repeat tests, imaging or biopsy. It can also identify slow-growing cancers that may never cause symptoms or shorten life, potentially leading to unnecessary treatment. False-negative results are also possible.
PSA cannot determine the precise cause of an abnormal value, locate a prostate lesion, establish tumour aggressiveness or diagnose cancer without additional evidence. An informed discussion with a healthcare professional is important before screening and after receiving the result.
When Should Medical Advice Be Sought?
Consult a healthcare professional for persistent urinary difficulty, a weak urine stream, frequent urination, blood in urine or semen, pelvic discomfort, unexplained weight loss or persistent back or bone pain. These symptoms can result from many conditions and do not necessarily indicate cancer, but they require proper evaluation.
Urgent medical assessment may be necessary if a person cannot pass urine, develops severe pain, experiences fever with urinary symptoms or becomes acutely unwell. PSA testing should not delay emergency care.
