The Herpes Simplex Virus Type 2 (HSV-2) IgM Antibody Test is a laboratory blood test that detects Immunoglobulin M (IgM) antibodies against Herpes Simplex Virus Type 2 (HSV-2). IgM antibodies are produced by the immune system during an early immune response and may be detected following a primary herpes infection or, in some cases, during viral reactivation.

HSV-2 is the virus most commonly associated with genital herpes, a sexually transmitted infection spread through direct skin-to-skin sexual contact. After the initial infection, HSV-2 remains dormant in nerve cells and may reactivate periodically, causing recurrent outbreaks.

Many individuals infected with HSV-2 have few or no symptoms and may unknowingly transmit the virus. When symptoms occur, they may include painful genital blisters or ulcers, itching, burning, pain during urination, swollen lymph nodes, fever, and flu-like symptoms, especially during the first episode.

Although the HSV-2 IgM Antibody Test detects an early immune response, current international clinical guidelines do not recommend HSV IgM testing for routine diagnosis of herpes simplex infection. HSV IgM antibodies may appear during both primary infection and recurrent episodes, may cross-react with other herpes viruses, and cannot reliably distinguish between HSV-1 and HSV-2 infections.

For patients with active genital lesions, HSV PCR testing from a lesion swab is the preferred diagnostic method because it directly detects HSV DNA with excellent sensitivity and specificity. When determining previous exposure, type-specific HSV-2 IgG antibody testing is the recommended serological investigation.

Doctors may occasionally request the HSV-2 IgM Antibody Test in selected clinical situations as part of a broader evaluation. However, results should always be interpreted together with clinical symptoms, patient history, physical examination, and other laboratory findings.
