PCNA (Proliferating Cell Nuclear Antigen) Antibody Test

The PCNA (Proliferating Cell Nuclear Antigen) Antibody Test is a laboratory blood test that detects autoantibodies directed against Proliferating Cell Nuclear Antigen (PCNA), a protein involved in DNA replication and DNA repair. These antibodies are considered rare antinuclear antibodies (ANA) and are most commonly associated with systemic lupus erythematosus (SLE).

PCNA is a nuclear protein that functions as a "sliding clamp" for DNA polymerase during DNA synthesis and plays an essential role in cell division, DNA replication, and repair of damaged DNA. In certain autoimmune disorders, the immune system mistakenly recognizes this normal cellular protein as foreign and produces antibodies against it.

The Anti-PCNA Antibody Test is usually performed as part of an extended autoimmune antibody panel in patients with suspected connective tissue diseases, particularly when systemic lupus erythematosus is being evaluated.

Symptoms of SLE may include:

However, they are detected in only a small percentage (approximately 2–10%) of patients with SLE, making them a low-sensitivity marker. Consequently, a negative Anti-PCNA result does not exclude lupus.

The PCNA antibody test is not used as a screening test for autoimmune diseases. Instead, it serves as an adjunctive investigation together with antinuclear antibody (ANA), anti-double stranded DNA (anti-dsDNA), anti-Sm, anti-Ro/SSA, anti-La/SSB, complement levels (C3 and C4), and clinical findings.
