HIV P24 ANTIGEN

The HIV-1 p24 Antigen Test is a laboratory blood test used to detect the p24 protein (antigen) of the Human Immunodeficiency Virus Type 1 (HIV-1). The p24 antigen is one of the earliest viral proteins to appear in the bloodstream after HIV infection, making this test valuable for early detection of HIV, often before HIV antibodies become detectable.

Human Immunodeficiency Virus (HIV) attacks the body's immune system, particularly CD4 (T-helper) lymphocytes, weakening the body's ability to fight infections. Without treatment, HIV infection can progress to Acquired Immunodeficiency Syndrome (AIDS). Early diagnosis allows prompt initiation of antiretroviral therapy (ART), improving long-term health outcomes and reducing the risk of HIV transmission.

After HIV infection, the p24 antigen usually becomes detectable approximately 2–4 weeks after exposure, before the body has produced measurable HIV antibodies. As antibodies develop, p24 antigen levels often decrease and may eventually become undetectable. For this reason, p24 antigen testing is especially useful during the early stage of HIV infection.

Today, stand-alone HIV p24 antigen testing is less commonly performed because most laboratories use 4th Generation HIV Antigen/Antibody Tests, which simultaneously detect HIV-1 p24 antigen and antibodies to HIV-1 and HIV-2. These combination tests provide higher sensitivity and improve early HIV diagnosis.

Doctors may recommend the HIV p24 Antigen Test in selected clinical situations, such as evaluation of recent HIV exposure, early HIV infection, or when additional laboratory assessment is required. If very recent infection is strongly suspected, HIV Nucleic Acid Testing (NAT/PCR) may also be recommended because it can detect the virus even earlier.

A reactive p24 antigen result requires confirmation using the recommended HIV diagnostic testing algorithm before a diagnosis of HIV infection is established.
