Small Dense LDL (sdLDL) Test
The Small Dense LDL (sdLDL) test measures the amount or proportion of small, dense low-density lipoprotein particles in the blood. LDL carries cholesterol from the liver to tissues throughout the body. Although LDL cholesterol is commonly described as bad cholesterol, LDL particles vary in their size, density and potential effect on cardiovascular health. Small dense LDL particles are smaller and denser than larger, more buoyant LDL particles and may be associated with an increased risk of atherosclerosis and cardiovascular disease.
A standard lipid profile reports total cholesterol, LDL cholesterol, HDL cholesterol and triglycerides. It estimates the amount of cholesterol transported within lipoproteins but does not directly describe LDL particle characteristics. A person may have an LDL cholesterol value within the laboratory reference range while still having a relatively high concentration of small dense LDL particles. The sdLDL test may therefore provide additional information in selected individuals whose cardiovascular risk is not fully explained by a conventional lipid profile.
Small dense LDL measurement is not a replacement for routine cholesterol testing or a complete cardiovascular assessment. The result must be interpreted alongside age, blood pressure, diabetes status, smoking, family history, body weight, kidney function, existing cardiovascular disease and other laboratory findings.
What Is Small Dense LDL?
LDL is not a single uniform particle. It consists of several subclasses that differ in size, density, composition and metabolic behaviour. Larger, more buoyant particles are generally described as LDL pattern A, whereas a predominance of smaller and denser particles is often called LDL pattern B. Small dense LDL particles contain less cholesterol per particle and may remain in circulation differently from larger LDL particles.
Small dense LDL particles may enter the arterial wall more readily, remain in the bloodstream longer and be more susceptible to oxidative modification. Oxidised or otherwise modified LDL can contribute to inflammation and plaque formation within arteries. These biological properties help explain why increased sdLDL has been investigated as a marker of atherosclerotic cardiovascular risk.
However, cardiovascular disease is complex and cannot be predicted using a single biomarker. An elevated sdLDL value does not mean that a heart attack or stroke will definitely occur, and a lower value does not guarantee protection.
Why Is the Small Dense LDL Test Performed?
A physician, cardiologist, endocrinologist or lipid specialist may request the sdLDL test when a more detailed assessment of lipoprotein-related risk is required. It may be considered when routine lipid results appear acceptable but the patient has other important cardiovascular risk factors or a strong family history of premature heart disease.
The test may also be used in people with high triglycerides, low HDL cholesterol, insulin resistance, metabolic syndrome, obesity, type 2 diabetes or fatty liver disease. These conditions are frequently associated with an atherogenic lipid pattern characterised by increased triglyceride-rich lipoproteins, reduced HDL cholesterol and a greater proportion of small dense LDL particles.
Assessment of Residual Cardiovascular Risk
Residual risk refers to the cardiovascular risk that remains despite apparently controlled LDL cholesterol or treatment with cholesterol-lowering medicine. In carefully selected patients, sdLDL measurement may help the treating doctor understand whether an atherogenic lipoprotein pattern is still present. The result may be considered with non-HDL cholesterol, apolipoprotein B, lipoprotein(a), triglycerides and other risk indicators.
Metabolic Syndrome and Insulin Resistance
Metabolic syndrome is a group of abnormalities that may include abdominal obesity, elevated blood pressure, high blood glucose, increased triglycerides and reduced HDL cholesterol. Insulin resistance can alter the production and processing of lipoproteins, encouraging the formation of smaller and denser LDL particles. Measuring sdLDL may provide supplementary information about this metabolic pattern.
Diabetes and Prediabetes
People with diabetes may have an increased cardiovascular risk even when their calculated LDL cholesterol is not markedly elevated. Diabetes is often accompanied by increased triglycerides, low HDL cholesterol and smaller LDL particles. The sdLDL test may help characterise this pattern, but it does not diagnose diabetes. Fasting plasma glucose, HbA1c and other appropriate investigations are used for diagnosing and monitoring diabetes.
Who May Need the sdLDL Test?
The test may be considered for people with premature coronary artery disease, a strong family history of early heart attack or stroke, metabolic syndrome, diabetes, obesity, high triglycerides, low HDL cholesterol or an unexplained cardiovascular event. It may also be considered when routine lipid measurements do not appear consistent with the patient's overall risk profile.
Small dense LDL testing is not routinely required for every healthy individual. Cardiovascular risk is usually assessed initially through medical history, physical examination, blood pressure measurement, diabetes screening and a standard lipid profile. The healthcare provider determines whether advanced lipid testing is likely to add useful information.
Sample Requirements and Test Preparation
The Small Dense LDL test requires a venous blood sample. Blood is collected from a vein, usually in the arm, using a sterile needle and an appropriate collection tube. The sample is then processed according to the laboratory's analytical requirements.
Is Fasting Required?
Fasting requirements may vary according to the laboratory method and the other tests ordered at the same time. Because triglyceride levels and recent food intake can influence the broader lipid assessment, fasting for approximately 9 to 12 hours may be advised in some circumstances. During fasting, plain water is generally permitted unless the healthcare provider gives different instructions.

Factors That May Affect the Result
Recent meals, alcohol consumption, acute illness, significant changes in body weight, poorly controlled diabetes, thyroid disorders, kidney disease, liver disease and certain medicines may influence the lipid pattern. The result should ideally be interpreted when the patient is medically stable. If a result does not correspond with the clinical picture, the doctor may recommend repeat testing.
How Is Small Dense LDL Measured?
Laboratories may assess small dense LDL using direct homogeneous assays, precipitation methods, electrophoresis, ultracentrifugation, nuclear magnetic resonance-based lipoprotein analysis or other validated techniques. Some methods report the concentration of cholesterol carried by small dense LDL particles, while others describe LDL particle size, particle number or subclass distribution.
Results obtained using different analytical methods may not be directly interchangeable. The patient should use the reference interval and units printed on the laboratory report. When serial testing is clinically required, using the same laboratory and method may make comparison more meaningful.
Understanding the sdLDL Test Result
The report may provide a numerical sdLDL concentration, a percentage, a particle-size classification or an interpretation based on the method used. Reference values differ between laboratories because methods, calibration systems and reporting units are not identical. The result should therefore be reviewed using the laboratory-specific reference interval.
Elevated Small Dense LDL
An elevated sdLDL result indicates an increased amount or proportion of smaller, denser LDL particles. This pattern may be associated with high triglycerides, low HDL cholesterol, insulin resistance, metabolic syndrome, type 2 diabetes, obesity and an increased risk of atherosclerotic cardiovascular disease.
Elevated sdLDL does not independently diagnose a blocked artery, coronary artery disease or any specific cardiovascular condition. It also does not determine the severity or location of arterial plaque. The treating doctor assesses the result alongside symptoms, blood pressure, family history, conventional lipid results, apolipoprotein B, glucose measurements and appropriate cardiovascular investigations.
Normal or Lower Small Dense LDL
A result within the laboratory reference interval suggests that small dense LDL is not increased according to the method used. However, it does not eliminate cardiovascular risk. A person may still have high LDL cholesterol, increased apolipoprotein B, elevated lipoprotein(a), hypertension, diabetes, smoking-related risk or established cardiovascular disease.
Preventive care and prescribed treatment should not be stopped solely because the sdLDL result is normal. All treatment decisions must be made by the healthcare provider after evaluating the complete risk profile.
Small Dense LDL and the Standard Lipid Profile
A standard lipid profile remains the foundation of lipid assessment. LDL cholesterol estimates or measures the cholesterol contained in LDL particles, while non-HDL cholesterol represents cholesterol carried by all potentially atherogenic lipoproteins. Triglycerides provide important information about triglyceride-rich particles, and HDL cholesterol is considered as part of the overall risk assessment.
The sdLDL test provides different but complementary information about LDL characteristics. It should not be interpreted as a substitute for LDL cholesterol, non-HDL cholesterol or other established cardiovascular risk markers. In many patients, routine measurements provide sufficient information to guide prevention and treatment.

A heart-healthy eating pattern generally emphasises vegetables, fruits, whole grains, pulses, nuts, seeds and appropriate sources of unsaturated fat while limiting trans fats, excessive saturated fat, refined carbohydrates and added sugars. Dietary recommendations should be individualised for medical conditions, nutritional needs and prescribed treatment.
Medicines such as statins or other lipid-lowering therapies may be prescribed according to established cardiovascular risk, LDL cholesterol, non-HDL cholesterol, apolipoprotein B and existing disease. An sdLDL result should not be used by patients to start, stop or alter medication without medical supervision.
Benefits and Limitations of the sdLDL Test
The principal benefit of the test is its ability to provide additional information about LDL particle characteristics that may not be visible in a standard lipid profile. It may be useful in selected patients with metabolic abnormalities, unexplained cardiovascular risk or discordance between conventional lipid results and the clinical picture.
Its limitations include variation between laboratory methods, lack of universally interchangeable reference limits and uncertainty about how much it improves risk prediction beyond established markers in every patient. Testing is therefore most useful when requested for a clear clinical reason and interpreted by a qualified healthcare professional.
