Sphingomyelin-IgG (Phospholipid)
Sphingomyelin-IgG (Phospholipid) is a specialised serological test that detects or measures immunoglobulin G antibodies directed against sphingomyelin, a phospholipid found in cell membranes and in the myelin sheath surrounding nerve fibres. The test may be requested as part of an extended phospholipid antibody evaluation when a clinician is investigating a possible autoimmune or thrombotic condition.
Phospholipids are essential structural components of cell membranes. Sphingomyelin is particularly abundant in nervous tissue, red blood cells and lipoproteins. Under certain circumstances, the immune system may produce antibodies that react with phospholipids or with proteins bound to phospholipid surfaces. These antibodies are collectively discussed within the broad group of antiphospholipid antibodies.
Sphingomyelin-IgG is considered a specialised or non-criteria phospholipid antibody. It is not one of the standard laboratory markers used in established antiphospholipid syndrome classification criteria. The standard or criteria-associated investigations are lupus anticoagulant testing, anticardiolipin IgG and IgM, and anti-beta-2 glycoprotein I IgG and IgM.
Consequently, a positive sphingomyelin-IgG result cannot independently diagnose antiphospholipid syndrome, explain a blood clot or predict a pregnancy outcome. Its clinical significance is less clearly established than that of the standard antiphospholipid markers. Doctors interpret the result cautiously and only in combination with the patient's history, examination and complete laboratory profile.
The test is generally performed on serum obtained from a venous blood sample. Depending on the laboratory method, the result may be reported numerically or as negative, borderline or positive. Reference intervals and analytical units vary, so the result must be interpreted using the information printed on the individual laboratory report.
Benefits of Sphingomyelin-IgG Testing
The potential value of sphingomyelin-IgG testing is greatest in selected patients whose clinical history suggests an immune-mediated thrombotic or pregnancy-related disorder but whose standard antiphospholipid antibody investigations are negative, incomplete or inconclusive. It may provide supplementary information, but it should not replace validated first-line testing.

Understanding Antiphospholipid Antibodies
Antiphospholipid antibodies are a heterogeneous group of autoantibodies that react with phospholipids, phospholipid-protein complexes or associated plasma proteins. Some clinically important antibodies can interfere with coagulation assays or contribute to cellular changes associated with thrombosis.
Antiphospholipid syndrome is a clinical autoimmune disorder characterised by specified thrombotic or pregnancy-related events together with persistent positivity for recognised antiphospholipid antibodies. Laboratory results alone do not establish the syndrome, and a history of thrombosis alone does not demonstrate that antiphospholipid antibodies caused the event.
The most widely used laboratory investigations include lupus anticoagulant testing, anticardiolipin IgG and IgM and anti-beta-2 glycoprotein I IgG and IgM. These tests have better-established clinical roles than antibodies against sphingomyelin and other non-criteria phospholipids.
Sphingomyelin-IgG may occasionally be measured as part of an extended phospholipid antibody panel. Research has explored various non-criteria antibodies, but their standardisation, clinical associations and optimal thresholds remain less certain. A specialist should determine whether such testing is appropriate.
Clinical Indications and Applications
Unexplained Venous Thrombosis: A clinician may consider an antiphospholipid antibody evaluation after a deep-vein thrombosis, pulmonary embolism or thrombosis at an unusual site, particularly when the patient is young or lacks a clear temporary risk factor. Sphingomyelin-IgG is supplementary rather than a first-line test.
Arterial Thrombosis: Selected patients with an unexplained stroke, transient ischaemic attack or another arterial event may undergo testing for acquired thrombophilia. Interpretation requires consideration of age, vascular risk factors and the timing of testing.
Pregnancy-Related Complications: Standard antiphospholipid testing may be considered in patients with a clinically relevant history of recurrent pregnancy loss, fetal loss, early severe pre-eclampsia or placental insufficiency. The significance of an isolated sphingomyelin-IgG result in this setting is uncertain and must not be overstated.
Systemic Autoimmune Disease: Antiphospholipid antibodies can occur in patients with systemic lupus erythematosus and other autoimmune diseases. Testing may form part of a broader evaluation when the patient has compatible clinical events. A positive sphingomyelin antibody does not diagnose lupus or another connective-tissue disease.
Negative Standard Antibody Tests with Continuing Suspicion: In selected cases, specialists may request non-criteria antibodies after standard testing is negative despite a strongly suggestive clinical history. Evidence for diagnosis and management based on these extended markers is still developing.
Why Doctors Recommend This Test
Doctors may recommend sphingomyelin-IgG when they need supplementary serological information beyond the standard phospholipid antibody panel. The decision usually follows review of the patient's clotting history, pregnancy history, autoimmune symptoms and previous laboratory results.
The test is not appropriate for routine screening of healthy individuals. Antibody positivity can occasionally occur without clinical disease, and testing people with a low probability of an autoimmune thrombotic disorder increases the likelihood that an isolated result will be misleading.
When antiphospholipid syndrome is suspected, the standard panel should not be omitted in favour of sphingomyelin-IgG. Lupus anticoagulant, anticardiolipin and anti-beta-2 glycoprotein I antibodies remain the principal laboratory investigations. Extended testing may be added only when clinically justified.
The doctor must also consider inherited thrombophilia, malignancy, immobilisation, surgery, hormonal treatment, pregnancy, infection, cardiovascular risk factors and other acquired causes of thrombosis. A sphingomyelin antibody result cannot distinguish among these possibilities.
Preparation Before the Test
Fasting is generally not required. Patients can usually eat, drink water and take routine medicines before sample collection unless other tests ordered at the same visit require fasting. Follow any instructions provided by the referring clinician or laboratory.
Inform the doctor about anticoagulants such as warfarin, heparin, apixaban, rivaroxaban or dabigatran. These medicines are particularly relevant to functional lupus anticoagulant assays, although they may not directly interfere with every solid-phase IgG antibody assay. Do not stop anticoagulant medication without explicit medical instructions.
Tell the healthcare team about recent infection, vaccination, pregnancy, autoimmune disease, thrombosis, surgery, hospitalisation and immunosuppressive treatment. Some antibody findings can be temporary during an infection or inflammatory episode.
If the test is being repeated to assess persistence, use the timing recommended by the specialist. For established criteria antibodies, persistence is generally assessed using specimens collected at least 12 weeks apart. The clinical meaning and repeat-testing approach for sphingomyelin-IgG are less standardised.
Blood Sample Collection Procedure
A trained phlebotomist collects blood from a vein, usually in the arm, using a sterile needle. The specimen is commonly placed in a serum separator or plain tube. After the blood clots, the laboratory separates the serum from the cells.
Collection normally takes only a few minutes. Mild discomfort, temporary bruising or light-headedness may occur. Applying firm pressure after the needle is removed can help reduce bleeding and bruising.
The serum must be transported and stored according to the assay manufacturer's and laboratory's validated requirements. A severely haemolysed, lipaemic, contaminated, incorrectly labelled or insufficient sample may need to be recollected.
Laboratory Analysis and Reporting
Sphingomyelin-IgG is generally measured using an immunoassay in which sphingomyelin or an associated antigen preparation is attached to a testing surface. If the patient's serum contains reactive IgG antibodies, they bind to the target and are detected through an enzyme or signal-based system.
The result may be reported in units defined by the assay and classified as negative, borderline, equivocal or positive. There is no universal reference interval applicable to every sphingomyelin antibody method. Interpretation must use the reference limits and comments supplied by the performing laboratory.
Results from different manufacturers or laboratories may not be directly comparable because antigen sources, calibration, units and cut-offs vary. If repeat testing is required, using the same method can improve comparability.

The report for the Sphingomyelin-IgG (Phospholipid) test is generally available within 3 to 5 working days. Turnaround time may vary according to sample transport, laboratory workload, batch testing, repeat analysis and referral-laboratory schedules.
Who Should Consider This Test?
- Patients undergoing a specialist-directed extended phospholipid antibody evaluation.
- Individuals with unexplained venous or arterial thrombosis and an appropriate clinical probability.
- Patients with selected recurrent pregnancy complications undergoing autoimmune assessment.
- People with systemic autoimmune disease and a clinically relevant thrombotic history.
- Patients with negative or inconclusive standard antiphospholipid tests but continuing specialist concern.
- Anyone specifically advised to undergo testing by a haematologist, rheumatologist, neurologist or obstetric specialist.
Understanding the Test Results
Negative Result: A negative result means that sphingomyelin-reactive IgG was not detected above the assay's cut-off. This does not exclude antiphospholipid syndrome or another cause of thrombosis. Standard lupus anticoagulant, anticardiolipin and anti-beta-2 glycoprotein I testing may still be required.
Borderline or Equivocal Result: A value near the laboratory threshold has uncertain significance. It may reflect low-level reactivity, analytical variation or a temporary antibody response. The doctor may recommend repeat testing or place greater emphasis on standard antibodies and clinical findings.
Positive Result: A positive result indicates that sphingomyelin-reactive IgG was detected. Because this is a non-criteria antibody, positivity does not independently establish antiphospholipid syndrome or prove that an antibody caused a previous clot or pregnancy complication.
Persistent Positive Result: Repeated positivity may provide more meaningful evidence of a stable autoimmune response than a single transient result. Nevertheless, persistence of sphingomyelin-IgG alone does not automatically satisfy accepted laboratory classification requirements for antiphospholipid syndrome.
The clinician may review lupus anticoagulant, anticardiolipin IgG and IgM, anti-beta-2 glycoprotein I IgG and IgM, complete blood count, coagulation studies and relevant imaging. Additional autoimmune or thrombophilia investigations depend on the patient's clinical circumstances.
Clinical Limitations
Sphingomyelin-IgG is a specialised non-criteria antibody with less established clinical validation and standardisation than conventional antiphospholipid markers. Its sensitivity, specificity and predictive value vary with the population and analytical method.
A positive result can occur without thrombosis or pregnancy complications. Temporary antibody production may accompany infections, inflammation or other immune stimulation. Conversely, a negative sphingomyelin result does not exclude antiphospholipid syndrome.
The test cannot identify the location of a clot, measure current clotting activity, predict exactly when a future event will occur or determine whether anticoagulant treatment is necessary. Management decisions must not be based on this result alone.
Testing during an acute event or while receiving treatment may complicate the interpretation of the overall antiphospholipid profile. Anticoagulants particularly affect clot-based lupus anticoagulant assays, although their influence differs from that on solid-phase antibody tests.
An isolated sphingomyelin-IgG result should be interpreted conservatively. Clinical history and validated standard antiphospholipid tests generally carry greater diagnostic importance.
Important Safety Information
Do not start aspirin, anticoagulants or other blood-thinning medicines solely because of a positive sphingomyelin-IgG result. These medicines can cause serious bleeding and should be used only under medical supervision. Likewise, do not stop prescribed anticoagulation because of a negative result.
Seek emergency medical attention for sudden chest pain, breathing difficulty, coughing blood, one-sided leg swelling, sudden weakness, facial drooping, speech difficulty, severe headache or sudden vision loss. Pregnant patients with heavy bleeding, severe abdominal pain, severe headache, visual disturbance or reduced fetal movement should obtain urgent obstetric care.
