About this test
The HOMOCYSTEINE - PLASMA test measures the concentration of homocysteine in plasma. Homocysteine is a sulfur-containing amino acid produced during the normal metabolism of methionine, an amino acid obtained from protein-containing foods. The body normally converts homocysteine into other useful substances with the help of vitamin B12, folate and vitamin B6.
When these vitamins are insufficient, when kidney function is reduced or when certain genetic, medical or lifestyle factors are present, homocysteine can accumulate in the blood. This condition is known as hyperhomocysteinemia. Elevated homocysteine has been associated with vascular disease and blood-clotting risk, but it is not a stand-alone test for diagnosing heart disease or predicting whether a cardiovascular event will occur.
The test is commonly requested while investigating possible vitamin B12 or folate deficiency, unexplained blood clots, premature vascular disease or suspected inherited homocysteine metabolism disorders. Results must be interpreted with symptoms, medical history and other laboratory investigations.

A doctor may recommend the test when vitamin B12 or folate deficiency is suspected, especially if routine vitamin measurements are borderline or symptoms do not match the initial results. Because these vitamins are involved in homocysteine metabolism, inadequate availability can cause the concentration to rise.
The test may also be requested for people who develop vascular disease or blood clots at a younger age than expected, have recurrent thrombosis or have a strong family history of an inherited metabolic disorder. Markedly elevated results, particularly in children or young adults with compatible symptoms, may require specialised evaluation for homocystinuria or another rare metabolic condition.
For cardiovascular assessment, homocysteine is interpreted as one potential risk-related marker among many. Blood pressure, cholesterol, diabetes, smoking, kidney function, family history, physical activity and body weight generally remain central to cardiovascular risk evaluation. Lowering homocysteine does not automatically eliminate cardiovascular risk.
Preparation Before Test
Fasting for approximately 8–12 hours may be recommended to standardise the result, although requirements can vary. Plain water is generally permitted. Follow the exact instructions provided by your doctor or Focus Diagnostics.
Tell the doctor about vitamin B12, folic acid, vitamin B6, multivitamins, methionine supplements and all prescription medicines. These can influence the result. Do not stop any prescribed medicine or supplement unless instructed by the treating doctor.
Avoid unusually strenuous exercise, excessive alcohol and smoking before sample collection if advised, as these may influence homocysteine concentration. Inform the healthcare team about pregnancy, kidney disease, thyroid disease, recent illness and any history of blood clots.
The test requires a venous blood sample collected into an appropriate tube. Prompt separation of plasma from blood cells is important because blood cells can continue releasing homocysteine after collection. Laboratory handling procedures are therefore essential for an accurate result.

Detailed Information
Homocysteine lies at an important point in methionine metabolism. It may be remethylated back to methionine through pathways that require folate and vitamin B12, or converted toward cysteine through pathways involving vitamin B6. Deficiency or impaired use of these vitamins can interfere with these reactions and increase plasma homocysteine.
A raised result may occur with vitamin B12, folate or vitamin B6 deficiency. Other possible influences include reduced kidney function, hypothyroidism, smoking, increasing age, certain medicines, genetic variants and rare inherited enzyme deficiencies. Because multiple factors can produce the same result, an elevated value does not identify its cause by itself.
Mild or moderate elevation is more common than severe elevation. Markedly high concentrations may require urgent medical evaluation, particularly when an inherited disorder is suspected. The degree of elevation, patient age, symptoms and family history guide the next steps.
Doctors may request vitamin B12, folate, methylmalonic acid, complete blood count, kidney-function tests, thyroid-function tests or genetic investigations alongside homocysteine. Methylmalonic acid can be particularly useful when vitamin B12 deficiency is suspected because it may help distinguish B12-related metabolic impairment from folate deficiency.
Although elevated homocysteine has been associated with coronary artery disease, stroke, peripheral vascular disease and thrombosis, the test cannot determine whether a person currently has a blocked artery or blood clot. Symptoms such as sudden chest pain, breathing difficulty, facial weakness, speech difficulty or one-sided limb swelling require urgent medical assessment rather than waiting for this test.
Reference intervals vary according to the laboratory method, fasting status, age and clinical context. A result within range does not exclude cardiovascular disease, and an elevated result does not prove that a cardiovascular event will occur. It should be reviewed by a qualified healthcare professional.
Treatment depends on the underlying cause. A doctor may recommend dietary changes, vitamin replacement or investigation of kidney, thyroid or inherited conditions. Patients should not begin high-dose vitamin therapy solely on the basis of an unreviewed result, as excessive supplementation can have adverse effects or mask another deficiency.
