The Uric Acid blood test measures the concentration of uric acid in the blood. Uric acid is a natural waste product formed when the body breaks down purines, which are substances found in the body's cells and in certain foods and beverages. Most uric acid dissolves in the blood, passes through the kidneys and is removed from the body in urine.
When the body produces excessive uric acid or the kidneys cannot remove it efficiently, the concentration in the blood may increase. This condition is called hyperuricemia. Persistently elevated uric acid may contribute to the formation of needle-shaped urate crystals in and around the joints, potentially resulting in gout. Excess uric acid may also contribute to certain kidney stones. However, an elevated uric acid result alone does not confirm gout, and many people with hyperuricemia never develop symptoms.
The test may be requested for people experiencing sudden joint pain, swelling, redness or warmth, particularly when the big toe, ankle or knee is affected. It may also be used to monitor people receiving treatment for gout or certain cancer treatments that can rapidly increase uric acid. Results must be interpreted with symptoms, medical history, medicines, kidney function and other relevant investigations.

Preparation Before Test
A venous blood sample is collected, usually from a vein in the arm. Preparation requirements can differ between laboratories and according to other tests ordered at the same time. If fasting is advised, only plain water should generally be consumed during the instructed fasting period.

Who Should Take This Test?
The Uric Acid test may be appropriate for people with sudden or recurrent joint pain, redness, warmth and swelling suggestive of gout. People with recurrent kidney stones, kidney dysfunction or risk factors for uric acid accumulation may also require testing. Purines are broken down primarily in the liver, producing uric acid as an end product. Uric acid circulates in the blood and is removed mainly by the kidneys, with a smaller amount eliminated through the digestive system. Blood uric acid can rise when production increases, kidney excretion decreases or both processes occur together.
Hyperuricemia may occur with reduced kidney function, dehydration, obesity, metabolic disorders, psoriasis, rapid cell turnover, certain cancers, chemotherapy, alcohol consumption, a high-purine diet or medicines such as some diuretics. Genetic factors can also influence how the body produces and removes uric acid.
When uric acid remains above its saturation point, monosodium urate crystals may form. Crystal deposition in a joint can trigger intense inflammation and produce a gout attack. Long-standing crystal accumulation may lead to firm deposits called tophi around joints and soft tissues. Urate crystals can also contribute to kidney stone formation or kidney-related complications in susceptible patients.
A high blood uric acid result does not automatically mean that a person has gout.
