CT TEMPORAL BONE WITH PNS is a specialized computed tomography examination that evaluates the temporal bones together with the paranasal sinuses. PNS stands for paranasal sinuses, which are air-filled spaces located around the nose, cheeks, eyes and forehead. The combined scan provides detailed images of the ear structures, mastoid air cells and sinonasal anatomy during one imaging appointment.
The temporal bones are located on both sides of the skull and contain the external ear canals, middle ears, delicate hearing bones, inner-ear structures, facial nerve canals and mastoid air cells. These structures are extremely small and closely arranged, so a dedicated thin-slice CT technique is used to display their bony anatomy. The paranasal sinus portion evaluates the maxillary, ethmoid, frontal and sphenoid sinuses, along with their drainage pathways and surrounding bones.
This combined examination may be useful when ear and sinus symptoms occur together, when chronic sinonasal disease may be contributing to ear complaints or when the referring ENT specialist needs a broader assessment before treatment. The exact anatomical coverage and reconstruction technique depend on the clinical indication and protocol selected by the radiologist.

Preparation Before Test
Most CT Temporal Bone with PNS examinations are performed without intravenous contrast and require little preparation. Patients can generally eat, drink and take their regular medicines normally unless the diagnostic centre or treating doctor provides different instructions. Do not stop antibiotics, nasal sprays, inhalers, ear drops or other prescribed medicines without medical advice.
Bring the doctor's prescription and previous medical records. Relevant documents may include hearing-test results, tympanometry, nasal endoscopy findings, earlier CT or MRI studies, operative notes and ENT consultation reports. Comparison with previous examinations can help the radiologist determine whether an abnormality is new, stable, improving or progressing.
Remove earrings, spectacles, hearing aids, hairpins, removable dental appliances and other metallic objects around the head when instructed. Metal can create streak artifacts that obscure the small structures of the temporal bones and nearby sinuses. Inform the technologist about cochlear implants, hearing implants, previous ear surgery, facial hardware or other implanted devices.
Intravenous contrast is not routinely required for standard evaluation of bone anatomy, chronic ear disease or uncomplicated sinus disease. It may be added for selected concerns involving complications, a mass, a deep infection, vascular structures or abnormal soft-tissue enhancement. If contrast is prescribed, fasting and a recent kidney-function test may be required. Inform the team about kidney disease, diabetes, asthma, thyroid conditions, significant allergies and any previous contrast reaction.
Pregnant patients or anyone who may be pregnant must inform the referring doctor and CT staff before the examination. CT uses ionizing radiation, so the healthcare team will consider the urgency, expected benefit and possible alternatives. Children also require age- and size-appropriate protocols.
During the scan, the patient lies on a motorized table with the head positioned carefully. Cushions or supports may be used to minimize movement. A CT scanner uses a rotating X-ray source and electronic detectors to collect information from multiple directions around the head. A computer converts these measurements into thin cross-sectional images. High-resolution reconstruction algorithms emphasize the fine bony structures of the temporal bones, while additional views demonstrate the paranasal sinuses and their drainage pathways.
The external ear canal carries sound toward the eardrum. Behind the eardrum lies the middle ear, which contains three tiny ossicles called the malleus, incus and stapes. CT can demonstrate their shape and alignment and identify selected forms of erosion, fixation or developmental abnormality. Audiometry and other hearing tests remain necessary because CT does not measure hearing function.
The inner ear includes the cochlea, vestibule and semicircular canals. CT provides detailed assessment of their bony anatomy. MRI is generally more suitable for evaluating the auditory nerves, internal auditory canals and many soft-tissue abnormalities. CT and MRI may therefore be complementary.
The mastoid portion of the temporal bone contains multiple air cells connected to the middle ear. Chronic disease may cause these spaces to become filled or poorly aerated. CT may demonstrate sclerosis, erosion or postoperative change. Abnormal soft tissue within the middle ear or mastoid can have several causes, so the imaging appearance must be correlated with clinical findings.
The paranasal sinuses include the maxillary sinuses in the cheeks, frontal sinuses above the eyes, ethmoid air cells between the eyes and sphenoid sinuses deeper within the skull. Small drainage pathways connect the sinuses to the nasal cavity. Inflammation or anatomical narrowing can interfere with drainage and contribute to recurrent or persistent symptoms.
CT can demonstrate mucosal thickening, fluid levels, complete opacification, bony wall changes and certain nasal anatomical variations. These findings do not always correspond directly to symptom severity, and mild mucosal changes may occur incidentally. The ENT specialist interprets the report together with symptoms and nasal endoscopy.
For suspected cholesteatoma, CT may demonstrate soft tissue associated with erosion of the ossicles, scutum or other temporal-bone structures. Similar soft tissue can occur with inflammation or granulation tissue, so specialized diffusion-weighted MRI may be required for further assessment.
In temporal-bone trauma, CT may show fractures involving the ear canal, mastoid, ossicular chain, inner-ear capsule, facial nerve canal or carotid canal. A dedicated CT angiographic study may be required if vascular injury is suspected. Significant head trauma may also require a separate CT Brain.
Before cochlear implantation, CT can help evaluate cochlear anatomy, mastoid aeration, facial nerve canal position and congenital variations. MRI may be added to assess the auditory nerves and inner-ear soft tissues. Implant planning also requires detailed hearing tests and specialist evaluation.
CT Temporal Bone with PNS uses ionizing radiation. The dose depends on the scan coverage, patient size and technical settings. Because this combined examination covers more anatomy than a single-region study, it should be performed only when both components are clinically justified. The imaging team selects appropriate settings to obtain useful images while limiting unnecessary exposure.
After a routine non-contrast scan, patients can generally resume normal activities immediately. If contrast is administered, follow the centre's instructions about hydration, observation and concerning symptoms. The final report should be reviewed by the referring ENT specialist or doctor, who will correlate the imaging findings with ear examination, nasal endoscopy, hearing tests and other investigations.
