About this test
The Hepatitis B Envelope Antigen (HBeAg) Test is a laboratory blood test that detects the Hepatitis B e Antigen (HBeAg), a protein produced by the Hepatitis B Virus (HBV) during active viral replication. It is an important marker used to assess viral activity, infectivity, and the stage of hepatitis B infection.
Hepatitis B is a viral infection that affects the liver and spreads through contact with infected blood, body fluids, contaminated needles, sexual contact, and from an infected mother to her baby during childbirth. The infection may be acute or chronic. Chronic hepatitis B can lead to serious complications such as cirrhosis, liver failure, and hepatocellular carcinoma (liver cancer) if not appropriately monitored and treated.
A positive HBeAg result generally indicates that the virus is actively replicating and that the infected individual is more likely to transmit HBV to others. In patients with chronic hepatitis B, HBeAg is commonly used to assess disease activity and monitor the response to antiviral treatment.
However, it is important to understand that some patients with chronic hepatitis B may be HBeAg-negative but still have active viral replication due to pre-core or basal core promoter mutations. Therefore, HBeAg results must always be interpreted together with HBV DNA (PCR) testing and other hepatitis B markers.
The HBeAg Test is usually ordered alongside:
- Hepatitis B Surface Antigen (HBsAg)
- Hepatitis B Surface Antibody (Anti-HBs)
- Total Anti-HBc
- Anti-HBc IgM
- Anti-HBe
- HBV DNA (PCR)
- Liver Function Tests (LFTs)
Together, these investigations provide a complete assessment of hepatitis B infection, infectivity, and treatment response.
Benefits of the Test
- Detects active hepatitis B viral replication.
- Helps assess HBV infectivity.
- Supports monitoring of chronic hepatitis B.
- Assists in evaluating antiviral treatment response.
- Useful for determining disease activity.
- Part of a comprehensive hepatitis B profile.
- Requires only a simple blood sample.
- Provides reliable laboratory results using validated immunoassay methods.
Why Doctors Recommend This Test
Doctors recommend the HBeAg Test to determine whether hepatitis B virus is actively replicating, evaluate disease progression, assess infectivity, monitor treatment response, and help guide management decisions in patients with acute or chronic hepatitis B infection.
Preparation Before the Test
No special preparation is required.
You may eat and drink normally unless instructed otherwise by your healthcare provider. Inform your doctor about previous hepatitis B infection, antiviral treatment, liver disease, vaccination history, or current medications. A blood sample is collected from a vein for laboratory analysis.
Normal Reporting Time
Typically: 1–3 Working Days
Reporting time may vary depending on laboratory methodology.

- Individuals with chronic hepatitis B.
- Patients with abnormal liver function tests.
- Pregnant women with hepatitis B.
- Healthcare workers exposed to HBV.
- Patients referred by hepatologists or infectious disease specialists.
