Legionella Antigen Detection (Urine)

The Legionella Antigen Detection (Urine) Test is a rapid laboratory test used to detect Legionella pneumophila antigen in a urine sample. It is primarily used to help diagnose Legionnaires' disease, a serious form of bacterial pneumonia caused by Legionella pneumophila, most commonly serogroup 1.

Legionella bacteria naturally live in freshwater environments but can multiply in cooling towers, air-conditioning systems, hot tubs, water tanks, fountains, plumbing systems, and hospital water supplies when water management is inadequate. Infection occurs when people inhale contaminated water droplets (aerosols). Legionnaires' disease is not spread from person to person in almost all cases.

Symptoms usually begin 2–10 days after exposure and may include:

The Legionella Urinary Antigen Test is widely used because it provides rapid results and can remain positive even after antibiotic treatment has started. A positive urine antigen result allows clinicians to begin appropriate antibiotic therapy without waiting for culture results.

However, it is important to note that most commercially available urinary antigen tests primarily detect Legionella pneumophila serogroup 1, which accounts for the majority—but not all—cases of Legionnaires' disease. Therefore, a negative urine antigen test does not completely rule out Legionella infection, especially infections caused by other Legionella species or serogroups. In patients with strong clinical suspicion, additional tests such as respiratory culture, PCR, or direct fluorescent antibody testing may be recommended.
