About this test
The TZANCK SMEAR is a rapid and simple cytological test used to diagnose certain viral skin infections, especially those caused by the Herpes Simplex Virus (HSV) and Varicella Zoster Virus (VZV). It is performed on samples taken from fresh skin lesions such as blisters, erosions, or ulcers.
In this test, cells are scraped from the base of a skin lesion and placed on a glass slide. The sample is then stained and examined under a microscope to identify characteristic cellular changes such as multinucleated giant cells, ballooning degeneration, and nuclear molding, which suggest viral infection.
Tzanck smear is a quick, cost-effective, and widely used diagnostic tool in dermatology for initial evaluation of vesicular skin lesions.
Benefits of the Test
- Rapid diagnosis of viral skin infections
- Helps detect herpes simplex and herpes zoster
- Simple, low-cost diagnostic procedure
- Useful in emergency dermatology cases
- Supports early treatment initiation
- Minimally invasive and quick reporting
Why Doctors Recommend This Test
Doctors recommend Tzanck smear when patients present with painful skin blisters, recurrent ulcers, or vesicular eruptions. It helps quickly identify viral causes, especially herpes infections, allowing doctors to start antiviral treatment without delay. It is often used as a first-line diagnostic test in dermatology before confirmatory PCR or serology tests.
Preparation Before Test
- No fasting required
- Do not apply creams or ointments on lesion before test
- Sample is collected from fresh, unruptured lesions if possible
- Inform doctor about ongoing medications
- Avoid self-treatment on affected area before consultation
Normal Reporting Time
The Tzanck smear test is a rapid diagnostic test, and results are usually available within 1–4 hours, depending on laboratory setup.
