About this test
CT VENOGRAM LOWER LIMB is a contrast-enhanced computed tomography examination used to produce detailed images of the veins in one or both legs. Depending on the prescribed protocol, the scan may also include the pelvic veins and inferior vena cava, which is the major vein carrying blood from the lower body toward the heart. The examination can help doctors assess the course, size and openness of the deep veins and identify selected causes of impaired venous blood flow.
The test is performed after an iodine-based contrast material is administered through a vein. CT images are acquired at a carefully selected time so that the veins are adequately filled with contrast. A radiologist then examines thin cross-sectional images and reconstructed views to look for filling defects, narrowing, obstruction, compression, collateral veins and other abnormalities. The exact technique may vary according to the patient's symptoms, medical history and the clinical question stated on the prescription.
Ultrasound with venous Doppler is commonly the first imaging examination used when deep vein thrombosis is suspected because it does not require radiation or intravenous contrast. CT venography may be recommended in selected cases when ultrasound findings are inconclusive, when veins in the pelvis or abdomen need evaluation, when the patient's anatomy limits ultrasound assessment or when a broader vascular overview is required. The most suitable investigation should be selected by the treating doctor.

Why Doctors Recommend This Test
A doctor may recommend CT venography when a patient has leg swelling, pain, heaviness, skin discoloration or another symptom suggesting impaired venous drainage. It may be considered when the clinical findings and initial ultrasound results do not agree or when the suspected abnormality is located in a region that is difficult to examine adequately with ultrasound.
The examination may help assess suspected deep venous thrombosis, particularly when the iliac veins or inferior vena cava need evaluation. A thrombus is a blood clot formed within a blood vessel. On CT venography, a suspected clot may appear as an area where contrast does not fill the vein normally. The finding must be interpreted carefully because timing, blood flow and technical factors can sometimes produce appearances that resemble a filling defect.
CT venography may also be used to investigate venous compression. A vein can become narrowed when it is compressed by a nearby artery, bone, enlarged structure or mass. This may interfere with blood return and contribute to swelling or the formation of collateral veins. The wider anatomical coverage of CT can help demonstrate both the affected vein and surrounding structures.
Other possible indications include evaluation of chronic venous obstruction, congenital venous abnormalities, previous thrombosis, postoperative changes or planning before selected vascular interventions. The scan is not appropriate for every patient with leg pain or varicose veins. Clinical examination and, in many cases, Doppler ultrasound are required before the doctor decides whether CT venography will provide useful additional information.
Preparation Before Test
CT venography requires intravenous iodinated contrast, so preparation is important. The diagnostic centre may ask the patient to avoid food for several hours before the examination. Clear-fluid instructions can differ depending on the protocol and the patient's condition, so patients should follow the instructions provided when the appointment is confirmed.
A recent kidney-function test, usually including serum creatinine or estimated glomerular filtration rate, may be requested before contrast administration. This is particularly important for patients with known kidney disease, diabetes, dehydration, a single kidney, previous kidney surgery or other relevant risk factors. The medical team reviews the result and clinical history to determine whether contrast can be administered safely and whether additional precautions are necessary.
Tell the imaging team about any previous reaction to CT contrast, iodine-based contrast or another injected imaging agent. Also disclose asthma, significant allergies, thyroid disease, kidney problems, heart failure, diabetes and all current medicines. A previous contrast reaction does not automatically mean the scan cannot be performed, but the radiologist may need to evaluate the risk, recommend preparation or consider an alternative examination.
Patients taking metformin or other diabetes medicines should inform the centre. Do not stop any medicine independently. Instructions regarding medicines depend on kidney function, the contrast protocol and institutional policy, and should come from the treating doctor or radiology team.
Pregnant patients or anyone who may be pregnant must inform the referring doctor and CT staff before the scan. CT uses ionizing radiation, and the healthcare team must assess the urgency, expected benefit and possible alternatives. Breastfeeding patients should also inform the team so that current contrast-related guidance can be discussed according to their individual circumstances.
Bring the doctor's prescription, previous Doppler ultrasound reports, earlier CT or MRI examinations, laboratory results and relevant clinical records. Wear comfortable clothing and remove metallic items when requested. An intravenous cannula will be placed, usually in the arm or hand, for contrast injection.
During contrast administration, some patients experience a temporary feeling of warmth, a metallic taste or the sensation of passing urine. These effects usually disappear quickly. Tell the staff immediately if you experience breathing difficulty, throat tightness, facial swelling, severe itching, widespread rash, dizziness or significant pain and swelling around the injection site.
Normal Reporting Time
The scan itself is usually completed within a short time after preparation and intravenous access. The total appointment may take longer because the team must review the medical history, kidney-function results and contrast-safety information before proceeding. Additional observation may be required after the scan when clinically appropriate.
A radiologist reviews the source images and reconstructed views before issuing the report. Many routine reports may be available within 24 to 48 hours. Urgent suspected thrombosis or another time-sensitive vascular finding may be prioritized according to clinical need and the centre's communication procedures. Who Should Take This Test?
CT Venogram Lower Limb may be appropriate for selected patients with suspected venous obstruction, unexplained swelling of one or both legs, inconclusive Doppler ultrasound findings or a need to assess pelvic and abdominal veins. It may also be used for selected patients with previous thrombosis, chronic venous obstruction, congenital venous abnormalities or pre-procedure planning.
The examination should be performed only when recommended by a qualified doctor. It is not a routine screening test for people without symptoms and is not automatically required for uncomplicated varicose veins. Because it uses radiation and intravenous contrast, its expected diagnostic benefit should outweigh its risks.
Patients with sudden leg swelling, pain and tenderness require prompt medical assessment, particularly when symptoms affect only one leg. If these symptoms occur with sudden breathlessness, chest pain, fainting, coughing up blood or a rapid heartbeat, emergency medical care is necessary because a clot may have travelled to the lungs. A routine imaging appointment should not delay emergency evaluation.
Patients with reduced kidney function, a significant previous contrast reaction, pregnancy or certain medical conditions need individualized assessment before CT venography. An alternative test such as Doppler ultrasound or magnetic resonance venography may be considered depending on the clinical question and available facilities.
Detailed Information
A CT scanner uses a rotating X-ray source and electronic detectors to collect information from many directions around the body. In CT venography, iodinated contrast is injected into a peripheral vein. Images are acquired after an appropriate delay so that the contrast circulates through the lower-limb venous system. Timing is essential because images obtained too early or too late may not show the veins optimally.
The radiologist examines the deep veins and other included vascular structures for normal contrast filling and continuity. A potential thrombus may appear as a filling defect, while chronic obstruction may be associated with narrowing, irregularity or collateral vessel formation. The images can also show surrounding muscles, bones, lymph nodes and other tissues, which may help identify an external cause of venous compression.
The veins evaluated depend on scan coverage. A lower-limb study may include the calf, popliteal and femoral venous systems, while an extended protocol may cover the iliac veins and inferior vena cava. The referring doctor should clearly state the suspected condition and required anatomical coverage. Imaging both legs or extending the scan into the abdomen affects radiation exposure and should be performed only when clinically justified.
CT venography has important limitations. Slow blood flow or uneven mixing of contrast can occasionally resemble thrombosis. Small calf-vein clots may be difficult to characterize depending on the protocol. Ultrasound also provides information about vein compressibility and real-time blood flow that CT does not provide in the same way. Radiologists therefore interpret CT findings alongside symptoms, laboratory results, ultrasound findings and other clinical information.
The examination exposes the patient to ionizing radiation. Radiation dose varies with scan coverage, patient size and technical settings. The imaging team uses an appropriate protocol to obtain diagnostically useful images while limiting unnecessary exposure. The benefit of identifying a clinically important venous abnormality may outweigh the radiation risk when the scan is properly indicated.
Intravenous contrast is generally well tolerated, but mild reactions such as nausea, itching or a limited rash can occur. Severe allergic-like reactions are uncommon but require immediate treatment. Contrast can also affect kidney function in susceptible individuals, which is why clinical history and kidney-test results may be reviewed beforehand. Contrast leakage outside the vein can cause local pain and swelling and should be reported immediately.
After the examination, patients may be advised to drink fluids if medically permitted. People with fluid restrictions, kidney disease or heart failure should follow their doctor's specific advice rather than increasing fluid intake independently. Normal activities can usually be resumed unless the treating doctor has provided other restrictions.
The scan report should be reviewed promptly by the referring physician. Imaging alone does not determine treatment. If thrombosis or significant obstruction is identified, the doctor considers the location, extent, symptoms, bleeding risk and overall medical condition before recommending anticoagulation, intervention, observation or further investigation.
