About this test
3D CT FACE is an advanced computed tomography examination that creates detailed cross-sectional images and three-dimensional reconstructions of the facial bones. The scan may include the forehead, eye sockets, cheekbones, nasal bones, upper jaw, lower jaw, sinus walls and other craniofacial structures, depending on the clinical indication and the protocol selected by the radiology team.
The facial skeleton consists of several small and irregularly shaped bones positioned close to the eyes, nose, teeth, sinuses, nerves and airway. These structures can overlap on conventional X-rays, making certain fractures or abnormalities difficult to evaluate completely. CT overcomes much of this limitation by acquiring thin image slices that can be viewed in axial, coronal, sagittal and oblique planes.
Specialized computer software can process the image data into three-dimensional reconstructions. These views can be rotated to show facial anatomy from different angles, helping radiologists and surgeons understand the size, shape, alignment and spatial relationship of a fracture or structural abnormality. The 3D reconstruction complements the original CT images but does not replace them, as some subtle findings are better seen on the thin cross-sectional slices.

Doctors commonly recommend a 3D CT Face scan following facial trauma caused by a road traffic accident, fall, sports injury, assault or direct impact. It may help identify fractures involving the nasal bones, cheekbones, eye sockets, upper jaw, lower jaw or frontal region. The scan can demonstrate displacement, fragmentation and extension of a fracture into adjacent structures.
Injuries around the eye require careful evaluation because fractures may affect the orbital walls and nearby muscles or tissues. CT can show bony injury and displacement, but an ophthalmological examination is necessary to assess vision, eye movement and direct injury to the eyeball. Sudden vision changes or severe eye pain after trauma require urgent medical attention.
A 3D CT Face scan may also be recommended before reconstructive or corrective surgery. Three-dimensional views can help surgeons understand facial symmetry, bone contours and the position of an abnormality. This information may be useful when planning fracture fixation, facial reconstruction or selected jaw procedures. Treatment decisions are based on the complete CT dataset, physical examination, dental assessment and other relevant investigations.
The examination may be used to evaluate congenital or developmental craniofacial abnormalities, unexplained facial asymmetry, bony swelling or suspected bone lesions. After surgery, CT may help assess alignment, healing, fixation plates, screws or other implanted material. Metal hardware can produce artifacts, although appropriate scanning and reconstruction techniques may reduce some of these effects.
If the principal concern involves the brain, blood vessels, nerves or other soft tissues, a different CT protocol or MRI may be required. A routine 3D CT Face scan is also different from a dedicated CT PNS, CT Brain, CT Angiography or dental cone-beam CT. The referring doctor should select the examination that matches the clinical question.
Preparation Before Test
Most 3D CT Face examinations performed primarily for facial-bone assessment do not require intravenous contrast or extensive preparation. Patients can generally eat, drink and take regular medicines as usual unless they receive different instructions from the diagnostic centre or treating doctor.
Wear comfortable clothing and remove spectacles, earrings, necklaces, hairpins, removable dental appliances and other metallic objects around the head and face when instructed. Metal can create streaks or artifacts that obscure nearby structures and reduce the quality of the three-dimensional reconstruction.
Bring the doctor's prescription, clinical records and previous imaging reports or films. Earlier X-rays, CT scans, MRI scans, operative notes and dental records may help the radiologist evaluate changes, healing or implant position. Inform the imaging team about previous facial surgery, dental implants, fixation plates, screws or other implanted devices.
Intravenous contrast is not routinely required for every facial-bone CT. It may be prescribed in selected cases involving a suspected infection, tumour, inflammatory process, blood-vessel abnormality or soft-tissue complication. If contrast is required, fasting and a recent kidney-function test may be requested. Inform the centre about kidney disease, diabetes, asthma, thyroid conditions, significant allergies and any previous reaction to contrast material.
Pregnant patients or anyone who may be pregnant must notify the referring doctor and CT staff before the examination. CT uses ionizing radiation, so the healthcare team must consider the clinical urgency, expected benefit and possible alternative tests. Children also require age- and size-appropriate scanning protocols.
During the examination, the patient lies on a motorized CT table with the head positioned carefully. Supports may be used to keep the head still. Movement can blur small facial bones and reduce reconstruction quality, so the patient should remain still and follow the technologist's instructions. Sedation is not generally required, but separate arrangements may be necessary for very young children or patients who cannot remain still.
Normal Reporting Time
Image acquisition is generally completed within a few minutes. Registration, preparation and careful positioning increase the total appointment time. Additional processing is required to create thin multiplanar images and three-dimensional reconstructions.
A radiologist reviews the original CT slices, multiplanar views and 3D images before preparing the report. Many routine reports may be available within 24 to 48 hours. Emergency trauma cases may be prioritized according to clinical need. Complicated fractures, postoperative studies and examinations requiring comparison with previous imaging may take longer. Who Should Take This Test?
3D CT Face may be appropriate for patients with suspected facial fractures following trauma, particularly when there is swelling, tenderness, deformity, restricted jaw movement, altered facial sensation or an abnormal X-ray. It may also be recommended for patients with facial asymmetry, congenital craniofacial abnormalities, suspected bone lesions or a need for reconstructive surgical planning.
Patients who have undergone facial fracture repair, jaw surgery or another craniofacial procedure may require this examination for follow-up when advised by their surgeon. CT is not necessary for every minor facial injury or episode of facial pain. The doctor considers the mechanism of injury, symptoms, examination findings and previous tests before requesting the scan.
Children and patients who may need repeated examinations require special consideration because CT uses radiation. Pregnant patients also need individualized assessment. The scan should be performed only when the expected diagnostic benefit justifies the exposure.
Patients with loss of consciousness, repeated vomiting, severe or worsening headache, seizures, confusion, uncontrolled bleeding, breathing difficulty, clear fluid leaking from the nose or ears, sudden vision changes or major facial trauma should seek emergency medical care rather than waiting for a routine appointment.
Detailed Information
A CT scanner contains a rotating X-ray source and electronic detectors. As the equipment rotates around the patient's head, it records information from multiple directions. A computer converts these measurements into thin cross-sectional images. The radiologist can review the images in several anatomical planes and generate three-dimensional representations of the facial skeleton.
The 3D images are particularly useful for demonstrating the outer contours of the facial bones, displaced fragments and overall symmetry. However, very thin or non-displaced fractures may be easier to identify on the original CT slices. The radiologist therefore interprets the complete dataset rather than relying only on the surface-rendered images.
In facial trauma, CT may identify fractures involving the nasal bones, orbital walls, zygomatic complex, maxilla, mandible, frontal bone and sinus walls. The examination can show whether fragments are displaced or whether a fracture affects a joint, dental-bearing segment or adjacent cavity. Physical and specialist examinations remain essential because CT alone cannot determine vision, nerve function, dental stability or the complete functional impact of an injury.
A fracture involving the eye socket may be associated with trapped or displaced orbital tissue. CT can show the bony defect and the position of surrounding structures, but the patient's eye movement, vision and symptoms must be assessed directly. Similarly, jaw fractures can affect chewing, bite alignment and sensation, requiring evaluation by an oral and maxillofacial specialist.
CT may also demonstrate fluid or blood in the sinuses, soft-tissue swelling, air in abnormal locations and some foreign bodies. Not every foreign material is equally visible, and direct clinical examination may still be required. If a penetrating eye injury is suspected, patients should avoid pressure on the eye and seek emergency specialist care.
For congenital or developmental conditions, three-dimensional reconstruction can demonstrate asymmetry and abnormal bone shape. Surgeons may combine these images with photographs, dental models or other investigations when planning treatment. Imaging supports surgical planning but does not predict every functional or cosmetic outcome.
CT uses ionizing radiation. The dose depends on scan coverage, patient size and technical settings. The imaging team selects an appropriate protocol to obtain diagnostically useful images while avoiding unnecessary exposure. Providing earlier examinations can help comparison and reduce avoidable repeat scans.
After a routine non-contrast scan, patients can generally resume normal activities immediately. If contrast is administered, patients should follow the centre's instructions about hydration, observation and symptoms that require medical attention. The final report should be reviewed by the referring doctor or relevant specialist, who will interpret the findings alongside symptoms, examination results and other investigations.
