Lipoprotein(a) [Lp(a)] Test

The Lipoprotein(a), commonly abbreviated as Lp(a), test is a blood investigation that measures the concentration of a genetically influenced cholesterol-carrying particle associated with an increased lifetime risk of atherosclerotic cardiovascular disease and calcific aortic valve stenosis. An Lp(a) particle resembles low-density lipoprotein, or LDL, because both contain cholesterol and apolipoprotein B. Preparation Before the Test

Fasting is generally not required when Lp(a) is measured alone. If the test is ordered with triglycerides, a complete lipid profile, glucose, or another fasting investigation, the doctor may request an 8 to 12-hour fast.

What Happens During the Test?

A trained phlebotomist cleans the skin over a suitable vein, usually in the arm, and collects a small blood sample using a sterile needle. The procedure generally takes only a few minutes.

After collection, the needle is removed and pressure is applied to the site. Mild discomfort, bruising, tenderness, or minor bleeding may occur. Serious complications are uncommon.

Elevated Lp(a): Values of approximately 50 mg/dL or 125 nmol/L and above are commonly considered elevated and a cardiovascular risk-enhancing factor. The effect is more important when other risks are also present.

Very High Lp(a): Markedly increased concentrations can indicate substantial inherited lifetime risk. The doctor may recommend intensive management of LDL cholesterol and other modifiable risk factors, assessment for existing cardiovascular disease when clinically appropriate, and testing of close relatives.

The result does not specify whether plaque or aortic valve narrowing is already present. Additional assessment may include standard lipids, apolipoprotein B, blood pressure, glucose or HbA1c, kidney function, ECG, imaging, coronary calcium assessment, or echocardiography, depending on the clinical situation.

Clinical Limitations
