About this test
The Apolipoprotein B (Apo B) test is a blood test that measures the amount of apolipoprotein B, the main structural protein found on potentially artery-damaging lipoprotein particles. These include low-density lipoprotein, very-low-density lipoprotein, intermediate-density lipoprotein, lipoprotein(a) and their remnants. Each of these atherogenic particles generally carries one Apo B molecule, so the measured Apo B concentration provides an estimate of the total number of circulating particles capable of entering artery walls.
A standard lipid profile measures the amount of cholesterol carried within different lipoprotein groups. Apo B provides related but distinct information because it reflects particle number rather than only the cholesterol content inside those particles. Two people with a similar LDL cholesterol value can have different Apo B levels if their LDL particles differ in size or cholesterol content. A higher number of Apo B-containing particles generally indicates greater opportunity for cholesterol to enter artery walls and contribute to atherosclerosis.

Why Doctors Recommend This Test
A doctor may recommend Apo B testing when a routine lipid profile does not provide a complete picture of cardiovascular risk. It can be useful when LDL cholesterol and other lipid markers appear inconsistent, a situation sometimes described as discordance. For example, a patient may have an LDL cholesterol value that appears acceptable but an elevated number of cholesterol-carrying particles.
Testing may be considered for people with high triglycerides, diabetes, insulin resistance, obesity, metabolic syndrome, chronic kidney disease or a strong personal or family history of premature cardiovascular disease. It may also be requested after a heart attack, stroke or diagnosis of peripheral arterial disease to help guide ongoing prevention strategies.
Apo B may also be used to monitor treatment in selected patients taking statins or other lipid-lowering medicines. Treatment decisions are not based on Apo B alone. Doctors consider the complete lipid profile, blood pressure, smoking status, diabetes, kidney health, age, family history and any established cardiovascular disease.
Preparation Before Test
A routine blood sample is required. Fasting is not always necessary for Apo B testing, but your doctor may request an eight-to-twelve-hour fast when a full fasting lipid profile or other fasting investigations are being performed at the same time. During a prescribed fast, plain water is generally permitted.
Maintain your usual diet unless your doctor provides different instructions. Inform the healthcare team about cholesterol-lowering medicines, hormones, supplements, pregnancy, recent serious illness and all other treatment. Do not stop statins or any prescribed medication before testing unless advised by the treating doctor.
Acute illness, infection, recent surgery and major changes in weight or diet may temporarily affect lipid-related measurements. Your doctor may recommend waiting until your health is stable when the test is intended for long-term cardiovascular-risk assessment.
Normal Reporting Time
The Apolipoprotein B test is generally reported on the same day or within 24 hours after the sample reaches the laboratory. Who Should Take This Test?
This test may be appropriate for people with elevated cholesterol or triglycerides, diabetes, obesity, metabolic syndrome, kidney disease or a strong family history of early heart disease. It may also benefit patients whose LDL cholesterol does not appear to match their overall cardiovascular-risk profile.
People with established coronary artery disease, previous stroke or peripheral arterial disease may undergo Apo B testing when their doctor requires an additional marker for treatment monitoring. It is not a standalone screening or diagnostic test, and its usefulness depends on the person's broader risk profile.
Detailed Information
A higher Apo B concentration generally indicates a greater number of atherogenic particles and may be associated with increased cardiovascular risk. A lower value usually indicates fewer of these particles. However, target values vary according to the laboratory method, medical guidelines and the patient's level of cardiovascular risk. People at very high risk may be given lower treatment targets than those receiving primary prevention.
Apo B is different from Apolipoprotein A1, the principal protein in high-density lipoprotein particles. Some doctors may evaluate both proteins or calculate an Apo B to Apo A1 ratio, but this ratio is not required in every patient. Apo B also includes the particle contribution from lipoprotein(a), although a separate lipoprotein(a) test is needed to measure that inherited risk marker specifically.
The result should be interpreted with total cholesterol, LDL cholesterol, HDL cholesterol, triglycerides, non-HDL cholesterol and relevant clinical factors. An elevated Apo B value does not prove that an artery is blocked, and a result within the reference interval does not eliminate cardiovascular risk. Imaging, examination and other tests may be required when symptoms or clinical findings suggest heart or blood-vessel disease.
