Jo-1 Antibody Test (Serum)
The Jo-1 Antibody Test (Serum) is a laboratory blood test used to detect Anti-Jo-1 antibodies, a type of autoantibody directed against histidyl-transfer RNA (tRNA) synthetase, an enzyme involved in protein synthesis. These antibodies are strongly associated with idiopathic inflammatory myopathies, particularly polymyositis, dermatomyositis, and antisynthetase syndrome.
Idiopathic inflammatory myopathies are autoimmune disorders in which the immune system mistakenly attacks muscles and, in some cases, the lungs, joints, and skin. Patients commonly experience progressive muscle weakness, especially affecting the shoulders, hips, neck, and upper arms, making activities such as climbing stairs, lifting objects, or standing from a seated position difficult.
Anti-Jo-1 antibodies are the most common myositis-specific autoantibodies and are considered an important marker of antisynthetase syndrome. This syndrome is characterized by a combination of:
- Muscle inflammation (myositis)
- Interstitial lung disease (ILD)
- Arthritis
- Raynaud's phenomenon
- Fever
- "Mechanic's hands" (rough, cracked skin on the fingers)
Because lung involvement may become severe, early identification of Anti-Jo-1 antibodies helps physicians diagnose the disease promptly and begin appropriate treatment before significant complications develop.
The Jo-1 Antibody Test is often ordered together with other investigations such as Creatine Kinase (CK), Aldolase, Antinuclear Antibody (ANA), Myositis-specific antibody panels, Electromyography (EMG), Muscle MRI, Pulmonary Function Tests (PFTs), and muscle biopsy when clinically indicated.
A positive Anti-Jo-1 antibody result supports—but does not by itself confirm—the diagnosis of antisynthetase syndrome or inflammatory myopathy. Results should always be interpreted together with the patient's symptoms, physical examination, imaging studies, and other laboratory findings.
