Preparation Before the Scan

No fasting is generally required for an Ultrasound Scrotum. Patients can usually eat, drink and take their regular medicines unless another examination scheduled for the same visit requires specific preparation.

Wear comfortable, loose-fitting clothing. You may be asked to change into a gown and remove clothing from the area being examined. Inform the radiology team about recent injury, previous surgery, known infection, infertility treatment and the location and duration of pain or swelling.

During the scan, the patient lies on the examination table. A towel or supporting material may be used to position the scrotum. A clear water-based gel is applied to the skin, and the radiologist or sonographer gently moves a handheld transducer over the area.

The gel helps sound waves travel between the transducer and the body. Mild pressure may be required to obtain clear images. Patients with acute pain or inflammation may experience temporary discomfort, so they should tell the examiner if the area is extremely tender.

Images of both testicles are usually obtained for comparison. Their size, position, internal appearance and blood flow may be evaluated. Varicoceles may be associated with discomfort, testicular size differences or fertility concerns.

Hydrocele: A hydrocele is a collection of fluid surrounding a testicle. Ultrasound can show the amount and appearance of the fluid and help determine whether another scrotal abnormality is present.

Epididymitis and orchitis: Epididymitis is inflammation of the epididymis, while orchitis affects the testicle. Ultrasound findings may include enlargement, altered echogenicity and increased blood flow. Urine, blood or microbiological tests may be needed to determine the cause.

Testicular torsion: Torsion can interrupt blood supply to the testicle. Doppler ultrasound may demonstrate reduced or absent blood flow, but clinical urgency remains essential. Sudden severe pain, nausea, vomiting or a high-positioned testicle requires immediate emergency assessment.

Epididymal cyst or spermatocele: These are commonly fluid-filled structures arising near the epididymis. Ultrasound can characterise their location and appearance and help distinguish them from solid abnormalities.

Testicular or scrotal mass: Ultrasound helps determine whether a lump is located within or outside the testicle and whether it appears solid or cystic. A suspicious intratesticular mass may require urgent urological evaluation and additional investigations.

Trauma: Following an injury, ultrasound may assess the integrity of the testicle, blood flow and the presence of bruising, bleeding or fluid accumulation. Significant trauma requires prompt clinical evaluation.

Undescended or incompletely descended testicle: Ultrasound may sometimes assist in locating a testicle within the inguinal region, although additional evaluation may be required if it cannot be visualised.

What the Results May Show

A report may describe the size, shape, position and internal echotexture of each testicle. It may comment on whether a focal lesion is present and whether blood flow appears symmetrical. The epididymides, scrotal wall, fluid collections and veins may also be documented.

Terms such as hypoechoic, hyperechoic, heterogeneous, cystic or solid describe the ultrasound appearance of tissue. These terms are imaging descriptions and should not be interpreted as a diagnosis without considering the complete report and clinical context.

Doppler findings may indicate normal, increased, reduced or absent blood flow. Increased flow can occur with inflammation, while reduced flow may raise concern about torsion or another vascular problem. Technical factors and the duration of symptoms can influence findings.

If an abnormality is identified, follow-up may include specialist consultation, repeat ultrasound, blood tests, urine tests, semen analysis, tumour markers, MRI or surgical assessment. The appropriate next step depends on the patient's age, symptoms and imaging findings.
