The Glucose - Fasting test measures the amount of glucose present in the blood after a defined period without food or calorie-containing drinks. Glucose is the body's primary source of energy and is obtained mainly from carbohydrates in food. The hormone insulin helps move glucose from the bloodstream into cells, where it can be used or stored.
After an overnight fast, the body normally maintains blood glucose within a controlled range. A persistently elevated fasting result may indicate impaired fasting glucose, commonly called prediabetes, or diabetes. A low result may suggest hypoglycaemia and requires interpretation based on symptoms, medicines, fasting duration and other medical factors.
Diabetes can develop without obvious early symptoms. When symptoms occur, they may include increased thirst, frequent urination, unexplained weight loss, tiredness, blurred vision, slow wound healing or recurrent infections. Fasting glucose testing is therefore commonly used for screening people who have risk factors even when they feel well.

Preparation Before Test

During the fasting period, do not consume food, tea, coffee, juice, milk, alcohol, chewing gum, sweets or any calorie-containing product. Plain water is usually permitted and can help maintain hydration.
Avoid smoking and strenuous exercise before sample collection because they may temporarily influence glucose levels. Schedule the sample for the morning when practical. Inform the healthcare provider about diabetes medicines, insulin and all other medications. Do not skip, delay or change prescribed treatment unless the doctor has given specific instructions, as medication plans vary between patients.
Tell the laboratory if the required fasting period was not completed, if any calories were consumed or if illness, stress, pregnancy or recent medication changes may affect the result. Excessively prolonged fasting is not recommended unless specifically directed by a healthcare professional.

Glucose is tightly controlled by insulin, glucagon and several other hormones. After fasting, the liver releases stored glucose to maintain energy for the brain and other organs. Insulin helps prevent glucose from rising excessively. In insulin resistance, body tissues do not respond effectively to insulin, and the pancreas may initially compensate by producing more. Over time, blood glucose can rise into the prediabetes or diabetes range.
For non-pregnant adults, commonly used clinical criteria consider fasting plasma glucose below 100 mg/dL within the usual normal range, 100–125 mg/dL consistent with impaired fasting glucose, and 126 mg/dL or higher within the diabetes range. A diabetes-range result usually requires confirmation on another day unless clear symptoms and other clinical evidence are present. Laboratories and clinical circumstances may require different interpretation.
A result below the laboratory reference interval may occur because of diabetes medicines, prolonged fasting, alcohol, severe liver or kidney disease, hormonal disorders or other medical conditions. Symptoms of significant hypoglycaemia require prompt medical attention.
Temporary glucose elevations may occur during acute illness, infection, surgery, significant stress or treatment with medicines such as corticosteroids. One result should therefore be interpreted together with symptoms, medication history and other investigations. A doctor may recommend repeat fasting glucose, HbA1c, an oral glucose tolerance test or additional testing before making a diagnosis or changing treatment.
