The term triphasic generally refers to imaging obtained during three contrast-related phases. Depending on the clinical requirement and local protocol, these may include the arterial phase, portal venous phase and delayed or equilibrium phase. A preliminary non-contrast series may also be acquired, but it is separate from the three post-contrast phases.

Different abdominal tissues and lesions absorb and release contrast at different rates. Comparing their appearance across multiple phases helps the radiologist evaluate enhancement patterns, blood supply, vascular relationships and contrast washout. This information can help distinguish among several types of liver lesions and guide further clinical management.

Although the liver is the principal focus, the scan may reveal abnormalities involving the pancreas, spleen, kidneys, adrenal glands, lymph nodes or major abdominal blood vessels. Some findings may require additional dedicated imaging.

Preparation Before Test

Preparation is important because the examination uses intravenous iodinated contrast. Patients are commonly instructed not to eat for approximately four to six hours before the appointment. A recent serum creatinine or estimated glomerular filtration rate test may be required to assess kidney function, particularly in patients with kidney disease, diabetes, advanced age or another relevant risk factor. Inform the radiology team about previous reactions to iodinated contrast, asthma, allergies, kidney disease, diabetes, heart disease, thyroid disorders, dehydration, recent severe illness or previous kidney surgery. Provide a complete list of prescription medicines, non-prescription products and supplements. Patients using metformin or other diabetes medicines should follow the instructions given by their doctor and radiology centre. Do not independently discontinue prescribed medication.

Inform the staff if you are pregnant, could be pregnant or are breastfeeding. CT uses ionising radiation, and iodinated contrast crosses the placenta. The examination should be performed during pregnancy only after an appropriate medical benefit-risk assessment.

Carry the referral, clinical history and previous ultrasound, CT, MRI, PET, laboratory, biopsy, surgery and treatment records. Prior images are especially important for assessing whether a liver lesion is new, stable, growing or responding to treatment.

Wear comfortable clothing without metal fasteners. Remove jewellery, belts and metallic objects from the scanning region. An intravenous cannula will be inserted into a vein, usually in the arm, for contrast administration.

Normal Reporting Time

The scanning appointment commonly takes approximately 20 to 40 minutes, although the actual image acquisition during each phase is much faster. Additional time is required for registration, intravenous cannula placement, contrast safety screening, positioning and precise phase timing.

The radiologist's report is generally available on the same day or within 24 hours. Complex cases may take longer when comparison with previous examinations, advanced image reconstruction or multidisciplinary review is necessary. Urgent or critical findings may be communicated to the referring doctor sooner.

A qualified radiologist reviews all phases together and prepares the final report. The report should be interpreted by the referring doctor or specialist together with the patient's symptoms, liver-function tests, tumour markers, clinical history and previous imaging.

Who Should Take This Test?

This examination may be appropriate for patients with an indeterminate liver lesion detected on ultrasound, CT or another imaging test. It may also be recommended for people with cirrhosis, chronic hepatitis or another chronic liver condition when a suspicious lesion requires detailed characterisation.

Patients with a known liver tumour may undergo the study for staging, treatment planning or post-treatment assessment. It may also be used when a person with another known cancer develops a suspected liver lesion and metastatic disease must be evaluated.

The scan should be performed only on medical advice because it involves ionising radiation and intravenous contrast. MRI liver with contrast or contrast-enhanced ultrasound may be more suitable in some situations. The referring doctor and radiologist select the examination most likely to answer the clinical question safely.

Patients with significantly reduced kidney function, a previous severe contrast reaction, pregnancy or another important risk factor require individual assessment. The examination may be modified, postponed or replaced with another imaging method when appropriate.

Detailed Information

During the procedure, the patient lies on a motorised table that moves through the circular CT scanner. A planning image is obtained, and the technologist confirms the anatomical coverage. A power injector administers iodinated contrast through the intravenous cannula at a controlled rate.

The arterial phase is timed to capture contrast within the arterial circulation. It helps evaluate hypervascular lesions, the hepatic arterial anatomy and the arterial blood supply of a mass. Precise timing is important because arterial enhancement occurs during a relatively short interval.

The portal venous phase is obtained after contrast has reached the portal venous system and enhanced the liver tissue more uniformly. This phase helps evaluate many liver lesions, portal vein patency, abdominal organs, lymph nodes and selected complications.

The delayed or equilibrium phase is acquired later. It helps assess whether a lesion retains contrast, becomes less enhanced than the surrounding liver or demonstrates a delayed scar or capsule. The exact timing varies according to the suspected condition and imaging protocol.

During contrast injection, the patient may experience temporary warmth, flushing or a metallic taste. These sensations are common and usually disappear quickly. Inform the technologist immediately if there is pain or swelling at the injection site, breathing difficulty, facial swelling, severe itching, dizziness or another unexpected symptom.

Most contrast reactions are mild, but serious allergic-like reactions are rare. Radiology departments maintain trained staff, emergency medicines and equipment to manage reactions. Contrast can also affect kidney function in susceptible patients, which is why risk assessment and kidney-function testing may be required.

The patient must remain still and may be asked to hold their breath briefly during each phase. Movement and inconsistent breathing can reduce image quality or make small lesions more difficult to evaluate.

The report may describe the liver's size, contour and density; the number, location and dimensions of lesions; their enhancement in each phase; and their relationship to blood vessels and bile ducts. It may also mention lymph nodes, ascites, portal hypertension, vascular thrombosis or abnormalities in other organs.

A triphasic CT can strongly suggest certain diagnoses, but it does not provide a definitive answer in every case. Small lesions, atypical enhancement patterns and changes caused by treatment or cirrhosis may remain indeterminate. MRI, contrast-enhanced ultrasound, interval follow-up or biopsy may be recommended.

CT uses ionising radiation, and multiphase imaging results in greater radiation exposure than a single-phase CT because several image sets are acquired. The protocol should therefore be medically justified and optimised according to the clinical question. Unnecessary repeat examinations should be avoided.

After the scan, the patient may usually return to normal activities. Drinking water may help maintain hydration unless fluid intake has been medically restricted. Follow any medicine-related instructions provided by the doctor. Seek medical advice if delayed rash, breathing difficulty, facial swelling, reduced urine output or persistent injection-site pain develops.
