About this test
The C-PEPTIDE - FASTING test is a blood test used to estimate how much insulin the pancreas is producing naturally. Insulin is initially created as a larger molecule called proinsulin. When proinsulin is divided, it releases insulin and C-peptide into the bloodstream in approximately equal amounts. Because C-peptide generally remains in circulation longer than insulin, measuring it can provide a useful indication of the body's own insulin production.
This test is commonly used during the evaluation of diabetes, unexplained low blood sugar and disorders affecting pancreatic beta cells. It may help doctors distinguish between reduced natural insulin production and insulin resistance. It can also provide useful information for people who already receive insulin injections because injected insulin does not contain C-peptide.
The test is performed after fasting so that recent food intake has less influence on insulin secretion. A single result does not independently diagnose a particular type of diabetes or determine treatment. Doctors interpret it with the fasting glucose level, medical history, symptoms, current medicines, kidney function and other laboratory findings.

A normal or high result with elevated glucose may indicate that insulin is being produced but the body's tissues are not responding effectively, as can occur with insulin resistance.
The test may also be requested when a person experiences sweating, trembling, dizziness, confusion, weakness or other symptoms associated with low blood sugar. C-peptide measured alongside glucose and insulin can help determine whether excess insulin is being produced within the body or whether another explanation should be investigated.
In people receiving diabetes treatment, the result may help assess residual beta-cell activity. However, treatment should never be started, stopped or changed solely on the basis of a C-peptide result without clinical advice.
Preparation Before Test
Fasting is generally required for approximately 8–12 hours before sample collection, unless the referring doctor or diagnostic centre provides different instructions. During the fasting period, plain water is usually permitted, but food, tea, coffee, juice, sweetened drinks, chewing gum and smoking should be avoided.
Tell the doctor or laboratory team about insulin, diabetes medicines, supplements and all other medications being taken. Do not stop prescribed medicines unless instructed by the treating doctor. Recent exercise, acute illness, stress, pregnancy, kidney impairment and medicines that affect blood glucose may influence interpretation. Detailed Information
C-peptide is released when pancreatic beta cells convert proinsulin into active insulin. Unlike insulin, C-peptide is not significantly removed during its first passage through the liver, and it usually remains measurable for longer. These properties make it a practical marker of endogenous, or internally produced, insulin secretion.
A low fasting C-peptide level may be associated with reduced beta-cell activity, particularly when the blood glucose level is elevated. This pattern may occur in type 1 diabetes, advanced beta-cell failure or after removal or substantial damage of pancreatic tissue. A normal or raised level may be seen when the pancreas continues to produce insulin, including in insulin resistance and many cases of type 2 diabetes.
A raised value during hypoglycemia can require further evaluation for inappropriate internal insulin secretion. Kidney function is important because the kidneys help clear C-peptide; impaired renal function may cause higher concentrations. Laboratory reference intervals also differ according to the testing method, fasting requirements and measurement units.
The result must therefore be interpreted in context. Doctors may review C-peptide together with fasting glucose, insulin, HbA1c, diabetes-related antibodies, kidney-function tests or a stimulated C-peptide study. The fasting and stimulated tests answer related but different clinical questions, so the appropriate method should be selected by the treating clinician.
