About this test
The Glucose - Random test measures the concentration of glucose in the blood at the time the sample is collected, regardless of when the person last ate. It is also known as a random blood sugar or RBS test. Unlike a fasting glucose test, it normally does not require an overnight fast and can be performed whenever symptoms occur or a rapid assessment is needed.
Glucose is the body's principal source of energy. After food is digested, glucose enters the bloodstream and the pancreas releases insulin, which helps move it into cells. Blood glucose normally changes throughout the day according to meals, physical activity, stress, medicines and health status. Because a random result is affected by the timing and content of recent food, it is interpreted differently from a fasting result.
A high random glucose value may indicate hyperglycaemia and can support the evaluation of diabetes, particularly when typical symptoms are present. A low value may indicate hypoglycaemia, especially when accompanied by sweating, trembling, dizziness, confusion, weakness or fainting. Abnormal findings may require repeat measurement and additional investigations.

It is useful when waiting for a fasting test would unnecessarily delay the initial assessment.
The test may also be ordered when symptoms suggest low blood glucose. These symptoms can include sweating, trembling, hunger, palpitations, dizziness, irritability, confusion, weakness, seizures or loss of consciousness. Severe symptoms require urgent medical evaluation and should not be managed by waiting for a routine laboratory report.
People with established diabetes may undergo random glucose testing during illness, after changes in treatment or when their symptoms do not match their usual readings. However, a single random value does not show long-term glucose control. HbA1c is commonly used to estimate average glucose over the previous two to three months.
Preparation Before Test
No fasting or special dietary preparation is generally required. The test can be performed regardless of when the patient last ate. A blood sample is collected from a vein in the arm. For accurate interpretation, tell the laboratory or doctor approximately when and what you last ate or drank.
Inform the healthcare provider about insulin, diabetes tablets, corticosteroids and all other medicines or supplements. Do not skip or change prescribed medication unless specifically instructed by the treating doctor. Also report recent strenuous exercise, alcohol consumption, acute illness, pregnancy, surgery or significant stress because these factors may influence blood glucose.
If symptoms of severe hyperglycaemia or hypoglycaemia are present, seek immediate medical care. Severe thirst with vomiting, abdominal pain, deep breathing, confusion, marked drowsiness, seizures or unconsciousness can represent a medical emergency.

Random glucose is a routine biochemistry test and is commonly reported within a few hours on the same day. The exact turnaround time may depend on the sample collection time, laboratory workflow, sample quality and whether repeat verification is necessary.
Who Should Take This Test?
This test may be appropriate for people experiencing symptoms of high or low blood glucose, individuals with diabetes who require laboratory monitoring and patients undergoing an initial assessment when fasting is not practical. It may also be included in a broader medical evaluation or health check.
People with obesity, hypertension, abnormal cholesterol, polycystic ovary syndrome, a sedentary lifestyle, previous gestational diabetes or a family history of diabetes have an increased risk of type 2 diabetes. A clinician may use a random result as an initial assessment but may recommend fasting glucose or HbA1c for formal screening.

Random glucose results vary with the timing of the last meal. A value obtained soon after eating may be higher than one measured several hours later. For this reason, random results do not use the same reference ranges as fasting glucose. The laboratory report should be interpreted with the patient's symptoms, meal timing, medicines and clinical history.
In a person with classic symptoms of hyperglycaemia or a hyperglycaemic crisis, a random plasma glucose of 200 mg/dL or higher may support a diagnosis of diabetes. When clear symptoms are absent, diagnosis generally requires confirmation using repeat testing, fasting plasma glucose, HbA1c or an oral glucose tolerance test. A value below 200 mg/dL does not automatically exclude diabetes.
Temporary elevations may occur during infection, severe illness, surgery, emotional or physical stress and treatment with medicines such as corticosteroids. High results may also occur when diabetes is undiagnosed or inadequately controlled. Depending on the degree of elevation, a doctor may request HbA1c, fasting glucose, urine or blood ketones, kidney function tests or other investigations.
Low glucose may occur because of insulin or diabetes medicines, delayed meals, prolonged fasting, alcohol, strenuous activity, liver or kidney disease, hormonal disorders or other medical conditions. The clinical significance depends on the measured value and whether compatible symptoms occur at the same time.
One random result should not be used to start, stop or change treatment without professional guidance. A healthcare provider should review the result together with symptoms, medication use, previous reports and additional laboratory findings.
