CT ABDOMEN WITHOUT CONTRAST is a computed tomography examination that creates detailed cross-sectional images of the abdomen without injecting intravenous contrast material. The scan uses X-rays and computer processing to display abdominal organs, blood vessels, bones, fat and other structures in thin image slices. These images can be reviewed in several anatomical planes, helping the radiologist examine areas that may not be visible clearly on a conventional X-ray.
The scan may include the liver, gallbladder, spleen, pancreas, kidneys, adrenal glands, bowel, abdominal lymph nodes, abdominal wall, major blood vessels and portions of the spine. The exact anatomical coverage depends on the referring doctor's request and the protocol selected by the radiology team. If the pelvis must also be assessed, a combined CT abdomen and pelvis examination may be required rather than an abdomen-only scan.
The phrase “without contrast” generally means that intravenous iodinated contrast is not administered. This can be useful when contrast is unnecessary for the clinical question, when a non-contrast protocol is specifically required or when intravenous contrast cannot be used safely. However, non-contrast CT and contrast-enhanced CT provide different types of information. The referring doctor and radiologist determine which protocol is appropriate based on the suspected condition.

Why Doctors Recommend This Test
Doctors may recommend a CT abdomen without contrast for several clinical reasons. One common indication is suspected urinary tract stones. A non-contrast CT can detect many kidney and ureteric stones, show their size and location and demonstrate signs of urinary obstruction. When the clinical concern includes the ureters or urinary bladder, the scan may need to extend through the pelvis according to a dedicated urinary-stone protocol.
The examination may also be requested to evaluate calcification, selected types of internal bleeding, organ enlargement, bowel obstruction, abdominal wall abnormalities or findings identified on an ultrasound or X-ray. In some situations, it is performed as an initial scan before a separate contrast-enhanced phase. The exact protocol depends on the clinical question and should be selected by the radiologist.
A non-contrast CT may be considered when intravenous contrast cannot be administered because of a previous severe contrast reaction, impaired kidney function or another medical concern. Avoiding contrast does not remove the need for medical justification because CT still uses ionizing radiation. It also does not guarantee that every abdominal condition can be assessed adequately.
Intravenous contrast improves the visibility of blood vessels and the enhancement patterns of many organs, infections and masses. Consequently, a scan performed without contrast may be less sensitive for some liver, pancreatic, vascular, inflammatory or tumour-related abnormalities. If the non-contrast examination does not answer the clinical question, the doctor may recommend contrast-enhanced CT, ultrasound, MRI, endoscopy or another investigation.
Preparation Before Test
Preparation varies according to the reason for the scan and the protocol used by the diagnostic centre. Although intravenous contrast is not administered, patients may still be asked to avoid food for a few hours, particularly when the radiologist may need to modify the protocol or when the patient's symptoms could require additional assessment. Some abdominal CT protocols may use oral contrast to outline portions of the digestive tract, even when intravenous contrast is not used. Other protocols, including many urinary-stone examinations, may not require oral contrast. The test name alone should not be used to decide preparation. Patients can usually take prescribed medicines with a small amount of water unless advised otherwise. Do not stop any medicine without instructions from the treating doctor or imaging team. Inform the staff about diabetes, kidney disease, recent abdominal surgery, previous imaging procedures and any significant medical condition.
Pregnant patients or anyone who may be pregnant must notify the referring doctor and CT team before the examination. CT uses ionizing radiation, so the medical team will assess the urgency, expected benefit and possible alternative imaging methods. Ultrasound or MRI may answer some clinical questions without radiation, but they are not suitable replacements in every situation.
Bring the doctor's prescription, relevant laboratory results and previous imaging reports or films. Earlier ultrasound, X-ray, CT or MRI studies can be helpful for comparison. The prescription should state the clinical indication and whether the abdomen alone or both the abdomen and pelvis need examination.
Wear comfortable clothing and remove jewellery, belts or other metallic objects in the scan area when requested. During the examination, the patient lies on a motorized table that moves through the CT scanner. The technologist may ask the patient to hold their breath briefly. Remaining still and following breathing instructions helps prevent motion blur and improves image quality.
Normal Reporting Time
The image-acquisition portion of a CT abdomen is generally quick, often taking only a few minutes. Registration, preparation, positioning and review of the prescribed protocol can increase the total appointment time. If oral contrast is required, additional waiting time may be necessary before scanning.
After the scan, a radiologist reviews numerous cross-sectional images and reconstructed views before preparing the report. Many routine reports may be available within 24 to 48 hours. Emergency cases involving severe symptoms or suspected urgent abnormalities may be prioritized according to clinical need. Who Should Take This Test?
CT Abdomen Without Contrast may be appropriate for patients whose doctor suspects kidney or ureteric stones, selected calcifications, bowel obstruction, organ enlargement, abdominal wall abnormalities or another condition that can be evaluated with a non-contrast protocol. It may also be considered when intravenous contrast cannot be administered safely or when a baseline non-contrast study is clinically necessary.
The examination may be recommended for selected patients with abdominal or flank pain, nausea, vomiting, abdominal distension, urinary symptoms or abnormal findings on an earlier test. These symptoms have many possible causes, so CT should be selected only after appropriate medical assessment.
A non-contrast CT is not automatically the best investigation for every type of abdominal pain. Ultrasound may be preferred for certain gallbladder, pelvic or superficial conditions, while contrast-enhanced CT or MRI may be more informative for some infections, tumours and vascular abnormalities. The treating doctor should choose the examination according to the patient's symptoms, age, medical history and suspected diagnosis.
Children and patients requiring repeated imaging need special consideration because CT involves radiation. Age- and size-appropriate protocols should be used when the examination is necessary. Pregnant patients require individualized assessment before scanning.
Patients with severe or rapidly worsening abdominal pain, a rigid or markedly swollen abdomen, repeated vomiting, blood in vomit or stool, fainting, breathing difficulty, high fever with deterioration or symptoms of shock should seek urgent medical attention rather than waiting for a routine imaging appointment.
Detailed Information
A CT scanner contains a rotating X-ray source and electronic detectors. As the scanner rotates around the abdomen, it records information from multiple directions. A computer converts these measurements into thin cross-sectional images. The radiologist can inspect each slice and create coronal, sagittal and three-dimensional reconstructions when helpful.
Different abdominal tissues absorb X-rays to different degrees. Bone and calcification generally appear relatively bright, while air appears dark. Fat, fluid and soft tissues have intermediate appearances. This natural contrast allows many structures to be evaluated even without injected contrast material. However, intravenous contrast can make blood vessels and the enhancement of solid organs more conspicuous, which is why some conditions cannot be characterized fully on a non-contrast examination.
For suspected urinary stones, non-contrast CT can show many stones directly and demonstrate secondary signs such as dilation of the urinary collecting system or inflammation around an obstructed kidney. Some very small or unusual stones can still be difficult to characterize. The radiologist also evaluates the scan for alternative causes of symptoms within the covered region.
In suspected bowel obstruction, CT may demonstrate dilated bowel loops, air-fluid levels and a possible transition point. Nevertheless, the ability to identify reduced blood supply, inflammation or a subtle mass may be limited without intravenous contrast. If complications are suspected, the radiologist or treating doctor may recommend another protocol.
Non-contrast CT can identify calcification in organs, vessels or masses. It may also detect some forms of acute bleeding because fresh blood can appear relatively dense. The appearance varies with timing, location and the patient's condition, so a complete clinical assessment remains essential.
A non-contrast examination has limitations in evaluating many liver and pancreatic lesions, abdominal infections, inflammatory bowel conditions and vascular diseases. A normal non-contrast CT does not exclude every abdominal disorder. The report may recommend additional imaging when a finding cannot be characterized or when the prescribed protocol is insufficient for the suspected condition.
CT exposes the patient to ionizing radiation. The dose depends on the scan length, patient size, technical settings and whether more than one acquisition is required. The imaging team selects an appropriate protocol to obtain diagnostically useful images while limiting unnecessary exposure. Patients should bring previous imaging to facilitate comparison and reduce avoidable repeat scans.
Because intravenous contrast is not used, there is no risk of an allergic-like reaction to injected iodinated contrast during this specific examination. There is also usually no need for contrast-related kidney-function assessment. However, medical history remains important because preparation and protocol decisions may change based on the patient's symptoms and condition.
After a routine non-contrast scan, patients can generally eat, drink and resume normal activities immediately unless their doctor has provided other instructions.
