Prolactin Test

The Prolactin test measures the concentration of prolactin in a blood sample. Prolactin is a hormone produced mainly by the pituitary gland, a small gland located at the base of the brain. Its best-known function is stimulating breast development and milk production during pregnancy and after childbirth. Prolactin also interacts with reproductive hormones and can influence ovulation, menstrual periods, fertility, testosterone production and sexual function.

Prolactin is normally present in both women and men, although expected concentrations differ according to sex, pregnancy status and laboratory method. Levels naturally rise during pregnancy and breastfeeding. Sleep, emotional stress, exercise, sexual activity, nipple stimulation, breast examination and even anxiety associated with blood collection may temporarily increase prolactin.

A healthcare professional may request this test when investigating unexpected breast-milk production, irregular or absent menstrual periods, infertility, reduced sexual desire, erectile dysfunction, low testosterone, headaches or vision changes. It may also be used to evaluate pituitary function, investigate a suspected prolactinoma or monitor treatment of a known prolactin-related condition.

What Is Prolactin?

Prolactin is a peptide hormone produced by specialised lactotroph cells in the anterior pituitary gland. Its release is normally controlled by dopamine from the hypothalamus. Dopamine generally suppresses prolactin secretion. Medicines or medical conditions that reduce dopamine activity can therefore increase the prolactin level.

During pregnancy, hormonal changes stimulate the pituitary gland and prolactin concentrations rise. After delivery, breastfeeding and nipple stimulation trigger further prolactin release, supporting milk production. Prolactin levels usually remain higher in people who breastfeed and gradually decrease when breastfeeding becomes less frequent or stops.

Outside pregnancy and breastfeeding, persistently elevated prolactin may interfere with the release of reproductive hormones from the hypothalamus and pituitary gland. This can affect ovulation, menstrual cycles, oestrogen production and testosterone production. The clinical effects depend on the degree and duration of the elevation and the underlying cause.

Why Is the Prolactin Test Performed?

The test is commonly used to investigate symptoms of hyperprolactinaemia, which means an abnormally increased blood prolactin concentration. In women, possible symptoms include milk production unrelated to childbirth or breastfeeding, irregular periods, absent periods, infertility, vaginal dryness or reduced sexual desire. In men, possible symptoms include reduced libido, erectile dysfunction, infertility, reduced facial or body hair and breast enlargement or discharge.

A prolactin measurement may be requested when a clinician suspects a prolactinoma, a usually benign pituitary tumour that produces prolactin. Depending on its size, a pituitary tumour may also cause headaches, changes in peripheral vision or abnormalities involving other pituitary hormones. An elevated prolactin result alone does not prove that a tumour is present.

The test may also help assess the effect of medicines known to raise prolactin, evaluate pituitary or hypothalamic disorders and monitor response to treatment. Patients with a known prolactinoma may undergo periodic prolactin testing to help determine whether medication is controlling hormone production.

Who May Need This Test?

A Prolactin test may be considered for women with unexplained milk discharge, irregular or absent menstrual periods, difficulty becoming pregnant or symptoms of reduced oestrogen. It may be considered for men with low testosterone, reduced sexual desire, erectile dysfunction, infertility or unexplained breast changes.

The test may also be requested for anyone with symptoms suggesting a pituitary mass, such as persistent headaches or unexplained visual-field changes. Because these symptoms have many possible causes, the doctor will determine whether prolactin testing, other pituitary-hormone tests or imaging is appropriate.

What Sample Is Required?

A venous blood sample is required. Serum is commonly used for prolactin measurement. The blood is generally collected in a serum-separator or red-top tube and processed according to laboratory requirements. The patient's age, sex, pregnancy status, symptoms and relevant medicines should be provided when requested.

Prolactin follows a daily rhythm and is affected by sleep. For greater consistency, sample collection may be scheduled a few hours after waking. The requesting doctor or laboratory may recommend morning collection or a specific time. When monitoring a known condition, collecting samples under similar conditions can improve comparability.

How Is the Prolactin Test Performed?

A trained phlebotomist collects blood from a vein, usually in the arm. An elastic band may be applied to make the vein easier to identify. After cleaning the skin, a sterile needle is inserted and a small amount of blood is collected into the appropriate tube. The procedure generally takes less than five minutes.

Some people experience brief discomfort, minor bruising, light-headedness or slight bleeding at the puncture site. Serious complications are uncommon. Patients with a bleeding disorder, those taking anticoagulants or those who have previously fainted during blood collection should inform the collection staff.

Is Fasting Required?

Fasting requirements can vary. Some patients may be asked to avoid food for several hours or provide an early-morning fasting sample, especially when confirming a mildly elevated result. Patients should follow the specific instructions provided by their doctor or laboratory.

Stress and physical stimulation can temporarily increase prolactin. The patient may be asked to rest quietly before sample collection and avoid strenuous exercise, sexual activity, nipple stimulation or breast examination for an appropriate period. Exact instructions should come from the healthcare professional.

High-dose biotin may interfere with certain laboratory immunoassays. Patients should disclose multivitamins and hair, skin or nail supplements containing biotin. They should also provide a complete list of prescription medicines, non-prescription products and recreational substances. Medicines must not be stopped unless the treating doctor gives clear instructions.

Medicines That May Affect Prolactin

Several medicines can raise prolactin by reducing dopamine activity or through other mechanisms. Examples may include certain antipsychotics, antidepressants, anti-nausea medicines such as metoclopramide, opioid medicines, oestrogen-containing products, verapamil and some acid-reducing medicines. The effect differs according to the medicine, dosage and patient.

A raised result should therefore be reviewed alongside the medication history. Patients should never discontinue psychiatric, hormonal or other prescribed treatment without medical supervision. The clinician will decide whether the medicine is a likely contributor and whether repeat testing, dose adjustment or an alternative treatment is appropriate.

Understanding the Prolactin Result

The result is commonly reported in nanograms per millilitre or milli-international units per litre. Reference intervals differ between laboratories, methods, sexes and pregnancy states. The result should always be compared with the interval printed on the laboratory report.

A single mildly elevated result does not automatically indicate disease. Prolactin can temporarily rise because of stress, sleep, exercise, recent food intake, sexual activity, nipple stimulation or the blood-collection experience. If the elevation is mild or unexpected, the doctor may request a repeat sample under more controlled conditions.

The degree of elevation may help guide further evaluation, but no single cut-off establishes every diagnosis. The doctor will consider symptoms, pregnancy status, medicines, thyroid function, kidney and liver health and other pituitary findings before deciding on further tests.

What Can Cause a High Prolactin Level?

Normal physiological causes include pregnancy, breastfeeding and sleep. Temporary increases may occur with physical or emotional stress, vigorous exercise, sexual activity, nipple stimulation, breast examination, chest-wall irritation or the stress of venepuncture.

Medical causes may include a prolactinoma or another pituitary-region mass that interferes with dopamine delivery to the pituitary. Primary hypothyroidism can also increase prolactin and should be considered during evaluation. Chronic kidney disease, certain liver disorders, polycystic ovary syndrome, seizures and chest-wall conditions may be associated with elevated levels in some patients.

Medication-related elevation is common and must be assessed carefully. A high prolactin concentration cannot distinguish among these causes by itself. The healthcare professional may request repeat testing, pregnancy testing, thyroid-function tests, kidney and liver tests, macroprolactin analysis, other pituitary hormones or pituitary imaging.

What Can Cause a Low Prolactin Level?

A low prolactin concentration is generally less common and may have limited clinical importance when there are no symptoms. It can occur with reduced pituitary function or as an effect of medicines that increase dopamine activity. In a person who has recently delivered, an unusually low level may be considered if there is difficulty producing breast milk.

The result should not be interpreted independently. If broader pituitary dysfunction is suspected, the doctor may assess cortisol, ACTH, TSH, free thyroxine, LH, FSH, testosterone, oestradiol, growth-hormone-related markers and other relevant tests.

Factors That Can Affect Test Accuracy

  • Collection shortly after waking or during a period of sleep-related elevation.
  • Emotional stress, pain or anxiety during blood collection.
  • Strenuous exercise before the test.
  • Recent sexual activity or nipple stimulation.
  • Pregnancy, breastfeeding or recent childbirth.
  • Medicines that alter dopamine or hormone activity.
  • High-dose biotin supplements with susceptible immunoassays.
  • Macroprolactin or other analytical interference.
  • Differences between laboratory methods and reference intervals.

If an elevated result does not match the patient's symptoms, repeating the test under standardised conditions may be appropriate. The laboratory and clinician may also consider whether macroprolactin or assay interference is affecting the reported value.

What Is Macroprolactin?

Macroprolactin is a larger molecular form in which prolactin is commonly bound to an antibody. It may be detected by routine prolactin assays but often has less biological activity than ordinary monomeric prolactin. Some patients with macroprolactinaemia have an elevated laboratory result without typical symptoms.

When the prolactin level is persistently elevated but the patient has few or no compatible symptoms, a doctor may request macroprolactin testing. This helps estimate the biologically active component and may reduce unnecessary investigation. Macroprolactin testing is different from a routine total prolactin test and is not required for every patient.

Prolactin and Pituitary Tumours

A prolactinoma is a pituitary adenoma that produces excessive prolactin. These tumours are usually noncancerous. Small tumours may primarily cause reproductive or sexual symptoms, while larger tumours can cause headaches, visual-field changes or reduced production of other pituitary hormones.

Diagnosis is not based on prolactin alone. If a pituitary lesion is suspected, evaluation may include detailed hormone testing, visual-field assessment and magnetic resonance imaging. In a patient with a large pituitary mass, the laboratory may perform sample dilution when necessary to address the high-dose hook effect, which can occasionally cause a falsely low or only modestly elevated result.

Related tests may include beta-hCG, TSH, free T4, LH, FSH, oestradiol, testosterone, kidney-function tests, liver-function tests, macroprolactin and other pituitary hormones. The correct tests depend on the patient's sex, symptoms, medical history and initial prolactin result.

Imaging should not be ordered solely because of a minor unexplained increase without clinical review. The doctor may first confirm the elevation, review medicines and exclude pregnancy, hypothyroidism and other common causes.

Who Should Interpret the Report?

A physician, endocrinologist, gynaecologist, fertility specialist or other qualified healthcare professional should interpret the report. The result must be considered with the patient's symptoms, medicines, pregnancy status and related investigations. Patients should not diagnose a pituitary tumour or change medication based only on the numerical result.

Important Test Limitations

The Prolactin test does not independently diagnose a prolactinoma, infertility, polycystic ovary syndrome, hypothyroidism or another pituitary condition. A normal value does not exclude every pituitary disorder, and an elevated value does not automatically mean a tumour is present.

Physiological variation, stress, medication effects, macroprolactin and analytical interference can influence the result. Repeat testing or specialised analysis may be required when the result is unexpected or inconsistent with the clinical findings.
